Equine Cushing's Syndrome We also find it very useful to assay insulin in both samples, as insulin concentrations are often two or more times higher than normal in horses with this condition. Grain concentrate rations will elevate insulin for several hours after feeding so do not draw pull samples within 5 hours after grain. Hay and pasture are allowed. Obtain blood samples with minimal stress to the animal to avoid artificially elevating the cortisol level.
  • For the "B.E.T." CUSHING’S SCREEN the following protocol is suggested:
HORMONES AM T Insulin Cortisol PM (8-10 hours later) T Insulin Cortisol REQUIRED: Two ml. serum at each time period. Serum should be separated and sent in a clean, labeled, unbreakable vial. ACTH assay is also available. A normal horse will have level of less than 30 pg/ml. PLASMA, FROZEN, in PLASTIC required.
  • Low Dose Dexamethasone Protocol:
Draw Baseline sample between 4 and 6 pm. Administer 0.04 mg/kg dexamethasone IM (20mg for a 1000 pound horse). Draw second sample 18-20 hours later (Noon the next day).

CUSHING'S SYNDROME IN HORSES
Cushing's Syndrome is well known in humans, cats and especially in dogs. It is also becoming more prevalent in horses, and yet there is little written about identifying it, and caring for horses who suffer from it. It is regarded as incurable in horses, though it is possible that horses can live reasonably happily for a long time whilst suffering from it.

There are times when the steroid control in the body becomes deregulated and excess steroids are present. Clinical signs, in dogs, include drinking too much (polydypsia), urinating a lot (polyuria), eating a lot (polyphagia), hair loss pigmentation of the skin enlarged abdomen due to an enlarged liver lethargy , etc.

Cushing, 1869–1939, American neurosurgeon), the result of excess secretion of adrenal cortical hormones either from an adrenal cortical tumour or adrenal hyperplasia which may be primary or secondary to a tumour of the anterior pituitary gland. The children present with obesity, hirsutism, acne and, if prepubertal, precocious puberty. Once the clinical diagnosis is made, adrenal ultrasound should be performed and if a tumour is identified this is further assessed by MRI. If biochemistry suggess a central cause, MRI of the pituity gland is indicated. See adrenal carcinoma

What's Cushing's
Cushing's syndrome is a collection of clinical signs and symptoms that are the result of hormonal imbalances. These are caused by either overactivity of the pituitary gland or a tumor in the gland itself.
The hallmark of Cushing's is an abnormally long, thick, curly or coarse hair coat that does not shed in the spring, although there are other obvious symptoms (see box below).

Pharmacology and Physiology
The precursor molecule from which all adrenocortical steroid hormones are synthesized is cholesterol [5]. NP-59 is a cholesterol analog which will localize to adrenal cortical lesions such as adrenal adenomas. It is most useful in the evaluation of incidentally detected adrenal masses and hyperaldosteronism. The examination is less useful in the evaluation of Cushings syndrome, which is most commonly due to bilateral adrenal hyperplasia. NP-59 is transported in the circulation like native cholesterol, bound to low-density lipoproteins. These will bind to specific low-density lipoprotein receptors on adrenocortical cells, and also within the liver. After binding to adrenocortical cells the NP-59 is internalized and esterified, but it is not further metabolized [1]. In the liver the agent is metabolically converted into bile salts (as with ordinary cholesterol) and excreted in the bile. The uptake of NP-59 is influenced by adrenocortical secretogogues such as adrenocorticotropic hormone (ACTH) and Angiotensin II which will increase uptake of the tracer [1]. Adrenal uptake of the agent is 50% dependent on ACTH and 10% dependent on angiotensin II. Dexamethasone and exogenous steroids will have the opposite effect, and reduce tracer uptake [1]. Uptake of the tracer is also inversely related to circulating levels of cholesterol, and its uptake may be severely reduced in patients with marked hypercholesterolemia due to competitive inhibition at the binding sites [1].

vWD - Von Willebrands Disease Every good breeder should be screening for vWD. A simple cheek swap sample collection, and the DNA test done by VetGen , will tell you if the tested dog is Clear, a Carrier, or Affected. Affected dogs should never be bred, and many breeders will also spay or neuter all Carriers. A dog who tests as a Carrier can be carefully bred to a Clear dog, and the resultant offspring will either be Clear or Carriers, with no chance of Affecteds. Breeding to an Affected dog will result in Carriers, Affecteds, or Clear, however if a Carrier is bred to an Affected, there will be no Clear dogs produced. To eliminate vWD EVERY breeder must screen their breeding animals, and use the utmost care if utilizing Carriers in their breeding program. Gus is a sweet 13 year-old neutered Welsh Terrier who developed the classic signs of hyperadrenocorticism or Cushings syndrome. He was drinking more, urinating more, had a voracious appetite, disturbed sleep, a distended abdomen, became obese, and developed thin skin and hair coat. His health deteriorated after being treated with oral steroids (prednisone) for allergies and then topical steroids (prednisolone acetate eye drops) following cataract surgery. This case report details how home food and nutritional therapy brought him back to good health without medications, and cured him of allergies! Adrenal hypertrophy Gus had undergone cataract surgery in July, 1999. Chronic corticosteroid therapy had made him a rotund 34 pounds, panting and constantly thirsty. An abdominal ultrasound exam in September, 1999 revealed a large left adrenal gland of 3.3 cm with multiple small cysts at the cranial pole. The right adrenal was not visualized. There was sludging of bile in the gall bladder and the liver was markedly enlarged. Several hyperechoic nodules were noted throughout the liver parenchyma. Surgery, an option
