What Can Cause Carpal Tunnel Syndrome?
The most common cause of Carpal Tunnel Syndrome is inflammation of the tendons in the tunnel which can normally be attributed to repetitive use of the hand and/or wrist. A number of sports can bring on repetitive stress injuries. The obvious sports associated with these ailments include both Tennis Elbow as well as Golfers Elbow . Just about any activity that involves repetitive movement, including rowing, skiing, archery, etc. can lead to injury. The above mentioned sports represent just a sampling of activities that stress the hand and wrist joints. Injuries and ailments that cause swelling or compression of soft tissue on nerves, such as sprains, leukemia, and rheumatoid arthritis can lead to stress injuries too. Diabetes, thyroid problems, and other systemic disorders are also associated with discomfort from stressed nerves, as is the fluid accumulation that sometimes accompanies pregnancy. Some authorities believe that a pyridoxine (vitamin B6) deficiency can also induce the symptoms.

Carpal tunnel syndrome occurs when tendons or ligaments in the wrist become enlarged, often from inflammation, after being aggravated. The narrowed tunnel of bones and ligaments in the wrist pinches the nerves that reach the fingers and the muscles at the base of the thumb. The first symptoms usually appear at night. Symptoms range from a burning, tingling numbness in the fingers, especially the thumb and the index and middle fingers, to difficulty gripping or making a fist, to dropping things. Some cases of carpal tunnel syndrome are due to work-related cumulative trauma of the wrist. Diseases or conditions that predispose to the development of carpal tunnel syndrome include pregnancy, diabetes, and obesity.

Brown, M.D. Along about the same time, the media publicized the fact that some individuals involved in repetitive type work activities, such as those who work on computers all day, have an increased incidence of carpal tunnel syndrome. In point of fact, most people who come in the hand surgeon’s office with carpal tunnel syndrome are perplexed as to why they have this disease because they do not engage in classical repetitive type work activities. Carpal tunnel syndrome is easily understood if one begins with the anatomy. The carpal tunnel is formed by a semi-circle of carpal bones on three sides. The fourth side that forms the carpal tunnel is the transverse carpal ligament. The ligament cannot stretch. Thus the carpal tunnel is a defined space that cannot enlarge. There is only so much room in that opening. Through that opening passes the median nerve, nine tendons, and spongy tissue around the tendons called tenosynovium. We start our lives with that extra space. When we run out of extra space due to the swollen tenosynovium then pressure is placed on the nerve. When this happens, one begins to develop carpal tunnel symptoms. Classic textbook carpal tunnel syndrome symptoms are tingling and numbness in the thumb, index and middle finger (median nerve distribution), aching in the forearm which can radiate to the shoulder and clumsiness or weak grip. Only about one or two patients out of ten presents with a classic textbook carpal tunnel picture. Some present with tingling in all fingers while others present with tingling only in the thumb or the middle finger. Some present with aching and pain in the hand while others have radiating pain just medial to the back.

This condition, or syndrome, has become the focus of much attention in the last few years due to suggestions that it may be linked to occupations that require repetitive use of the hands - such as typing. In reality, there are many people who develop this condition - regardless of the type of work that they do. The following documents attempt to explain what carpal tunnel syndrome is, how it is diagnosed, and describe the treatment options available.
Anatomy
The Median Nerve
Carpal tunnel syndrome (CTS) is a condition which results when the median nerve does not work properly. Usually, this is thought to occur because there is too much pressure on the nerve as it runs into the wrist through an opening called the carpal tunnel. It may be easier to understand how this occurs if you understand some of the anatomy of the wrist. The median nerve runs into the hand to supply sensation to the thumb, index finger, long finger, and half of the ring finger. The nerve also supplies a branch to the muscles of the thumb, the thenar muscles . These muscles help move the thumb and are very important in moving the thumb so that you can touch each of the other fingers. This motion is called

WHAT ARE THE CAUSES OF CARPAL TUNNEL SYNDROME?
What Are The Causes?
  • Repetitive and/or static wrist and finger flexion causes the flexor muscles which 'close' the hand to develop more strength than the wrist and finger extensor muscles that 'open' the hand. This quickly leads to a "muscle imbalance" of the wrist and hand because of the lack of activities that require direct stimulation of the extensor muscles; such as opening a door or holding onto the steering wheel of a car with the backside of the hands.
  • This imbalance causes the carpal bones to shift into the carpal tunnel from the sides and from above, while the hands are in the palm down position.
  • Performing repetitive wrist and finger flexion causes inflammation of the flexor tendons due to friction within the severely compressed carpal tunnel; ultimately aggravating the existing symptoms, and leading to possible irreversible damage of the underlying tendons, blood vessels and median nerve.

June 1997 In recent years, reports of repetitive motion injuries have risen dramatically in workplaces across the country. These problems, frequently termed "Cumulative Trauma Disorders" are being reported at alarming rates in all types of workplaces - from meatpacking plants to newspaper pressrooms. According to the Bureau of Labor Statistics, “disorders associated with repeated trauma” account for about 60% of all occupational illnesses. Of all these disorders, carpal tunnel syndrome is the condition most frequently reported. What is Carpal Tunnel Syndrome (CTS)? The carpal tunnel receives its name from the 8 bones in the wrist, called carpals, that form a tunnellike structure. The tunnel is filled with flexor tendons which control finger movement. It also provides a pathway for the median nerve to reach sensory cells in the hand. Repetitive flexing and extension of the wrist may cause a thickening of the protective sheaths which surround each of the tendons. The swollen tendon sheaths, or tenosynovitis, apply increased pressure on the median nerve and produce Carpal Tunnel Syndrome (CTS). What are the Symptoms of CTS?
