Watkins, M.D. There is a great deal of controversy about the diagnosis of Bipolar Disorder in Childhood. Sometimes it can be difficult to differentiate AD/HD, irritable depression and Bipolar Disorder in children. Books such as The Bipolar Child have received popular attention and raised public consciousness about difficult to treat children with explosiveness and rapid mood swings. While the book gives excellent descriptions of individual children, it is not a research-based document. We have seen an increase in the use of antipsychotics (such as risperidone and haloperidol )and mood stabilizers (such as valproate and lithium) in younger children.

Former U.S. Surgeon General An estimated 2.3 million Americans have bipolar disorder, also called manic-depressive illness. A person with bipolar disorder can go from feeling very, very high (called mania ) to feeling very, very low (depression).  With proper treatment, people can control these mood swings and lead fulfilling lives. While the rate of bipolar disorder is the same among African Americans as it is among other Americans, African Americans are less likely to receive a diagnosis and, therefore, treatment for this illness. Most African Americans with bipolar disorder are going undiagnosed and untreated. Several factors have contributed to African Americans not receiving help for bipolar disorder and other mental illnesses.  Some of the reasons are:
  • A mistrust of health professionals, based in part on historically higher-than-average institutionalization of African Americans with mental illness; and on previous mistreatments, like such tragic events as the Tuskegee syphilis study.

    Bipolar Affective Disorder
    is also known as Manic Depressive Disorder or Manic Depression, is a brain disorder that causes unusual shifts in a person's mood, energy, and ability to function. Bipolar Disorder causes ups and downs that are different from the normal ups and downs that most people go through, the effects of Bipolar Disorder are severe. The effects of Bipolar Disorder can result in damaged relationships, poor school or job performance, even suicide. There is good news though, Bipolar Disorder can be treated effectively with medication and therapy, and people with this illness can lead full and productive lives. Bipolar Affective Disorder affects about 2.2 million American adults 18 years of age or older during any given year. It typically shows up in late adolescence and early adulthood. Bipolar Affective Disorder can also show up in early childhood or later in life. Like diabetes or heart disease, bipolar disorder is a long-term illness that must be carefully managed throughout a person's life.
    The Course of Bipolar Disorder
    Episodes of mania and depression typically recur throughout a person's lifespan. Between the episodes, most people with Bipolar Disorder are free of symptoms. However, about one-third of the people have some residual symptoms, and about one-percent experience chronic unremitting symptoms despite treatment. People with bipolar disorder can lead healthy and productive lives when the illness is effectively treated. Without treatment, however, the natural course of bipolar disorder tends to worsen. Over time a person may suffer more frequent (more rapid-cycling) and more severe manic and depressive episodes than those experienced when the illness first appeared. But in most cases, proper treatment can help reduce the frequency and severity of episodes and can help people with bipolar disorder maintain good quality of life.

    A patient information handout on bipolar disorder, written by the authors of this article, is provided on page 1357. This article exemplifies the AAFP 2000 Annual Clinical Focus on mental health. Bipolar disorder most commonly is diagnosed in persons between 18 and 24 years of age. The clinical presentations of this disorder are broad and include mania, hypomania and psychosis. Frequently associated comorbid conditions include substance abuse and anxiety disorders. Patients with acute mania must be evaluated urgently. Effective mood stabilizers include lithium, valproic acid and carbamazepine. A comprehensive management program, including collaboration between the patient's family physician and psychiatrist, should be implemented to optimize medical care. (Am Fam Physician 2000;62:1343-53, 1357-8.) B ipolar disorder is characterized by variations in mood, from elation and/or irritability to depression. This disorder can cause major disruptions in family, social and occupational life. Bipolar I disorder is defined as episodes of full mania alternating with episodes of major depression. Patients with mania often exhibit disregard for danger and engage in high-risk behaviors such as promiscuous sexual activity, increased spending, violence, substance abuse and driving while intoxicated.

    What is bipolar disorder? Bipolar disorder is an illness that causes extreme mood swings. This condition is also called manic-depressive illness. It may be caused by a chemical imbalance in the brain. Bipolar disorder sometimes runs in the family. If you have a parent with bipolar disorder, you have more of a chance of getting it too. Both men and women get bipolar disorder. All age groups can get it. What are some of the signs of bipolar disorder? You may feel very happy, full of energy, and able to do anything. You might not even want to rest when you feel this way. This feeling is called mania (say: may-knee-ah). Later on, you may feel very sad and depressed. You may not want to do anything when you feel this way. This is called depression. Bipolar disorder has many other signs:

    Bipolar disorder, often called manic depression, is one of the major mood disorders and is distinguished by behavior that fluctuates from extreme highs (mania) to serious lows (depression) interspersed with periods of "normal" mood. Simply put, a person who suffers from the condition is unable to keep a sense of calm about his or her life.
    Mania is a term used a lot these days (like "basketball mania"), but in medicine, mania refers to behavior that includes profuse and rapidly changing ideas, exaggerated sexuality, extreme gaiety, intense irritability, and decreased sleep. Depression is used frequently to describe a feeling of sadness. With bipolar disorder, however, the lows of depression are characterized by extreme hopelessness and a feeling of worthlessness accompanied by thoughts of suicide.
