AEROMEDICAL CONCERNS
:Adjustment disorders are often associated with decreased concentration, depression, anxiety, impairment of occupational or social functioning, inattention, indecisiveness, fatigue, and insomnia, all of which are incompatible with aviation duties. This is one of the most common psychiatric diagnoses among aviators.
WAIVER
:Adjustment Disorder Temporarily NPQ for aviation until resolved. When fully resolved, the patient is PQ, no waiver required.
INFORMATION REQUIRED
: Upon return to an up status, FS must submit a brief summary of pertinent symptoms and treatment, including any mental health records or Medical Board reports if applicable, for review by NOMI.
TREATMENT
: Psychotherapy during the symptomatic period is not compatible with aviation duties.
FOLLOW-UP REQUIREMENTS
: Psychiatric follow-up is at the discretion of the mental health provider. Adjustment Disorders diagnosed by mental health personnel are not considered resolved until a mental health provider makes that statement in the patient's health record.

Definition
An adjustment disorder is a debilitating reaction, usually lasting less than six months, to a stressful event or situation. It is not the same thing as post-traumatic stress disorder (PTSD), which usually occurs in reaction to a life-threatening event and can be longer lasting.
Description
An adjustment disorder usually begins within three months of a stressful event, and ends within six months after the stressor stops. There are many different subtypes of adjustment disorders, including adjustment disorder with:
  • Depression Anxiety Mixed anxiety and depression Conduct disturbances Mixed disturbance of emotions and conduct Unspecified.
Adjustment disorders are very common and can affect anyone, regardless of gender, age, race, or lifestyle. By definition, an adjustment disorder is short-lived, unless a person is faced with a chronic recurring crisis (such as a child who is repeatedly abused). In such cases, the adjustment disorder may last more than six months.
An adjustment disorder occurs when a person can't cope with a stressful event and develops emotional or behavioral symptoms. The stressful event can be anything: it might be just one isolated incident, or a string of problems that wears the person down. The stress might be anything from a car accident or illness, to a divorce, or even a certain time of year (such as Christmas or summer).

Overview
Studies examining the prevalence of mental disorders in cancer [ ] show that most cancer patients do not meet the diagnostic criteria for any specific mental disorder; however, many patients do experience a variety of difficult emotional responses. [ ] In order to effectively match patient needs and treatment interventions, health care professionals must be able to distinguish the periodic difficulties that characterize normal adjustment from more serious mental disorders. Psychosocial distress exists on a continuum (Figure 1) ranging from normal adjustment issues through the Diagnostic and Statistical Manual of Mental Disorders (DSM-IV) Adjustment Disorders; [ ] to a level close to, but below, the threshold (i.e., meets some diagnostic criteria but not all) of diagnosable mental disorders; to syndromes that meet the full diagnostic criteria for a mental disorder (e.g., major depressive disorder). What is an adjustment disorder?
An adjustment disorder is defined as an emotional or behavioral reaction to an identifiable stressful event or change in a person's life that is considered maladaptive or somehow not an expected healthy response to the event or change. The reaction must occur within three months of the identified stressful event or change happening. The identifiable stressful event or change in the life of a child or adolescent may be a family move, parental divorce or separation, the loss of a pet, birth of a brother or sister, to name a few.
What causes adjustment disorders?
Adjustment disorders are a reaction to stress. There is not a single direct cause between the stressful event and the reaction. Children and adolescents vary in their temperament, past experiences, vulnerability, and coping skills. Their developmental stage and the capacity of their support system to meet their specific needs related to the stress are factors that may contribute to their reaction to a particular stress. Stressors also vary in duration, intensity, and effect. No evidence is available to suggest a specific biological factor that causes adjustment disorders.

Adjustment Disorders are reactions to stress that have lasted less than six months. The types of reactions can be very different, sadness, anxiousness, anger, or a combination of all. Adjustment Disorders become apparent within three months of some stress in a child's life. During such times, it is common for a child's schoolwork to suffer. Again, if a child has always (or for a year or more) shown the symptoms and had poor school achievement, the problem is not an Adjustment Disorder. Parents' divorce, a death or serious illness in the family, or being in an automobile accident are all typical of the stressful events which cause Adjustment Disorders in children. A child who has experienced such stress may appear normal for a month or so, then change. The difficulty with such reactions is that just when everyone else is ready to forget the painful event, this child begins to show that he or she is still in turmoil about it.
What To Do
Usually, Adjustment Disorders are solved by good communication. Sometimes the family can provide this healing talk. Talk to the child about their feelings about the death, or divorce, or whatever. Follow the principles in HELP for Parents, Active Listening , in talking with the child.
