I. History
-Adolph Wilhelm Otto wrote first description of arthrogryposis in 1841. II. Definition: -Presence of multiple joint contractures at birth (Avenues) -“Classic” case; hands, wrists, elbow, shoulders, hips, feet, and knees (Avenues) -Most severe: nearly every joint may be involved including jaw and back (Avenues) -Non-progressive -Not only bilateral clubfeet and dislocated hips ( www.neuro.wustl.ed -Muscle may be missing -Normal cognitive abilities -Curved fixation of a joint in a flexed or contracted position; may be due to adhesions in or around the joint -Multiplex congenita: congenital generalized fixation of joints, may be due to a variety of changes in the spinal cord, muscles or connective tissue (Taber’s Encyclopedia)

Arthrogryposis is not actually a disease, but a description of the symptoms that occur about 1 in 10,000 births. The simplest definition would be strong or stiff joints. Through lack of movement while in the womb, the babies muscles do not develop properly and the bones likewise fail to develop, often they are small and brittle. The lack of movement also causes the joints to be very stiff, and the pressure of the womb bearing down on these precious children often causes the limbs to be distorted in appearance- the most common being club feet, dislocated hips, and cupped hands. There are some 350 different kinds of Arthrogryposis, which can make the specific type a very rare occurrence. This in turn, leads to difficulty in diagnosing the condition and treating it appropriately. http://www.sonnet.com/awnues

Zrenner Background: Arthrogryposis multiplex congenita is a nonprogressive condition, which is characterized by congenital contracture of several joints on different body areas and may also occur as a manifestation of other syndromes. In such syndromes an ophthalmologic manifestation is described as blepharophimosis in Marden-Walker syndrome. Other ophthalmological findings are described in a few case reports (ophthalmoplegia, optic disc dysplasia, pathological changes of the retinal vessels, pigmentepitheliopathy and abnormal electroretinogram (ERG)). Patient/Results: A 12-year-old girl with distal arthrogryposis multiplex congenita presented with progressive visual loss since one year. Visual acuity was 20/40 in OD and 20/25 in OS. Visual fields were concentrically constricted. Funduscopy revealed an atrophic retinal pigment epithelium on the whole fundus with vital discs. In the scotopic electroretinogram amplitudes were dramatically decreased or absent; cone signals were delayed. The multifocal ERG revealed pathologically reduced amplitudes as well in the macular region as in the periphery. Discussion: The girl demonstrated arthrogryposis and retinopathy, other syndromal manifestations or cerebral retardation could not be seen. Here we present an unusual case of what seems to be a new arthrogryposis syndrome.

Background and Diagnosis
Arthrogryposis multiplex congenita is a nonprogressive disease that consists of two or more different joint contractures present at birth. There are more than 150 different conditions that are classified under the term arthrogryposis. Causes of arthrogryposis may include abnormalities of muscle, nerve, connective tissue, or limitation of movement in utero. The majority of this tutorial will focus on the most common form of arthrogryposis, amyoplasia. Other disorders in the arthrogrypotic syndromes include Larson’s syndrome, distal arthrogryposis syndrome, and multiple pterygium syndrome. In 1841, Adolf Wilhelm Otto named the disease congenital myodystrophy. He described the patient as a "human wonder with curved limbs." In 1923, Stern coined the term arthrogryposis multiplex congenita. In 1983, Hall and colleagues named the most common type of the disease amyoplasia. Amyoplasia consists of multiple joint contractures, which are symmetrical in positioning of the limbs. The etiology does not have a genetic basis but is sporadic in nature. There have been reports of epidemics in the 1960s in areas of eastern Europe and Great Britain. There seems to be an association with a decrease of anterior horn cells and other neural elements of the spinal cord. There is also a questionable relationship to oligohydramnios.

These defects result in a severe limitation in range-of-motion in the joints. The condition is probably of diverse genetic etiology that results from changes in the spinal cord, muscles, or connective tissue.
