  • Long-term treatment with high doses of corticosteroids leads to the development of truncal obesity and focal fatty deposition. These deposits characteristically are located on the face, the nuchal and truncal areas, and episternally, as well as in the mediastinum and epicardium. We studied a patient with juxta-articular adiposis dolorosa who had L-tryptophan-associated eosinophilia-myalgia syndrome and was treated with high doses of prednisone. This is the first reported case of adiposis dolorosa occurring as a complication of corticosteroid treatment. Alterations of fat metabolism induced by corticosteroid excess may have played a role in the development of this unusual painful syndrome.
Adiposis dolorosa is an uncommon disorder of subcutaneous tissue that was first described by Dercum in 1892 at Jefferson Medical College, Philadelphia, Pa. The understanding of this disease has not progressed very much in the past 98 years. It remains better understood as a clinical entity than as a physiologic or metabolic process.

Background: First described in 1892 by the American neurologist Francis Xavier Dercum at Jefferson Medical College in Philadelphia, Pa, Dercum disease is an unusual progressive syndrome of unknown etiology characterized by multiple painful lipomas that arise in adult life, most often affecting postmenopausal women who are obese (Dercum, 1982). The onset is insidious, and the pain is out of proportion to the physical findings and is often described by patients as “all fat hurts.” The pain increases with the increase in fatty tissue and in connection to menstruation. Estrogen at menopause does not reduce the pain. Since the original description of this disease, the clinical spectrum has changed to include, in addition to the painful nodular fatty deposits, other components of adiposis dolorosa. General obesity; fatigability; weakness; and a wide variety of unexplained emotional disturbances, such as depression, confusion, and dementia, are reported. This observation is why Dercum disease has been proposed to be relabeled as Dercum syndrome (Palmer, 1981). Adiposis dolorosa is a disease that has been classified by the World Health Organization (WHO), and a paraphrase from the National Organization of Rare Diseases (NORD) says “Dercum Disease is a rare disorder in which there are fatty deposits which apply pressure to the nerves, resulting in weakness and pain. Various areas of the body may swell for no apparent reason. The swelling may disappear without treatment, leaving hardened tissue or pendulous skin folds.”

Overview Article This article, by Håkan Brorson and Birger Fagher, is a good description and overview of the disease. (please be patient - it's a long article with lots of pictures and takes time to load). Original article in Swedish. A Pain in the Groin - This article is of great interest because it is the story of an older woman with a history of Dercum's Disease who goes to her doctor with a pain in her groin. They end up doing a hysterectomy and other stuff, and the pain did not go away. Finally, after some months, as the pain got worse, the doctors were able to convince the women to go back into surgery (she was not keen on it, since the previous surgeries did not solve the problem) and they finally operated on her again and took out a sizable lipoma.
Maureen has talked to a lady who had a similar pain, and the doctor took out her appendix to relieve the pain. He took out the wrong thing. Again, it was a Dercum's lipoma.
