    A cleft is a separation in a body structure. Clefts that occur in the oral-facial region often involve the lip, the roof of the mouth (hard palate) or the soft tissue in the back of the mouth (soft palate). Two major types of oral-facial clefts are cleft lip/palate and isolated cleft palate. Babies with cleft lip/palate have a cleft lip which usually is accompanied by cleft palate. In isolated cleft palate, the cleft palate occurs by itself, without cleft lip or other malformations. These two forms of oral-facial clefts are considered separate birth defects. Here we refer to each specific type of cleft by name, and use the term oral-facial cleft when the information applies to both.
    There also are about 400 syndromes in which babies have some form of oral-facial cleft along with a wide variety of other birth defects. The diverse problems of these babies are not described here.

    What is a cleft lip or palate?
    The term "cleft" refers to an opening. An opening in the lip is called cleft lip. An opening in the roof of the mouth (palate) is called cleft palate. The separate parts that form a baby's face and palate come together during early pregnancy. A cleft occurs if the parts do not fuse completely. One out of every 700 babies has some form of cleft lip or cleft palate. Up to 5,000 babies are born with a cleft lip or palate in the United States each year. Cleft lip used to be referred to as a hare lip because it was thought the upper lip looked like a rabbit's lip. The term hare lip should not be used. The defect should be referred to as cleft lip.

    CLEFT LIP AND CLEFT PALATE What is a cleft lip/cleft palate? A cleft lip is a separation of the two sides of the lip. Sometimes this includes the bone of the upper jaw. A cleft palate is an opening in the roof of the mouth in which the two sides of the palate did not join together. Cleft lips and palates can be unilateral or bilateral. Why did this happen? Sometime between the 6th and 11th week when your baby was developing, the parts of the lip and/or palate failed to come together properly. We all have clefts of our lips and palates initially. Some join together and others do not. Why some do not, we may never know. Sometimes clefts can be found in families who have had clefts in other family members. Some clefts can be linked to certain syndromes. Here are some statistics to help you understand the frequency of clefting:
    • Clefts occur in 1 in 700 newborns Cleft palates are not racially influenced Clefts occur in more males than females There is only a 5%, chance that another child will be born with a cleft. If another child is born with a cleft, the incidence increases.

    Cleft Lip and Cleft Palate How do cleft lip and cleft palate occur?
    Each of us had a cleft lip and cleft palate during the early weeks of development in our mother's womb. Normally, the tissues that form the palate and the upper lip come together in the middle and join (fuse). You can see the lines of fusion in the "Cupid's bow" under your own nose, and the ridge and pale line in the middle of your palate (the roof of your mouth). If your baby has a cleft, this fusion failed to happen when he or she was developing. Why does fusion of the palate fail to happen? In most cases, we simply don't know why lip and palate development go wrong. About 1 in 600 babies has a cleft lip or cleft palate. Race and gender play a small role. Clefts are most common in Asians. They are less common in whites and least common in blacks. Girls are more often affected than boys. In some families, clefts appear in several family members, so heredity is important. Sometimes substances in the environment, called teratogens, may be associated with clefts. But most babies with clefts have no known relatives with clefts and no known exposure to teratogens. A few babies with clefts also have other abnormalities. Your baby's doctor will look for these other abnormalities and tell you what he or she finds. Did we do anything wrong?

The defect occurs in early pregnancy, when the left and right sides of the lip and roof of the mouth normally fuse together.

    The lip and palate (roof of the mouth) develop separately. A baby can have a cleft lip, cleft palate, or a combination of the two. What causes cleft defects? Treatment for cleft defects: Most cleft lips can be repaired within the first few months of life (usually when the baby is about 10 weeks old). A cleft palate is usually repaired later than cleft lips (usually when the baby is between nine and 18 months old). Timing of surgery will be determined by your physician(s) based on:

    The face forms, in the first few weeks after conception, from several elements joining together: sometimes the process is not quite completed and the elements remain separate with a cleft between them, sometimes on one side (unilateral), sometimes on both sides (bilateral). The cleft may be complete or incomplete, and the problem can be picked up on a scan during the pregnancy.
    The palate (roof of the mouth) forms by the two shelves growing inward to separate the nasal space from the mouth. Sometimes this process is not quite completed and a cleft remains. This is not visible on a scan.
    The Team
    Although a cleft is not a serious deformity in terms of the baby's health it affects the face, the jaws and the palate, and may affect appearance, the teeth and the swallowing and speech mechanism: sometimes there are problems with hearing.
    A team of experts is therefore required to work together to provide best care for the child from birth until growth is finished at about 18 years.
    Therefore plastic surgeon, orthodontist, oral and maxillo-facial, and ear nose and throat surgeons work together with nurses, counsellors, anaesthetists and speech and language therapists. A geneticist may advise on the chances of any other child having the same problem. Health visitors and parent support groups are also important.

    If you're considering surgery to correct cleft lip or palate . . . In the early weeks of development, long before a child is born, the right and left sides of the lip and the roof of the mouth normally grow together. Occasionally, however, in about one of every 800 babies, those sections don't quite meet. A child born with a separation in the upper lip is said to have a cleft lip. A similar birth defect in the roof of the mouth, or palate, is called a cleft palate. Since the lip and the palate develop separately, it is possible for a child to have a cleft lip, a cleft palate, or variations of both.
    If your child was born with either or both of these conditions, your doctor will probably recommend surgery to repair it. Medical professionals have made great advances in treating children with clefts and can do a lot to help your child lead a normal, healthy, happy life.
    This information will give you a basic understanding of the operation when it can help, how it's performed, and what results you can expect. It can't answer all of your questions, since a lot depends on you individual circumstances. Please be sure to ask your surgeon if there is anything you don't understand about the procedure.
    The importance of a team approach
    Children born with a cleft lip or palate may need the skills of several medical professionals to correct the problems associated with the cleft. In addition to needing plastic surgery to repair the opening, these children may have problems with their feeding and their teeth, their hearing, their speech, and their psychological development as they grow up.
