HOME PAGE Case Studies: Conversion Disorder January 22, 1995 Patient is a 52-year-old woman whose husband is president of a local union. She has become increasingly weak and has been unable to function- particularly with her hands. She has also has difficulty walking, and has lost weight and become lethargic. She has been hospitalized, and biopsies have shown a change in the musculature- particularly of the thenar area (the muscle between the thumb and forefinger). A diagnosis of amyotrophic lateral sclerosis, or Lou Gerhig's Disease, has been made. Patient gives a history of being very unhappy and isolated. She feels that the marriage to her husband of 27 years deteriorated. He spends very little time with her and she feels quite alone, particularly since the children have grown. She has done some private duty nursing, and her last case was a man who had Lou Gerhig's disease and recently died. Patient will be seen on a regular basis. She was placed on an antidepressant, Prozac, 20 mg. once a day. March 15, 1995

HOME PAGE Case Studies: A Case of Conversion Disorder and Rage October 19, 1984 P.T. is a 21 year-old man who was referred to this office for psychiatric problems secondary to a work related accident. He was working on a construction site and driving a front-end loader when a dump truck crashed into the loader. He was trapped under the loader for approximately 25 minutes. He was taken to Shock Trauma at the University of Maryland Hospital. After a careful evaluation, it was found that although there were some severe strains, there were no actual physical injuries that required any type of surgical intervention. He was sent home on muscle relaxants and pain killers. Since he has been home he has found that he is quite limited physically. He is bent over, and walks with a great deal of difficulty- even with the assistance of a recently purchased cane. He has been followed by a number of physicians, all of whom feel that there is no obvious physical injury. They suspect that this could be a conversion disorder whereby he believes that he has been physically injured, but indeed has no obvious physical injury to the point that any type of surgical intervention is required. Patient gives a history of being raised in an extremely volatile household. His father, a policeman, would come home after duty and be extremely assaultive to the family. His father would also drink heavily. He remembers his mother being struck, and he and his brother being thrown around the room and being quite terrified. He would often urinate in his pants when these traumatic episodes would occur. He was a bed-wetter until the age of thirteen.

Conversion Disorder
DSM IV Criteria The patient complains of symptoms or deficits affecting voluntary muscles, or deficits of sensory function that suggest a neurological or medical condition. The temporal relation of symptoms to a stressful event suggests association of psychological factors. Symptoms are not intentionally produced. Symptoms are not explained by an organic etiology. Symptoms result in significant functional impairment. Symptoms are not limited to pain or sexual dysfunction, and are not explained by Symptoms most commonly are sensory (blindness, numbness) and motor deficits (paralysis, mutism), and Abnormalities usually do not have an anatomical distribution and the neurological exam is normal. Patients often lack the characteristic normal concern about the deficit. This characteristic lack of concern has been termed A la belle indifference. Conversion disorder can coexist with depression, anxiety disorders, and schizophrenia. Conversion symptoms often will temporarily remit after the disorder has been suggested by the physician.

I was recently "diagnosed with conversion disorder, where they think mental anxiety or whatever somehow turns into physical symptoms. Has anyone ever had any experience with this? How is it treated? If symptoms are every "subconsciously self-induced", how are they ever "un-induced"? My symptoms (much like MS symptoms) are very real, and the weakness and balance problems are almost disabling. I can no longer drive or walk around without falling, yet the doctors can't seem to find any answers (either that, or they are all reading from different textbooks and come up with different possibilities but no definite answers). My husband and I get the very distinct feeling that this is more a diagnosis of exclusion than anything, that they just don't want to deal with it. I realize the mind and the body are very connected, but I also KNOW these problems are not all in my head. Please share your experiences with me. Thanks. Next Article

I am a female. I have been diagnosed with conversion disorder and I have not made the first visit to the psychologists office, I am afraid. I suffer from an incident where I was EXTREMELY afraid for my life and since then I have big problems sleeping, sometimes I don't sleep at all and I jump at the slightest noise. Sometimes I wake up in a panic because of frequent nightmares and I am shaking so bad I can't get back to sleep. WHen I have a "seizure" I get a "drunk" feeling and my speech slurs really bad and I shake, but I am aware of my surroundings. At first it affected my performance at work, now they are less frequent, but still happening and I am still afraid to see someone. My fiancee is very supportive though. Is there anyone that can give me some info on this, especially if its happened to you. Thank you JR Next Article

Various tests have been designed to assess the so-called "truthfulness" of a patient's pain or the degree of effoft they put forth as a measure of the validity of the test. One of main theoretical difficulties practicioners must overcome in these patients is having a diagnostic grid that is too small to understand properly the scope of the clinical material that one may encounter. It seems to me that the problem lay in the seeming dichotomy between mind and body. Modern medicine has become filled with splitters those who for the sake of clear diagnostic criteria have arbitrarily decided that some conditions are physical (or use the term organic) while others are mental (also called functional). Of course there is truth to this, however, clinicians must face the fact that the body and psyche are irrevocably linked. This has been repeatedly proven scientifically and experimentally.
