Atrial fibrillation and complete heart block
  • Fibrillary waves of atrial fibrillation and no P waves.
  • Regular ventricular rhythm
  • The wider the QRS of the ventricular escape rhythm the less reliable the escape mechanism.
  • AF with complete heart block can be easily missed and is an indication for a permanent pacemaker.

http://www.askaheartsurgeon.com/irregularaf.htm
Atrial Fibrillation: What is it? What does atrial fibrillation look like on an EKG? Symptoms of Atrial fibrillation Prevention Atrial fibrillation due to coronary artery disease can be prevented by modifying the appropriate cardiac risk factors: eating a low-fat diet; controlling cholesterol and hypertension; not smoking; controlling weight; and getting regular exercise. Unfortunately, some causes of atrial fibrillation are not preventable. Treatment Prognosis When an underlying cause of atrial fibrillation is identified and treated, the arrhythmia frequently resolves. Achieving a normal heartbeat is less likely for individuals who have long-standing rheumatic heart disease or any condition in which the atria are enlarged.

Intracardiac Atrial Fibrillation Database
This database consists of endocardial recordings from the right atria of 8 patients in atrial fibrillation or flutter. A decapolar catheter with 2-5-2mm spacing (7mm spacing between bipoles) was placed in four separate regions of the heart. In each region, the 5 bipolar signals were recorded along with 3 surface ECG leads. Data were digitized at 1kHz. Catheter bipoles are labeled (distal to proximal) CS12, CS34, ..., CS90. Regions are:
    SVC
The distal tip of the catheter (CS12) is close to the annulus of the superior vena cava
    IVC
The proximal tip of the catheter (CS90) is close to the annulus of the inferior vena cava
    TVA
The distal tip (CS12) is close to the tricuspid valve annulus
    AFW
The entire catheter rests against the atrial free wall

While jogging at Camp David on a Saturday afternoon (May 4, 1991), Bush developed shortness of breath, chest tightness, and a general feeling of fatigue. A White House physician discovered Bush had a rapid irregular heartbeat, ultimately diagnosed as atrial fibrillation due to hyperthyroidism (see below). [ Bush was transferred to Bethesda Naval Hospital by helicopter. Cardiologist Allen M. Ross prescribed digitalis, procainamide, and Coumadin [ Note: Digitalis slows the heart, procainamide can change the rhythm pattern from atrial fibrillation back to normal, and Coumadin prevents blood clots, one of the main complications of atrial fibrillation.) Abrams writes: According to the doctors' plan, if the drugs failed to affect the arrhythmia as initially they did an electrical shock would be administered the following day, a common way of returning a patient's rhythm to normal. When it was announced that Dan Quayle would be acting president under the Twenty-fifth Amendment provisions while Bush was under anesthesia for the cardioversion if it was required a different kind of shock reverberated across the nation. The prospect of Quayle as president brought home sharply the electorate's lack of confidence in his ability to lead. [ The drugs were effective. By 10:25 pm on May 5, Bush's heart rhythm was normal. About 5 am the following morning, however, atrial fibrillation recurred. It was decided (by whom?) to continue the drugs rather than use electrical cardioversion. Bush returned to the White House later that day. [

    Epidemiology
Atrial fibrillation (AF) is the most common arrhythmia physicians face in clinical practice, accounting for about one-third of hospitalizations for arrhythmia. The prevalence of AF is 0.5 percent in those 50 to 59 years old and 8.8 percent in those 80 to 89 years old, and the incidence ranges from 0.2 percent per year in men 30 to 39 years old to 2.3 percent per year in men 80 to 89 years old. The chronic cardiac conditions most commonly associated with the development of AF are:
    • Rheumatic mitral valve disease.

Pre-publication history Background Atrial fibrillation is a common serious cardiac arrhythmia. Knowing the prevalence of atrial fibrillation and documentation of medical management are important in the provision of primary care. This study sought to determine the prevalence of atrial fibrillation in a primary care population and to identify and quantify the treatments being used for stroke prevention in this group of patients. Methods A prevalence study through chart audit was conducted in the family medicine practice at the Sunnybrook campus of the Sunnybrook and Women's College Health Sciences Centre. The main outcome measures were the prevalence of atrial fibrillation in our primary care practice and the use of warfarin for stroke prevention in this population.

Blood-Thinning Drug's Benefit in Stroke Prevention
Using a blood-thinning drug in elderly people with an irregular heartbeat may provide stroke-preventing benefits that had previously been found only in younger people, according to a report in the April 7, 2000, issue of Stroke: Journal of the American Heart Association Atrial fibrillation is an irregular heart rhythm, or arrhythmia, affecting about two million Americans, according to the American Heart Association. The condition leads to nearly 90,000 strokes or mini-strokes (transient ischemic attacks) each year. One way to treat atrial fibrillation is with a clot-inhibiting drug called warfarin. Elderly people are, by virtue of their age, at greater risk of stroke. Additionally, warfarin must be used cautiously in those over 80 years of age because of the increased risk of internal bleeding, one of the major potential side effects of using the drug.
