In August 1989, a previously healthy 4-year-old boy in Michigan was diagnosed with acrodynia, a rare manifestation of childhood mercury poisoning. Symptoms and signs included leg cramps; rash; itching; excessive perspiration; rapid heartbeat; intermittent low-grade fevers; irritability; marked personality change; insomnia; headaches; hypertension; swelling; redness and peeling of the hands, feet, and nose; weakness of the pectoral and pelvic girdles; and nerve dysfunction in the lower extremities. A urine mercury level of 65 ug/L was measured on a 24-hour urine collection. Treatment with intensive chelation therapy increased his urine mercury excretion 20-fold. Examination of his mother and two siblings found urine mercury levels greater than or approximately equal to his; his father had elevated, although lower, levels. Parents and siblings were asymptomatic, although electromyographic abnormalities were detected in one sibling. The Michigan Department of Public Health (MDPH) identified inhalation of mercury-containing vapors from phenylmercuric acetate contained in latex paint as the probable route of mercury exposure for the family; 17 gallons of paint had been applied to the inside of the family's home during the first week of July. Samples of the paint contained 930-955 ppm mercury; the Environmental Protection Agency (EPA) limit for mercury as a preservative in interior paint is 300 ppm. During July, the house was air-conditioned, and the windows were not opened.

Epidemiologic Notes and Reports Elemental Mercury Poisoning in a Household Ohio, 1989
On November 22, 1989, a 15-year-old male who had been hospitalized in Columbus, Ohio, was diagnosed with acrodynia, a form of mercury poisoning. This report describes the investigation by the Columbus Health Department (CHD) to determine the source of the patient's exposure to mercury. On November 29, the CHD investigated the apartment where the family had lived since August 26, 1989. Neighbors reported that the previous tenant had spilled a large jar of elemental mercury within the apartment. Although this tenant could not be located for confirmation, mercury vapor concentrations in seven rooms ranged from 50-400 ug/m3 (the Agency for Toxic Substances and Disease Registry's acceptable residential indoor air mercury concentration is less than or equal to 0.5 ug/m3 (2)). The apartment was sealed, pending decontamination efforts which are ongoing. In three other apartments in the same building, air mercury concentrations were less than the measuring instrument's detection limit of 10 ug/m3. The CHD did not detect evidence of mercury cross-contamination in a mobile home where the patients' family had relocated in November 1989. Although nonoccupational elemental mercury poisoning occurs less frequently than occupational mercury poisoning (3), cases of elemental mercury exposure and toxicity in children have been reported (3-9). Because mercury vapors are dense and tend to settle, children playing near the floor may be exposed to mercury if it is present (8). Moreover, children may be physiologically more susceptible to the health hazards of mercury exposure than adults.

2. SYMPTOMS
2.1 Symptoms in chronic inorganic Hg-intoxication.
The heavy, mostly occupational, chronic inorganic mercury intoxication has been described as having a triad of symptoms:
  • 1) Tremor (or more wide: neurological symptoms)
  • 2) Gingivitis / stomatisis (or more wide: local oral symptoms)
  • 3) Erethismus (or more wide: psychiatric symptoms)
Tremor and gingivitis are not always present and usually not in milder forms. Tremor can cause alteration of the handwriting. The tremor is usually an intentional tremor. Gingivitis is an inflammation of the gingiva (gum). The Erethismus (from Greek; excite) is a pathologically increased excitability . Erethism is usually the first symptom to develop. It goes along with some or many of the following symptoms;
  • irritability,
  • outbursts of temper,
  • stress intolerance,
  • decreased simultaneous capacity,
  • increased sensitivity to sounds and light,
  • resentment of criticism,
  • loss of self-confidence,
  • timidity,
  • excessive shyness

    The disease that we now know as poliomyelitis was not designated as such until about the middle of the 19th Century. Prior to that, it was designated by many different names at various times and in different localities. The simple designations, paralysis, palsy and apoplexy, were some of the earliest names applied to what is now called poliomyelitis. Paralysis, resulting from poisoning, has probably been known since the time of Hippocrates (460-437 B.C.), Boerhaave, Germany, (1765) stated: "We frequently find persons rendered paralytic by exposing themselves imprudently to quicksilver, dispersed into vapors by the fire, as gilders, chemists, miners, etc., and perhaps there are other poisons, which may produce the same disease, even externally applied." In 1824, Cooke, England, stated: "Among the exciting causes of the partial palsies we may reckon the poison of certain mineral substances, particularly of quick silver, arsenic, and lead. The fumes of these metals or the receptance of them in solution into the stomach, have often causes paralysis."
