Causes, Incidence, and Risk Factors
Normally, the left side of the heart produces a higher level of blood pressure in order to pump blood to the body; the right side pumps blood through the lungs under much lower pressure. Any condition that leads to prolonged high blood pressure in the arteries or veins of the lungs (called pulmonary hypertension) will be poorly tolerated by the right ventricle of the heart. When this right ventricle fails or is unable to properly pump against these abnormally high pressures, this is called cor pulmonale. Almost any chronic lung disease or condition causing prolonged low blood oxygen can lead to cor pulmonale. A few of these causes include:

    Definition
Cor pulmonale is an increase in bulk of the right ventricle of the heart, generally caused by chronic diseases or malfunction of the lungs. This condition can lead to heart failure Description Cor pulmonale, or pulmonary heart disease, occurs in 25% of patients with chronic obstructive pulmonary disease (COPD). In fact, about 85% of patients diagnosed with cor pulmonale have COPD. Chronic bronchitis and emphysema are types of COPD. High blood pressure in the blood vessels of the lungs ( pulmonary hypertension ) causes the enlargement of the right ventricle. In addition to COPD, cor pulmonale may also be caused by lung diseases, such as cystic fibrosis pulmonary embolism , and pneumoconiosis. Loss of lung tissue after lung surgery or certain chest-wall disturbances can produce cor pulmonale, as can neuromuscular diseases, such as muscular dystrophy . A large pulmonary thromboembolism (blood clot) may lead to acute cor pulmonale. Causes and symptoms Any respiratory disease or malfunction that affects the circulatory system of the lungs may lead to cor pulmonale. These circulatory changes cause the right ventricle to compensate for the extra work required to pump blood through the lungs. The right ventricle has thin walls and is crescent-shaped. The resulting pressure causes the right ventricle to dilate and bulge, eventually leading to its failure.

    The Heart Failure Center
Congestive heart failure (CHF) is a serious condition in which the heart is not pumping well enough to meet the body's demand for oxygen. It got its name because the heart is failing to pump efficiently, which often results in congestion in the lungs. As a result, the heart tries to overcompensate for the problem, which only makes the problem worse. The increased workload of the cardiac muscle may lead to changes in various parts of the body, as they try to compensate for the muscle's weakened ability to function. These changes can include enlarged heart , hypertrophy (thickening) of the cardiac walls, tachycardia (an abnormally fast heartbeat), left ventricular hypokinesis (the left ventricle does not contract well) and kidney malfunction. Conditions that could lead to conjestive heart failure include the following:
    • Coronary artery disease
    • High blood pressure (hypertension)
    • Heart attack
    • Cardiomyopathy
    • Valvular disease (e.g., valvular stenosis or valvular regurgitation)
    • Infection in the cardiac valves (valvular endocarditis) or of the cardiac muscle (myocarditis)
    • Congenital disease (a cardiac condition present since birth)
    • Severe lung disease (e.g., pulmonary hypertension)

    Congestive Heart Failure (CHF)
Congestive heart failure (CHF) is a serious condition in which the cardiac muscle is not pumping well enough to meet the body's demand for oxygen. This decreased cardiac output can be quantified with a measurement called the ejection fraction. Congested heart failure is sometimes referred to as cardiac asthma. Right sided heart failure is often called cor pulmonale. Contrary to its name, CHF does not necessarily indicate that the cardiac muscle has completely failed/stopped, which is the case when someone has gone into cardiac arrest. It means that the cardiac muscle is operating at a decreased efficiency level and, therefore, is working harder to try and make up for the shortcoming in function. For example, the cardiac muscle may pump more frequently to compensate for its weakened pumping ability. The longer the cardiac muscle overworks itself to compensate for its shortcomings, the more its pumping ability is damaged and the more likely that serious pumping failure will result. The increased cardiac workload may lead to changes in various parts of the body, including an

In 1985 the Ontario legislature established the Industrial Disease Standards Panel (IDSP) to investigate and identify diseases related to work. The Panel is independent of both the Ministry of Labour and the Workers' Compensation Board. At the end of each fiscal year the WCB reimburses the Ministry for the Panel's expenditures. The Panel's authority flows from section 95 of the Workers' Compensation Act and its functions are set out as follows: (8) (a) to investigate possible industrial diseases; to make findings as to whether a probable connection exists between a disease and an industrial process, trade or occupation in Ontario; to create, develop and revise criteria for the evaluation of claims respecting industrial diseases; and to advise on eligibility rules regarding compensation for claims. Decisions of the Panel are made by its members who represent labour, management, scientific, medical and community interests. Once the Panel makes a

    Occupational Disease Panel
When the Workers' Compensation Act was amended in 1985, the Occupational Disease Panel was created. The Panel was set-up to provide the Workers' Compensation Board with scientific and policy advice about the recognition and compensation of work-related diseases. The need for the Panel was identified by several individuals, including Professor Weiler, who studied the workers' compensation system and identified systemic under-compensation of disease claims. The Panel is a decision-making body that is composed of representatives of the public and the scientific community and technical and professional persons. It has a small staff and is assisted by consultations with renowned experts in the areas of medicine and science. The statute specifically sets out the following mandate ( Workers' Compensation Act , R.S.O. 1990, c. W. 11, s.95):
