The Holmes-Adie pupil is considered a variation of normal but is rarely the result of a lesion in the efferent parasympathetic pathway. The Holmes-Adie syndrome is the association of a Holmes-Adie pupil with absent deep tendon jerks. Clinical features include:

Adie's Pupil Options Adie's pupil, sometimes called the Holmes-Adie syndrome, is an unusual neurologic disorder in which the ability of the pupil to constrict is impaired, usually in one eye (although the second eye has a tendency to become involved eventually, usually at a later time, in about 20-30% of cases). It is generally associated with loss of some reflexes, such as the knee-jerk. It occurs mainly in middle age, and more commonly in women than in men. The cause is unknown, but it is thought to be a form of neuropathy, in which the nerves that control the pupils and the reflexes selectively degenerate. Some people think this is due to an attack by a virus, other think it is "auto-immune", meaning that the immune system makes antibodies that attack these specific nerves. There is a feeling that light is too bright in the affected eye (because the pupil helps to reduce light intensity by constricting in bright light). The pupil also helps to focus light in the eye, and sometimes the nerves that control the lens in the eye may also be involved, so that the vision from the affected eye is often blurry. However, the other eye is usually normal. Usually over time, the patient adapts to the large pupil (which generally is permanent but non-progressive) and does not notice it anymore.

A woman friend has been diagnosed as having 'Holmes-Adie Syndrome' following an attack of labyrithitis, first diagnosed some months ago.
Her neurologist said that having this syndrome was only 'of passing interest' and indeed, she has slowly regained most of her former health. Does anyone have information on Holmes-Adie syndrome? We'd like to know. Next Article

In 1932 Adie [1] described a syndrome in which unilateral accommodation and pupillary contraction were abnormal. lt typically was observed in young women with no history or evidence of syphilis and involved a unilateral dilated pupil that was unresponsive to light. He noted that deep tendon reflexes were absent or markedly diminished in these patients. He concluded that it was "a benign disorder sui generis". We report a recent case of Adie syndrome. Case Report
A 37-year-old woman came to the office with right pupil dilatation. lt was noticed by a nurse for the first time earlier that morning and had not been present previously according to the patient's report. The patient's only visual complaint was mild photophobia in sunlight. She also had complaints of sinus congestion, headaches, and intermittent mucopurulent rhinorrhea believed to be due to an exacerbation of seasonal allergies. She denied fever, extremity weakness, paresthesias, or a history of sexually transmitted diseases. The patient had a remote history (more than 10 years ago) of an episode of left arm and facial weakness, which resolved. On a physical examination she had anisocoria with a 5-mm right pupil and a 3-mm left pupil. Pupillary light response was absent on the right and decreased accommodation was present. Extraocular movements were intact bilaterally. Findings on a fundal examination were normal, and her blood pressure was 128/84 mmHg. No other abnormalities were noted. Magnetic resonance imaging (MRI) was ordered and findings were normal except for sinusitis.

The patient (usually a female) has anisocoria with the enlarged pupil failing to react to light. However, the pupil constricts in response to accommodation with delayed dilatation (this results in reversal of anisocoria).
Note: Adie's pupil is bilateral in 15% of cases and in long-standing cases the pupil may be miosed. Further examination:
  • mention you would like to examine the iris on the slit-lamp for segmental palsies and vermiform movement of the iris mention that you would like to test the tendon reflexes. This is absent in 70% of patient (Holme-Adie's syndrome)
Questions: 1. How can one confirm Adie's pupil?
Answer
The iris shows denervation hypersensitivity. A low concentration of pilocarpine for example 0.1% can cause pupil constriction in Adie's pupil but not in the normal. However, this test may not be confirmatory in acute cases. 2. How can you differentiate a dilated pupil from third nerve palsy from that caused by Adie's pupil or drug-induced?
