Clefts are gaps in the developing lip and/or palate. They may be small notches or large open spaces. They occur because of problems during prenatal development. The two sides of the developing face fail to meet and join properly.

Cleft Lip and Palate
Cleft lip and cleft palate are conditions which occur when tissues that usually form the lip or the roof of the mouth fail to grow together, creating a gap in the lip or a hole in the roof of the mouth. The reasons for this are not entirely known, but the condition is among the most common problems noted at birthit occurs once in every 1000 live births (the same frequency as club foot). A recent study(March 1996) indicates that smoking during pregnancy may double the risk that the child will be born with a cleft lip or palate. Since it occurs this frequently, almost everyone knows someone who has a cleft lip or cleft palate. 1. Do you know anyone with a cleft lip or palate? Yes No The repaired cleft lip may be noticed in the adult as an uneven upper lip and a scar which runs from the nose down through the upper lip. Persons with a repaired cleft palate may have a nasal quality to their speech. This can occur because there is a shortage of tissue in the roof of the mouth after the repair in some cases and sounds can accidentally leak out the nose when certain words are spoken. In most cases following reconstructive surgery the patient can expect to live a normal life and the cleft condition should hopefully not stand in the way of any dreams or aspirations. 2. Cleft lip and/or palate repair is expensive. If the parents have no insurance would you favor programs providing tax dollars to pay for such care?

CLEFT LIP AND PALATE One in every 700 births results in a cleft lip and/or palate. Many of these congenital deformities occur in families that have no history of clefts. While it is known how cleft deformities occur, it is seldom known why.
Cleft defects develop very early in pregnancy. They result from an abnormality in the genes (units of heredity) or chromosomes (packets of genes). These elements contain the program that determines how the child will be formed. Why and how abnormalities in the genes and chromosomes develop is largely unknown. An individualized genetic assessment may be helpful in identifying the cause of the malformation.

    Not necessarily, if the surgical intervention is timely.
    Q: At what age should he be operated?

    The surgery should be done between 6-18 months. But palate repair carried out at around 12 months of age is generally preferred. Unfortunately for some children, early surgery may not be possible because of other handicapped conditions and extensiveness of the cleft of palate.
    Q: Will my child require a second surgery?

    Q: My child does not have a visible cleft lip and palate. But still, he has been advised surgery because of his hypernasality (Has a nasal voice).

    INTRODUCTION
    Cleft lip and palate represents the second most frequently occurring congenital deformity (after clubfoot deformity). Cleft lip, cleft palate or both affects approximately 1 in 750 births. Clefting is associated with many problems including cosmetic and dental abnormalities, as well as speech, , hearing and facial growth difficulties. The otolaryngologist is uniquely qualified to identify and manage many of these problems, and holds a key role on the cleft palate team.

    Cleft lip can be an isolated event or can be found in conjunction with a cleft palate. It is caused by the failure of the normal structures to fuse together during the formation of the lip. The cleft is an opening in the lip. The skin, muscle, and oral lining tissue is present but has not fused leaving an opening. Cleft lips occur more often on the left side rather than the right side for reasons that we don't understand. More than half of cleft lips occur with a cleft palate where there is a failure of the two sides of the roof of the mouth to fuse in the midline to form a complete palate. Clefts of the lip, incomplete or complete, are associated with some degree of nasal distortion. The children with incomplete cleft lips have a smaller nasal deformity than the children with complete cleft lips. Clefts that involve the lip and the palate are more significant in that cleft palates can affect feeding, weight gain, middle ear function, speech and facial growth. Each of these issues may be noted at different ages, i.e. difficulty feeding and middle ear dysfunction are infant and early childhood issues. Speech difficulties present themselves later around two years of age. Facial growth and dental occlusion are preteen or adolescent issues. A craniofacial team is essential to provide the children and their families the care they need to give the best result.

    In the early weeks of development, long before a child is born, the right and left sides of the lip and the roof of the mouth normally grow together. Occasionally, however, in about one of every 800 babies, those sections don't quite meet. A child born with a separation in the upper lip is said to have a cleft lip. A similar birth defect in the roof of the mouth, or palate, is called a cleft palate. Since the lip and the palate develop separately, it is possible for a child to have a cleft lip, a cleft palate, or variations of both.
    If your child was born with either or both of these conditions, your doctor will probably recommend surgery to repair it. Medical professionals have made great advances in treating children with clefts and can do a lot to help your child lead a normal, healthy, happy life.

    Common Congenital Disorders The cleft lip deformity is the most common congenital deformity seen in the United States. The overall occurrence is one in seven hundred live births. Some ethnic groups (eskimos, asians) have a higher number of babies with cleft lip. Sometimes the cleft lip is associated with a cleft in the roof of the mouth (cleft palate). The cleft lip maybe unilateral (one side) or bilateral (on either side of the nose). The cleft lip maybe complete (no bridging tissue across the cleft) of incomplete. The deformity seen in your baby with a cleft lip includes the lip, muscles around the lip, and the nose. All these deformities should be addressed at the initial surgery. The alveolus (the bone where the teeth come out) may also have a cleft, which may be address early if the deformity is not too severe, often the alveolus cleft is corrected at seven or eight years old. The repair of the lip is usually completed around three months of age. In preparation

    A baby which was abandoned by its parents in Portugal was identified partly through a distinctive cleft upper lip. BBC News Online looks at the condition and how its treated. A 'cleft' or split forms parts of the face which developed separately during early pregnancy, do not then join together properly. A cleft can affect the lip, the palate and the gum. The severity of the condition can vary. A cleft lip creates an opening between the mouth and the nose, and can look as though there is a split in the lip. It can range from a slight 'notch' in the coloured part of the lip to a complete separation in one or both sides of the lip extending up and into the nose. Some babies who have a cleft lip may also have a cleft in the gum. Again, that can either be a small split, or complete division.

    Scientists have identified a crucial faulty gene that causes an inherited form of cleft lip and palate. The form of the condition, known as Van der Woude syndrome, can cause severe facial disfigurements in babies.

    Geoff Murray The researchers discovered the gene - known as Interferon Regulatory Factor 6 (IRF6) - by examining information generated from the Human Genome Project. Much of the work was based on an analysis of samples from a set of twins in Brazil, where, highly unusually, one boy was born with Van der Woude syndrome, and the other without. In the long-term, it could lead to better understanding of why and how cleft lip and palate occurs, and could lead to treatments during pregnancy that might prevent the development of the condition. Important advance Researcher Professor Michael Dixon, of the University of Manchester, said: "This research is of significant global interest.
