Till today the pathologic mechanisms of the disease are not completely known. Buerger’s disease has to be differentiated from atherosclerotic peripheral obliterate disease. The diagnosis Buergers’ disease is made if certain criteria are fulfilled and no other reason initiating the vascular occlusion can be proved. Although there are some specific histological findings a histologic investigation is not required for the diagnosis. A lot of smaller studies have investigated possible pathogenic mechanisms. An infection with Reckettsia was discussed as well as allergic, immunologic, hematologic or hereditary disorders. Psychosomatic conflicts were hypothesised to trigger the onset of Buerger’s disease. Most of this study results were never repeated or independently proved in larger populations. Effective surgical or pharmaceutical therapeutic options are not available yet. Usually a pharmaceutical therapy with anticoagulation, dilution of the blood, vasodilatation and wound care is done. Analgetic therapy using opioids is often necessary. But because a causal therapy is not known a lot of the patients suffering from Buerger’s disease get amputated. Even if an amputation can be avoided severe disturbances of the peripheral perfusion persists.

The occlusions are made by inflammatory thrombi, which differ from normal thrombi in that they include many inflammatory cells, with neutrophil polymorphs being the most prevalent . Male, heavy tobacco users, 25-50 years old are mostly affected PATHOLOGY There are three phases in this thrombocytic disease. The acute phase is characterised by inflammation. Firstly, platelet adhesiveness is increased and the lumen is obstructed by a highly cellular thrombus, which contains a dense mass of neutrophil, lymphocytes and giant cells. The affected areas are interspersed along the blood vessel. Mononuclear leukocytes and lymphocytes remain in the lumen, forming foamy giant cells . At the periphery of the thrombus, microabscesses that contain polymorphs and multinucleated giant cells may form . The inflammatory cells then migrate into the surrounding vessel wall, involving the intima, media and adventitia. The inflammatory reaction spreads to the vessel coats, adjacent vessels and nerves. In the subacute phase , inflammatory cells are less involved. Perivascular fibrosis, recanalization of the thrombus and vascularization of the blood vessel wall is characteristic
