Packard-based Physician Named Director of Cardiac Surgery for Children's Hospital Oakland
Electrophysiology and Arrhythmia Services
Electrophysiology (EP) is a subspecialty of cardiology that examines the electrical behavior of the heart by recording electrical activity from within the heart chambers. One catheterization laboratory procedure is called electrophysiology (EPS) study, which specialized cardiologists use to diagnose or treat a rhythm problem of the heart. Doctors thread specialized catheters through blood vessels in the patient's leg or neck, up to pace the heart and to measure and record the electrical activity of the heart. When doctors detect abnormal electrical pathways, which can cause dangerous heart rhythm abnormalities, they can do another procedure called radiofrequency (RF) ablation to cure the problem. During RF ablation, the doctors use a special mapping catheter, which is positioned precisely, to deliver radio frequency energy to a very tiny area of the heart, destroying the abnormal pathway or tissue. The Pediatric Arrhythmia Center at Stanford and UCSF is directed by Dr. George Van Hare. It is based at both Packard Children's Hospital and UCSF Medical Center, and includes faculty and staff at both facilities. Dr. George Van Hare performed the first pediatric radiofrequency catheter ablation in the United States.

Arrhythmia is an abnormal rhythm or rate of heart beat and is caused by a disturbance in the electrical nerve impulses of the heart. Tachycardia is where the rate is faster than 100 beats per minute, and bradycardias is where the rate is slower than 60 beats per minute. In atrial flutter, the beat rate is 200 to 400 bests per minute, but not every impulse reaches the ventricles. A totally uncoordinated beating of the heart at about 300 to 500 beats per minute is called atrial fibrillation.
The most common cause of arrhythmia is arteriosclerosis where deposits narrow the blood vessels and inadequate blood supply reaches the heart muscle. Arrhythmias occur more frequently after a heart attack. Caffeine and other drugs can also cause tachycardia. and Certain antidepressant drugs can cause serious cardiac arrhythmias if taken in high doses.

What is an arrhythmia? Normally the heart of an adult contracts between 60 and 100 beats per minute. An arrhythmia is a change in the regular beat of the heart. Your heart may beat too fast (a condition called tachycardia), too slow (a condition called bradycardia), or irregularly (often called fibrillation or flutter). It can occur in one of the two upper chambers of the heart the atria or in one of the lower two chambers, the ventricles. Arrhythmias may occur at any age, and are most often a nuisance rather than a serious problem. Arrhythmias are disorders of your heart's electrical system. Normally each heartbeat begins in the right atrium. A specialized group of cells in this chamber the sinus node acts as your heart's natural pacemaker by sending out an electrical impulse. This impulse spreads throughout the atria to an area between the two atria called the atrioventricular (AV) node.

Your heart beats about 100,000 times a day. Most of the time it never misses a beat, but sometimes it does. When your heart speeds up or slows down for no clear reason, you have an arrhythmia, meaning "without rhythm." Almost everyone has brief moments of uneven heartbeats. During a typical day, in fact, one healthy adult in five is likely to have an arrhythmia of some sort, usually a feeling that the heart is racing or fluttering. Usually, there's nothing to worry about uneven heartbeats for a few seconds are harmless, unless the person starts to worry a lot about them. Understanding what's going on so you can stay calm is a crucial first step. But some frequent or prolonged changes in the heart's steady beat make it hard for your heart to do its job. In the worst cases, arrhythmias can lead to cardiac arrest, when the heart stops working. More than 4 million people in the United States have some form of arrhythmia. About one in a hundred dies from an arrhythmia each year. The good news: You can take steps to prevent arrhythmias by dealing with the problems that cause them. That means making smart choices about such things as your diet and exercise habits. If you have arrhythmias, you and your doctor can do a lot to treat them and keep them from coming back.

MIT-BIH Arrhythmia Database
Since 1975, our laboratories at Boston's Beth Israel Hospital (now the Beth Israel Deaconess Medical Center) and at MIT have supported our own research into arrhythmia analysis and related subjects. One of the first major products of that effort was the MIT-BIH Arrhythmia Database, which we completed and began distributing in 1980. The database was the first generally available set of standard test material for evaluation of arrhythmia detectors, and has been used for that purpose as well as for basic research into cardiac dynamics at about 500 sites worldwide. Originally, we distributed the database on 9-track half-inch digital tape at 800 and 1600 bpi, and on quarter-inch IRIG-format FM analog tape. In August, 1989, we produced a CD-ROM version of the database; since that time, we have supplied it on CD-ROM only. The MIT-BIH Arrhythmia Database contains 48 half-hour excerpts of two-channel ambulatory ECG recordings, obtained from 47 subjects studied by the BIH Arrhythmia Laboratory between 1975 and 1979. Twenty-three recordings were chosen at random from a set of 4000 24-hour ambulatory ECG recordings collected from a mixed population of inpatients (about 60%) and outpatients (about 40%) at Boston's Beth Israel Hospital; the remaining 25 recordings were selected from the same set to include less common but clinically significant arrhythmias that would not be well-represented in a small random sample.

An arrhythmia is an abnormal heartbeat. The heartbeat is controlled by an electrical signal that lets the heart muscle know when to contract and relax. The expanding and contracting heart brings blood into and out of the heart, thereby distributing the blood to other areas of the body. The electrical signal that triggers the heartbeat is usually generated on the right side of the heart. Thus the right side of the heart is called the "pacemaker," as it controls the pace at which the heart beats.
