Background Blastomycosis may be a benign and self-limiting infection or a chronic granulomatous and suppurative mycosis in which the primary infection is initiated in the lungs with frequent, subsequent dissemination to other body sites, especially the skin and bone. The disease is most prevalent in males 40-60 years of age and in children. Blastomycosis may coexist with bronchogenic carcinoma, histoplasmosis, severe pulmonary disease, or tuberculosis. Rev Infect Dis 1991 Sep-Oct;13(5):1005-8 Abstract quote Four cases of blastomycosis seen in two acute care hospitals in Harare, Zimbabwe, are described. All patients had symptoms of at least 2 months' duration before presentation, and all had radiographic evidence of pulmonary consolidation. Three patients had confirmed bone involvement, and two had chronic discharging sinuses.

Blastomycosis can be beaten
By Elsie Neufeld By Shauna DeGagné Miner and News Staff Four years ago, local resident Nicole Theriault was sick. She knew something was seriously wrong with her, but doctors couldn't figure it out. Large cysts were appearing under her skin, making daily living very uncomfortable. "It felt like someone was squeezing everything inside of me," said Theriault, a mother of a young daughter. "It felt like arthritis." After two years, Theriault found a cyst in her breast while doing a self-examination. Thinking it was breast cancer, she had surgery to get it removed. It turned out to be non-cancerous. She also had a cyst the size of a softball removed from her elbow. Because Theriault didn't have a family doctor, it was hard to keep track of what was going on but finally, after a trip to the emergency room because her elbow wasn't healing properly, the mystery was solved.

Blastomycosis Blastomycosis is a systemic infection caused by Blastomyces dermatitidis Spores enter the patient via the respiratory tract and a primary infection establishes itself in the lungs. Solitary cutaneous lesions are also possible with this organism, however it is more common for cutaneous lesions to be linked to a systemic infection. It is possible to miss the organism in the cytology, such that results from other complementary tests may be required for a diagnosis, such as serology and culture. Three yeasts of Blastomyces sp. can be seen in the microphotograph below on the left in a background of neutrophils and erythrocytes; the outline of these organisms have been "cut out" and pasted to the right of the microphotograph to eliminate background for a better appreciation of detail of the microorganism. Tom Volk's Fungus of the Month for January 2001
This month's fungus is Blastomyces dermatitidis , cause of blastomycosis, also known as North American Blastomycosis, Gilchrist's disease, Chicago disease, or Namekagon River Fever
For the rest of my pages on fungi, please click http://TomVolkFungi.net It's the beginning of the spring semester, and I'll again be teaching medical mycology, the study of fungi that infect humans and other animals. This month's fungus is one of the nasty ones that can cause a severe lung infection in humans. It is also especially common in some areas as a disease of dogs. Blastomyces dermatitidis is one of the dimorphic fungi, which means it has two forms. At room temperature is takes the mycelial form shown below, forming a single conidium that looks kind of like a lollipop. At 37 o C, the mycelium transforms to a large yeast (8-15 um) with characteristic thick cell walls and broad-based buds (shown to the left). In this form, and with the typical lactophenol cotton blue stain, it is known as the Big Blue Broad Based Budding yeast - Blasto Each yeast is multinucleate with 2-5 nuclei per cell. The mechanism for the phase transition is unknown, but the reason for the transition is quite clear, as described below. . The fungus can also form a sexual state; the so-called teleomorph form is

Description
Causes and symptoms Diagnosis ... Resources Definition Blastomycosis is an infection caused by inhaling microscopic particles (spores) produced by the fungus Blastomyces dermatitidis . Blastomycosis may be limited to the lungs or also involve the skin and bones. In its most severe form, the infection can spread throughout the body and involve many organ systems (systemic). Description Blastomycosis is a fungal infection caused by Blastomyces dermatitidis . Although primarily an airborne disease, farmers and gardeners may become infected from contact with spores in the soil through cuts and scrapes. The fungus that causes the disease is found in moist soil and wood in the southeastern United States, the Mississippi River valley, southern Canada, and Central America. Blastomycosis is also called Gilchrist's disease, Chicago disease, or North American blastomycosis. Another South and Central American disease, paracoccidioidomycosis, is sometimes called South American blastomycosis, but despite the similar name, this disease is substantially different from North American blastomycosis. Canine blastomycosis, a common dog disease, is caused by the same fungus that infects humans. However, people do not get this disease from their dogs except only very rarely through dog bites. Blastomycosis is a rare disease infecting only about 4 in every 100,000 people. It is at least six times more common in men than in women and tends to more often infect children and individuals in the 30-50 year old age group. People who have diabetes mellitus or who are taking drugs that suppress the immune system (immunocompromised) are more likely to develop blastomycosis. Although people with AIDS can get blastomycosis because of their weakened immune system, blastomycosis has not been one of the more common fungal infections associated with AIDS.
