adiposis dolorosa Dercum's disease is the formation of multiple fatty deposits in various sites. This is a rare disease that predominantly affects women over men in a ratio of 30:1. It is rare for this condition to affect women before the menopause. This condition may be familial but the majority of cases are sporadic. It is characterized by the formation of circumscribed painful adipose tissue deposits in the subcutaneous tissues of the extremities and other parts of the body. The most common site for the deposits is the knees. The size of lesions varies from 0.5 to 5 cm. Patients are often obese. The disease may be associated with emotional instability, fatigue, weakness and, very occasionally, dementia. Histological examination reveals no abnormalities of the adipose tissue. There is no fat necrosis as seen in panniculitis. Giant cell formations may be seen. There is no satisfactory treatment. It has been reported that intravenous lidocaine brought relief in two cases.

adiposis dolorosa of Dercum Dercum's disease is the formation of multiple fatty deposits in various sites. This is a rare disease that predominantly affects women over men in a ratio of 30:1. If you have questions you think belong in a Frequently Asked Questions section, please send them to me, and I will start one. more information (editors only) Dercum's disease, Dercum disease, Adiposis Dolorosa, Morbus Dercum, Adiposita Dolorosa, Fettvävsreumatism, painful fatty tumors, lipomas, weakness, obesity. This category is about a disease that has been identified for over a hundred years, yet no recent research exists on the cause of it. It was originally named by Dr. Francis X. Dercum in 1892 as adiposis dolorosa. It has since picked up his name as an identifier and both names seem to be equally popular. It generally occurs in women (less 16% are men), between the ages of 25-40 (in Sweden) or 40-65 (in the USA). They cite obesity as a cause, but they have it backwards the disease actually causes the obesity and weight reduction efforts have little effect on the pain or the obesity. Two articles has attributed it to an autosomal dominant genetic disease, but there is no explanation why it can be benign in some and so debilitating in others.

Associated persons: James Meschter Anders Francis Xavier Dercum Unknown Vitaut Description: Rare condition characterized by scattered areas of painful cutaneous nodules or fat accumulations. Development of the nodular, tender lipomas is gradual with localization largely to forearms and thighs. It occurs chiefly in menopausal obese women of middle age and may be familial. Symptoms are asthenia, headache, frequently amenorrhoea and ecchymoses; diminution of sweating. Mental depression and deterioration have been observed in an appreciable number of these patients. Long progressive course. Death often caused by cardiac failure. Most cases of this rare disorder are sporadic but autosomal dominant inheritance has been documented. Dercum in 1888 first described the syndrome in a fifty-one year old woman and presented three more cases in 1892, writing:

Adiposis dolorosa Appelée également obésité douloureuse ou maladie de Dercum. Elle se caractérise par des dépôts graisseux à limite nette, douloureux, disposés sur les membres et le tronc. Ce type d’obésité est caractérisé par :
  • Fréquence plus importante pour les femmes que pour les hommes (30 pour 1) Problèmes psychiques (instabilité émotionnelle, démence) Asthénie (fatigue importante) des muscles Début des troubles le plus souvent après la ménopause Fréquence des cas familiaux (la transmission de cette maladie se ferait suivant un mode dominant – il suffit qu’1 des 2 parents soit porteur du gène défaillant pour que ses enfants développent la maladie) Une évolution défavorable.
Les zones les plus fréquemment atteintes sont les régions situées à proximité des articulations et plus particulièrement les genoux. La taille des dépôts graisseux varie de ½ cm à 6 cm. Cette maladie serait en rapport avec un dérèglement endocrinien (dérèglement hormonal), mais aucun moyen moderne d’exploration n’a permis d’apporter de preuves formelles. On rencontre parfois des troubles hormonaux (touchant la thyroïde ou l’hypophyse) associés à la maladie de Dercum.
