Definition Return to top A cystic hygroma is a thin-walled, sac-like structure filled with lymph. It occurs most commonly in the head and neck area and often appears as a soft bulge under the skin. Causes, incidence, and risk factors Return to top This condition is a congenital (present before birth) abnormality caused by embryonic lymphatic tissues. Cystic hygromas are tumors derived from these tissues. Symptoms Return to top A common symptom is a neck mass noted at birth, or discovered later in an infant after an upper respiratory tract infection. Signs and tests Return to top Chest X-ray Ultrasound CT scan Treatment Return to top Treatment involves complete removal of the abnormal tissue whenever possible. However, cystic hygromas can often invade other neck structures, making this impossible. In these cases, other treatments have been attempted with only limited success. These include injection of sclerosing agents, steroids, chemotherapy agents, and radiation therapy. Expectations (prognosis) Return to top The outlook is good if surgery can totally remove the abnormal tissue. In cases where complete removal is not possible because of involvement of neighboring nerves or blood vessels, recurrence is common.

D'Alessandro, M.D. Peer Review Status: Internally Peer Reviewed Clinical Presentation: Fifty percent present in the first year and 90% present by two years of age. Usually the lesion is just cosmetically annoying but they can compress the airway and esophagus if they invade the mediastinum. The clinical course is characterized by intermittent or progressive growth, spontaneous regression, hemorrhage, and infection. Etiology/Pathophysiology: The lesion spreads and engulfs adjacent tissues without invading them. Pathology: Multilobular, multiloculated mass composed of cysts measuring 1 mm to 5 cm in diameter that contain serous fluid and are lined by lymphoendothelial tissue. Imaging Findings: Seventy five percent are in the neck, 20% in the axilla, and 5% in the retroperitoneum. Two to three percent of the cervical and axillary masses extend into the mediastinum. DDX: Hemangioma - often calcified, causes skeletal abnormalities, see cardiomegaly and increased pulmonary blood flow if there is AV shunting. Clinical History: This was a term female who had a prenatal diagnosis of a large right sided cystic neck and facial mass as seen on a prenatal ultrasound at 34 weeks gestation. The mother had an elective cesarean section in order to ensure proper management of the airway during delivery. During the cesarean section, the baby's head was delivered onto the mother's abdomen with the umbilical cord still attached. A laryngoscopy was performed. The airway was suctioned and an endotracheal tube was placed. The cord was then clamped. The baby was taken to an adjoining operating room and a direct laryngoscopy was performed which showed a sufficient airway. The baby was then extubated and did well on room air for 3 hours, but then required CPAP for adequate oxygenation. And yes, the negativity coming from the medical community is HORRENDOUS! May I simply suggest providing stories to them, maybe your brother initially, since your sil seems to be shutting down. There are lots of stories out there you have seen some good ones already on this prenatal diagnosis page. There are others, too, that feature families having to deal w/ drs' negativity I suggest the story "Amazing Grace" at www.benotafraid.net as well as the article "Witness to Love" on the same site. Your brother and sil need to know that others have gone thru the same thing and that others have come thru it. Perhaps the baby did not survive, but when the parents were able to take control of the situation, they were at least able to value the time they had w/ their child for the remainder of the pregnancy. They sometimes refer to this as "the gift of time". I really wish your brother and his wife peace and the strength of their convictions as they face this situation!

Previous Message Date Posted: 02:03:32 01/19/03 Sun Hello, My sister in law, Jill and brother in law, Ron, found they were expecting.. which was such a joy.  But unfortunately a few weeks ago (during a sonogram) they found complicatons with the baby.  After seeing a specialist and other tests they were informed that the baby was diagnosed with Cystic Hygroma.   This week after the last sonogram it showed the baby was surrounded by fluid.  The baby's head is lemon shaped due to the fluid pressing against it's temples, there is fluid in the lungs, the kidneys are under developed and they feel it is best for them to just terminate the pregnancy.           Ron is a strong Catholic and very Pro Life,  Jill, I know

Matorin, M.D. The nature of lymphatic malformations (LM) has sparked great interest since they were first described by Redenbacher in 1828. Many of the early researchers believed that lymphatic malformations were neoplasms. Currently, most researchers agree that LM are not neoplastic and have adopted the term "lymphatic malformation" to emphasize this fact. Two major theories of the development of the lymphatic system have been proposed to explain the origin of LM. Sabin proposed that the lymphatic system develops from five primitive sacs which sprout from the venous system. In the head and neck, endothelial outbuddings from the jugular sac spread centrifugally to form the lymphatic system. McClure and Huntington proposed that the lymphatic system develops from mesenchymal clefts in the venous plexus reticulum and spread centripetally toward the jugular sac. LM develop from sequestration or congenital blockage of the primitive lymphatic anlage.
