Cleft Lip and Palate: Van der Woude Syndrome
After a nearly 20-year search, a team of scientists report in the October 2002 issue of Nature Genetics that it has discovered a gene involved in causing Van der Woude Syndrome, the most common form of syndromic cleft lip and palate. The Inside Scoop recently spoke with two authors on the paper: NIDCR grantees Jeff Murray, M.D., and Brian Schutte, Ph.D., both of whom are scientists in the Department of Pediatrics at the University of Iowa. They shared their thoughts on the difficulty in identifying the Van der Woude Syndrome gene, the possible scientific benefits of studying twins, and the implications of this gene discovery on the more common non syndromic cleft lip and palate.
When did the search for the Van der Woude Syndrome gene begin? Murray: The search began around 1985, at least for our group. At that time, the tools had improved to search for genes involved in "single gene" disorders, that is, where a change in a single gene causes an inherited condition. Van der Woude Syndrome is the best example of a single-gene disorder that causes cleft lip and palate that looks like the "non-syndromic" form, one of the most common birth defects in the world. In 1990, your group narrowed the search to a specific segment of chromosome 1. Over the next several years, you searched this region and excluded a number of candidate genes as the cause of Van der Woude Syndrome. One of these genes was IRF6, the subject of this month's paper. What lead you to reject it originally in the mid-to-late 1990s?

http://www.nidcr.nih.gov/news/inside_scoop_cleft_lip_palate.asp Scientists report in this month's Nature Genetics they have discovered the gene that causes Van der Woude syndrome, the most common of the syndromic forms of cleft lip and palate. The term "syndromic" means babies are born with cleft lip and palate, in addition to other birth defects. According to the scientists, the discovery could very possibly direct them to genes involved in "non-syndromic" cleft lip and palate, one of the most common birth defects in the world. Among Caucasians, non-syndromic cleft lip and palate occurs in an estimated 1 in every 1,000 live births, and the frequency seems to be even higher in some Asian countries, such as China and The Philippines. "Since there is so much clinical overlap between the two, we expect that similar genes and maybe even the same genes will be involved in the non-syndromic form," said Jeff Murray, M.D., a scientist at the University of Iowa and an author on the paper. Murray noted that the gene, called IRF6, seems to play a key role in the normal formation of the lips, palate, skin, and genitalia. He said further study of the gene should provide precise molecular clues into normal human development and suggest specific biological strategies to prevent birth defects, such as cleft lip and palate.

Children's services 15 Cleft lip and palate services (all ages) The main recommendation from the Clinical Standards Advisory Group (CSAG) in their report on cleft lip and/or palate was that the expertise and resources for cleft lip and palate services should be concentrated within a small number of designated centres throughout the UK. The designated centres ('hubs') would also have 'spoke' arrangements to ensure that accessibility was optimal without compromising the quality of care. This recommendation was accepted by the government and HSC (1998) 198 identified cleft lip and palate services as a national priority for the new arrangements for commissioning specialised services. Specialised services commissioning groups are at present confirming the designated cleft centres in England. This process is expected to continue throughout 2001/02 and beyond. Where new centres have been designated, new-born babies with clefts should only be referred to these units. Patients already undergoing treatment may however continue to receive non-surgical care in their existing units provided these remain viable and are fully integrated into the clinical network managed from the designated centre. If for any reason existing centres are not part of the new network of care, their records will nevertheless need to be transferred or otherwise available to the designated centre.

What is cleft lip and palate?
Cleft lip (split of the upper lip) and cleft palate (split of the roof of the mouth) are the most common types of congenital (birth) defects. Because the lip and the palate develop separately, it is possible for the child to have a cleft lip, a cleft palate, or both cleft lip and cleft palate. Cleft lip:
A cleft lip is a condition that creates an opening in the upper lip between the mouth and nose. It looks as though there is a split in the lip. It can range from a slight notch in the coloured portion of the lip to complete separation in one or both sides of the lip extending up and into the nose. A cleft on one side is called a unilateral cleft. If a cleft occurs on both sides it is called a bilateral cleft. A cleft in the gum may occur in association with a cleft lip. This may range from a small notch in the gum to a complete division of the gum into separate parts.

The Vanderbilt Bill Wilkerson Center is dedicated to pursuing a mission of providing patients with the highest quality of care in a compassionate environment, improving upon current clinical treatments through the conduction of ethical and relevant clinical and basic science research, and training motivated individuals to disseminate these goals and become future leaders in the fields of otology and Communication Sciences. New - cleft lip and palate Analysis standard error estimate r2 showed significant main effects for age place articulation. In addition to the languageintervention provided by the -pathologists, occupational therapists at the center work on desensitizing the children to stimuli that might overwhelm them. In particular, findings would seem to provide a relatively large set clinically-based information that clinicians can use to evaluate their own clinical practice. These agents result in an initial increase in urination, they eliminate excess fluid. The center's final funding need is for curriculum materials software. We have computerized our processes as far as possible so that we know each patient receives the same quality care as the next. Cleft lip and Palate Cleft lip and/or palate is a common condition seen in children where parts of the facial tissues have not fused together at around 6-7 weeks in the womb. It is seen in very many variations, with parts of the lip and palate (including the jaw -mandible-bone) which may be fully or partially divided. Sometimes the cleft is not obvious on first examination because the surface tissues are united, but the underlying muscle is divided; this is called a submucous cleft palate or a forme fruste cleft lip. The cleft can occur on one or both sides of the lip/palate, and can occur together with some other abnormal developments (although the majority of children have no other problems at all). Sometimes clefting runs in families, but again, the majority occur in isolation. Considerable research is currently taking place both in Oxford and around the world to try to find out the cause of clefting. Clefts of the front of the face can often be seen on ultrasound scans taken of babies during pregnancy, but cleft palates cannot be seen before birth.
