Buerger's disease
Buerger's disease is a progressive inflammatory disorder of the blood vessels, mainly in the legs. Its cause is unknown, but it is aggravated by smoking tobacco and seldom seen in people who have never smoked. It causes severe pain, can be seriously disabling and can lead to gangrene requiring amputations. No antibiotics have been found effective for treating the inflammation of the walls of arteries and veins. Anticoagulants are not considered helpful for reducing thrombus formation in this disease. Vascular surgery is not practicable, because most of the blood vessels involved are too small. Onset of symptoms may be gradual or sudden. Coldness, numbness, tingling or burning pain in the feet and legs are usually noticed first. The feet may become reddish-purple colored in a dependent position but very pale when elevated above the level of the heart. Walking may become extremely painful. Exposure to cold temperatures makes the symptoms worse, and most people with this disease feel better in a warm climate. Any slight injury to the feet or legs tends to become infected and enlarged. If gangrene develops, amputation is usually required.

Poor circulation occurs when the flow of blood through a part of the body is impaired. It can occur because of a gradual narrowing of an artery which may be associated with diabetes mellitus, raised blood pressure, raised blood levels of cholesterol and smoking. Blood clots can dramatically block off the circulation of blood in an artery. Sudden narrowing or spasm of blood vessels which interferes with circulation can be caused by certain drugs, exposure to cold and smoking. The commonest of poor circulation is atherosclerosis . This narrowing of the arteries is aggravated by smoking, the level of blood cholesterol and by high blood pressure. In the past the usual approach to poor circulation in the legs was to get people to stop smoking and then to operate on their arteries. It is possible to put plastic tube with a balloon attached into the narrowed artery, stretch it and then remove the tube and the balloon - this is called an angioplasty There is also increasing evidence that atherosclerosis is reversible. If in addition to stopping smoking, blood pressure is controlled and blood cholesterol lowered to 5.5 MMOL/L

Tribal population threatened by crippling disease
By Our Staff Reporter ATTAPPADY (Palakkad), JAN. 16. The flow of spurious liquor and the widespread cultivation of ganja (Cannabis sativa) in the forests of Attappady, the first `total liquor-prohibited zone' of the State, have resulted in the spread of `Thrombo Angiitis Obliterans' (TAO), a deadly maiming disease among the tribals. A number of tribals have succumbed to the killer disease. Many have lost their legs. An unofficial survey found more than 200 people affected by the disease and the legs of many have been amputated. Some affected tribals are recuperating in various hospitals in Palakkad and in the Medical Colleges of Thrissur and Kozhikode. The TAO, commonly known as Buergers disease, is the outcome of the large-scale use of narcotic substances and excessive smoking. Even before the arrack ban came into being in the State in 1996, the Attappady tribal belt was declared as a `total liquor-prohibited zone'. However, the fact is that the Attappady Hills are floating in spurious liquor. Earlier, liquor was sold mostly in the arrack and toddy shops but now it is available in every nook and cranny of Attappady. The State Government totally failed to stop the manufacture and sale of spurious liquor and the large-scale cultivation of ganja in the Attappady Hills.

The importance of proper circulation
Circulatory disorders are quite common in middle-aged and elderly folk. Hypertension is one of them. It is caused by cholesterol plaque deposits along the walls of the arteries, making them harden and constrict. Because the arteries are constricted, the blood exerts great force against the walls of the blood vessels, causing the blood pressure to rise.
Hardening of the arteries is another consequence because the arteries narrow due to these same fatty deposits. Buergers disease, common to those who smoke, is a chronic inflammation of the veins and arteries in the lower extremities. Raynauds disease is marked by constriction and spasm of the blood vessels in the extremities. The fingers toes and tip of the nose. This disease if left untreated can lead to gangrene.

    This diary is a little different than most on this site as I am not mad at anyone or anything, just a little disappointed in a few people within the system. I just feel it might help others in some way or another. First off... I am a 45 year old male in very good health and have not needed to see the doctor for more than a few times in the last 20 years. About 4 weeks ago I began to develop some redness and blisters on my three outside toes of my right foot. Being a typical male and thinking it was just a severe case of some type of athletes foot I treated it myself with over the counter creams. As you have probably guessed... it didn't go away. Just seemed to get worse. As the toes got redder and blacker and the itch became unbearable my male brain finally began to realise that something was wrong and maybe I should consult a doctor. When my doctor finally got close enough to take a look, he immediately concluded it wasn't athletes foot, that it even looked gangrenous and possibly a clot in my foot somewhere. Well I of course in my mind thanked him for his considerate bedside manner. It did look pretty bad though. Without delay he referred me to a vascular surgeon. Like most, you are probably thinking... hmmmm... 45... male... must be high cholesterol. Sorry to disappoint you but I have an ldl cholesterol level of 2.00, I am not over weight and do not have high blood pressure, so that conclusion is highly unlikely.

    Patient assessment - examination On inspection of the leg, in addition to any ulcers or gangrene, there may be loss of hair, thickening of toenails and thinning of the skin. The foot becomes very pale on elevation and red when put on the ground. This is called Buerger's sign and is shown by the foot on the left in the picture below: Severe disease may present with gangrene: Despite the red appearance, the foot will be cold to the touch. The pulses in the leg will be felt for:
    • Femoral - in the groin Popliteal - behind the knee Dorsalis pedis - in the foot (this may be missing in 10% of the normal population) Posterior tibial - behind the inner ankle
    The distribution of missing pulses will give an indication of the part of the arterial tree of the leg that is diseased. To give a more accurate picture the blood pressure at the ankle is measured using a hand-held doppler and a blood pressure cuff: The highest pressure at the ankle is compared to the highest arm pressure to give an index, the Ankle Brachial Pressure Index (ABPI). This is used to give an estimate of disease severity. In some cases there may be areas of gangrene but with a full set of pulses. This indicates disease in the very small blood vessels supplying the skin - these may become blocked by very small blood clots, inflammation in the arteries (vasculitis), Buergers disease or severe spasm (frostbite and Raynaud's syndrome).
