Chronic obstructive pulmonary disease, or COPD, is characterized by abnormalities in the lungs that make it difficult to exhale normally. Generally, two distinct diseases are involved: emphysema and chronic bronchitis . Emphysema and chronic bronchitis cause excessive inflammatory processes that eventually lead to abnormalities in lung structure that permanently obstruct airflow (hence the term "chronic obstructive"). Symptoms include shortness of breath (dyspnea), wheezing, chronic cough, barrel chest, and pursed-lip breathing (see Symptoms). Asthma is usually reversible, but some asthmatics have permanently narrowed airways and suffer COPD. According to the World Health Organization (WHO), 75% of deaths from COPD that occur in developed countries are directly related to smoking tobacco. Risk Factors Tobacco use is the number one risk factor. Heavy smokers are at greatest risk. Cigarette smokers are at greater risk than cigar and pipe smokers. All smokers are at greater risk than lifelong nonsmokers.

What is Chronic Obstructive Pulmonary Disease?
COPD (chronic obstructive pulmonary disease) refers to a spectrum of respiratory diseases which are characterized by some mixture of chronic cough, increased sputum production, dyspnea, airflow limitation, and impaired gas exchange. The major characteristic of COPD is the presence of chronic airflow limitation that progresses slowly over a period of years and is irreversible (Canadian Respiratory Review Panel, 1998).
Facts
COPD is generally a silent and unknown killer in Canada, and threatens to be one of the main causes of disability and death in the new millennium. In regard to mortality, COPD is:
  • the fourth leading cause of death for men and seventh for women, and killed 9,618 Canadians in 1997 (LCDC, Health Canada);
  • expected to be the 3rd leading cause of death worldwide by 2020 ( Lancet

While COPD is a significant cause of death, the impact of the disease may be even more severe. As the National Mortality Database does not record underlying causes of death such as COPD, true mortality statistics may be underreported (Canadian COPD Alliance, 2000). Concerning morbidity:

Problem/Condition: Chronic obstructive pulmonary disease (COPD) includes chronic bronchitis and emphysema but has been defined recently as the physiologic finding of nonreversible pulmonary function impairment. This surveillance summary reports trends in different measures of COPD during 19712000. Reporting Period Covered: This report presents national data regarding objectively determined COPD (19711994); COPD-associated activity and functional limitations (19801996); self-reported COPD prevalence, COPD physician office and hospital outpatient department visits, COPD hospitalizations, and COPD deaths (19802000); and COPD emergency department visits (19922000). Description of Systems: CDC's National Center for Health Statistics (NCHS) conducts the National Health Interview Survey annually, which includes questions concerning COPD and activity limitations. NCHS collects physician office-visit data in the National Ambulatory Medical Care Survey, emergency department and hospital outpatient department data in the National Hospital Ambulatory Medical Care Survey, hospitalization data in the National Hospital Discharge Survey, and death data in the Mortality Component of the National Vital Statistics System. Data regarding pulmonary function were obtained from the National Health and Nutrition Examination Surveys (NHANES) I (19711975) and III (19881994), and data regarding functional limitation were obtained from NHANES III, Phase 2 (19911994).

CDC MODERATOR: Thank you all for joining us. My name is Elwin Grant, and I am a press officer assigned to the CDC's Office of Communication. Today, we will be discussing two articles. One is a surveillance report. First, we have Dr. David Mannino, and that is spelled M-a-n-n-i-n-o, who is CDC's pulmonary disease expert with the CDC's National Center for Environmental Health. He will be discussing a report which summarizes obstructive pulmonary disease surveillance in the United States for the period of 1971 through 2000. And then we have Dr. Anthony Marfin, and that's spelled M-a-r-f-i-n, who is CDC's West Nile Virus expert assigned to CDC's National Center for Infectious Diseases. When asked to picture a patient presenting with a 'crippling disease of the airways', most people think of an asthmatic, possibly a child, in the throes of an acute exacerbation, desperately searching for their metered-dose inhaler and relief. Compare this with the COPD patient, over 50 years old with breathlessness, wheeze and productive cough, whose disease is probably self-inflicted by smoking and who responds relatively poorly to treatment. No wonder there is such a gulf between these two images in the public perception and, until recently, that of healthcare workers.

Causes, Incidence, and Risk Factors
The leading cause of COPD is smoking, which can lead to the two most common forms of this disease, emphysema and chronic bronchitis. Prolonged tobacco use causes lung inflammation and variable degrees of air sack (alveoli) destruction. This leads to inflamed and narrowed airways ( chronic bronchitis ); or permanently enlarged air sacks of the lung with reduced lung elasticity ( emphysema ). Between 15-20% of long-term smokers will develop COPD. Rarely, an enzyme deficiency called alpha-1 anti-trypsin deficiency can cause emphysema in non-smokers.

WHY? ( Why this site?)
I am creating this site hoping you will find my links useful/enjoyable/interesting. After years of traveling and teaching (languages and ESOL) I had to retire because of two bouts of respiratory arrests due to emphysema. I stopped smoking (cold turkey, but you don't have to do it this way), went to a rehabilitation program, joined a gym and to reduce the craving for cigarettes kept busy gardening (since my yard is small, mostly herbs and flowers). I always (well, since 1987) loved computers (WHERE ARE YOU?! OH MY COMMODORE 64!) especially DOS, so it is evident that this is first my interest. My links make sense once you read the few lines above. Hope to hear from you soon. Back to Top
