An acoustic neuroma (sometimes termed a neurolemmoma or schwannoma) is a benign (non-cancerous) tissue growth that arises on the eighth cranial nerve leading from the brain to the inner ear. This nerve has two distinct parts, one part associated with transmitting sound and the other sending balance information to the brain from the inner ear. These pathways, along with the facial nerve, lie adjacent to each other as they pass through a bony canal called the internal auditory canal. This canal is approximately 2 cm (0.8 inches) long and it is here that acoustic neuromas originate from the sheath surrounding the eighth nerve. The facial nerve provides motion of the muscles of facial expression.

Please see: A Survey of Neurosurgeons' Preferences: Radiosurgery, Resection or Observation for Patient's with Acoustic Neuromas The primary clinical issues include avoiding tumor-related or treatment-related mortality, prevention of further tumor-related neurologic disability, minimizing treatment risks such as spinal fluid leakage, infections, or cardiopulmonary complications, maintaining regional cranial nerve function (facial, trigeminal, cochlear, and glossopharyngeal/vagal), avoiding hydrocephalus (obstruction of spinal fluid), maintaining quality of life and employment, and reducing cost. All treatment choices should strive to meet all of these goals. Making the Diagnosis Most patients present with hearing loss or imbalance and have an audiogram performed. This may show that the cause of the hearing problem is due to nerve dysfunction. An imaging test is then obtained. In the decade before CT guidance, imaging studies to evaluate patients with acoustic neuromas included plain radiographs, air cisternograms, and later skull base tomography. Parenchymal imaging with computed tomography (CT) provided good visualization of the extracanalicular portion of the tumor, but small tumors in the internal auditory canal often could not be seen. With the widespread availability of magnetic resonance imaging (MRI), problems in diagnosis no longer should exist. We now have exquisite imaging of both the intracanalicular and extracanalicular components of the tumor using high resolution imaging. One millimeter slices can be obtained quickly to image the cranial nerves themselves and identify the tumor through uptake of contrast enhancement. For patients in whom an evaluation of an acoustic neuroma is proposed, a (MRI) should be performed. (Fig. 7)
