    Causalgia may be described as an uncommon pain syndrome characterized by posttraumatic pain most commonly affecting the upper or lower extremities. Clinically, the presenting pain is usually paroxysmally produced by local friction, or even air currents, which affect the nerve supply innervating that given region where the pain syndrome is symptomatically manifested. This pain phenomenon varies in its clinical presentation from immediately post-injury to several weeks later. The character of the injury may be as trivial as a sprain but any type of trauma may be etiological. Although the etiology is idiopathic, it is most often due to complete, or incomplete, laceration of the nerve, or plexus, involved. The cardinal symptom is severe burning pain of paroxysmal character. There may be an intolerance to dryness in the area of pain and possible cutaneous coolness, color changes involving redness, or cyanosis, with glossiness, and edema. The pain is commonly exquisitely hyperalgesic. The symptoms may not present for several weeks following the etiological event, but gradually cutaneous symptoms indicative of vasodilation appear. Later, the skin may become cold, cyanotic, pale, diaphoretic, thin, and glossy with brittle and ridged nails. These cutaneous arteriovenous characteristics usually exist even in the presence of blood flow studies which show equality with the uninvolved extremity. These findings suggest that such nerve damage may affect surface vasodilatation without influencing larger deep vessels. Reduction, or complete elimination, of the pain by sympathetic block is diagnostic.

    A causalgia é uma dor grave caracterizada por uma sensação de queimação intensa, especialmente associada a uma deformação violenta e súbita dos nervos, causada por projéteis ultra-rápidos, tais como as balas. Calcula-se que se manifesta em 2 a 5% dos casos de lesão dos nervos periféricos. Em 85% dos casos, a causalgia persiste durante mais de seis meses após a lesão e pouco a pouco desaparece espontaneamente; contudo, um ano depois, cerca de 25% ainda se queixa de dor. A causalgia, caracterizada como uma dor de queimação possui um certo número de características da dor do membro fantasma, além de outros aspectos que lhe são próprios. O seu caráter dominante é a inexorável intensidade da dor que evoca algumas imagens do "Inferno de Dante". A causalgia implica, evidentemente, a presença de influxos sensitivos anormais, provenientes das zonas inervadas pelo nervo lesado. Já que o toque mais ligeiro pode provocar dor, estes indivíduos evitam todos os estímulos táteis. A dor limita, pois, o movimento que, por sua vez, reduz a quantidade de combinações de influxos cutâneos e propioceptivos habituais do membro. Os influxos são, por conseqüência, duplamente anormais, devido à lesão do nervo e à exagerada proteção do mesmo. Uma das características mais notáveis da dor causálgica é a facilidade com que é desencadeada ou intensificada por uma infinidade de estímulos nociceptivos. A dor é provocada pelo mais leve estímulo sensitivo; por vezes, este mesmo não existe. Ruídos súbitos, alterações rápidas de estimulação visual, perturbações emocionais, reações de sobressalto são causas possíveis de agravamento da dor.

    A Laboratory Model for Clinical Pain Mitchell Max's clinical trials with diabetic neuropathy and postherpetic neuralgia patients addressed the major enigma of the pain field: the mechanism involved in chronic neuropathic pain. The classic neuropathic pain disorders are the agonizing conditions known as causalgia "the most terrible of all tortures", in the words of S. Weir Mitchell , the Civil War physician who first described it and reflex sympathetic dystrophy (RSD) (more recently termed chronic regional pain syndrome). Usually occurring after nerve injury to a hand, foot, arm or leg, causalgia and RSD may be triggered by any minor injury, by a surgical procedure, or a serious trauma. They are characterized by pain which persists after the original injury has healed; by hyperalgesia , intensified perception of a mild painful stimulus, and allodynia , perception of pain in response to the lightest of touches; even by spontaneous pain, where the patient reports pain although no stimulus is present! The pain often proves resistant to all treatment, including medication, counter- stimulation, anesthetic blocks, psychotherapy, and even neurosurgery. The unhappy victim of such a syndrome becomes withdrawn and depressed, resists all physical contact, and guards the affected limb constantly.

    CAUSALGIA
    Causalgia is characterized by persistent, diffuse burning pain which tends to recur from various stimuli, such as contact with air or drying of a peripheral nerve. The pain may be severe and accompanied by circulatory disturbances, discoloration, coldness and excessive sweating of the involved part. The muscles of an involved arm or leg may atrophy from immobilization to avoid pain.
    There is no effective drug for curing causalgia, but some drugs may relieve the pain for a few hours after each dose. Most of these drugs have undesirable side effects and are addictive.
    Acupuncture has been very successful in giving lasting relief from the pain of causalgia When the pain is relieved, the patient becomes able to use the involved part normally and thereby reduce the other symptoms of this condition.
    Improving nutrition will speed recovery from all the symptoms of causalgia Daily intake of vitamins is recommended
    As pain is relieved, mild exercise of the involved part should begin. It is important to avoid chilling and medications
