All those with bipolar disorder experience mood swings that alternate from periods of severe highs (mania) to severe lows (depression). However, while these abnormally intense moods usually last for weeks or months in adults with the illness, children with bipolar disorder can experience such rapid mood swings that they commonly cycle many times within a day. Symptoms in young children can present differently than in adults and are frequently mistaken for those of attention deficit/hyperactivity disorder (ADHD); therefore, the bipolar diagnosis is often missed. Children tend to have less discreet episodes of mania and depression than adolescents or adults. Often these children are irritable, explosive and excessively active. Some children show more typical features of mania, including grandiosity, excessive talking, high energy, difficulty sleeping and impulsive behavior such as precocious sexuality. Teens with bipolar disorder are more likely than younger children to show distinct episodes of mania and depression. Their grandiosity and impulsivity during mania may result in social and academic problems as well as substance abuse. Some diagnosed with conduct disorder or substance abuse may have unrecognized, underlying bipolar disorder. Similarly, some adolescents diagnosed with schizophrenia because of psychotic thoughts may have bipolar disorder with psychotic features. A psychiatrist must thoroughly evaluate a child or adolescent to make a diagnosis of bipolar disorder. The evaluation generally includes interviewing the child, a physical examination, blood tests to ensure no underlying medical problem exists, and review of school records. Parents can further describe the problems and medical and family history. Often, a family history of depression or bipolar disorder can be a clue to diagnosis. Bipolar disorder tends to be inherited. Children with immediate relatives who have mood disorders are more likely to have bipolar disorder.
