Equally plain is it there we have not myxoedema to deal with. ... It would seem, then, that we have here to deal with a connective-tissue dystrophy, a fatty metamorphosis of various stages of completeness, occurring in separate regions, or at best unevenly distributed and associated with symptoms suggestive of an irregular and fugitive irritation of the nerve-trunks – possibly a neuritis … Inasmuch as fatty swelling and pain are the most prominent features of the disease, I propose for it the name Adiposis Dolorosa." Brenner, Sarah, and Yael Politi. "Dermatologic Diseases and Problems of Women Throughout the Life Cycle," International Journal of Dermatology 34, no. 6 (June 1995): 369-379. The last century has brought dramatic changes in the lives of Western women. To the normal physiologic developments that take place in menarche, pregnancy, and menopause are added the stresses of combined family life and career. These in turn are exacerbated by longevity and the cultural norms against aging, which have given rise to esthetic interventions that offer options as well as pose new threats. This review surveys dermatologic diseases of women throughout the life cycle, from infancy and childhood through the childbearing years to menopause and old age. The size of the subject precludes complete coverage of all diseases and problems; rather, topics were selected in an attempt to cover a variety of dermatologic issues in women's lives. INFANCY TO ADOLESCENCE Genodermatoses
A small number of genodermatoses are highly predominant in or exclusive to the female gender. They usually present at birth or soon after.

  • Long-term treatment with high doses of corticosteroids leads to the development of truncal obesity and focal fatty deposition. These deposits characteristically are located on the face, the nuchal and truncal areas, and episternally, as well as in the mediastinum and epicardium. We studied a patient with juxta-articular adiposis dolorosa who had L-tryptophan-associated eosinophilia-myalgia syndrome and was treated with high doses of prednisone. This is the first reported case of adiposis dolorosa occurring as a complication of corticosteroid treatment. Alterations of fat metabolism induced by corticosteroid excess may have played a role in the development of this unusual painful syndrome.
Adiposis dolorosa is an uncommon disorder of subcutaneous tissue that was first described by Dercum in 1892 at Jefferson Medical College, Philadelphia, Pa. The understanding of this disease has not progressed very much in the past 98 years. It remains better understood as a clinical entity than as a physiologic or metabolic process.

I submitted my site for your award and was going to sign it before I left, but I got so caught up in the journal of your lovely wife that I totally forgot.I would also like to say that my heart and prayers go out to you. I am not totally done reading it yet. I returned today to start on part 2 and remembered that I had to sign the guestbook. You are a wonderful man for having stuck by your wife's side through all of this. You can tell through each and every word that you write that you loved your wife with all your heart. There are not many people that I could say that about.Your wife's situation is very similar to mine at the present time. I think that is why it has touched me so deeply. I don't know if you want to hear what is going on with me, so if you don't, just stop reading.

Angiolipoma is a common variant which has a proliferation of small vessels scattered throughout the fat. Unlike common lipomas, these tumors are usually painful. Department of Pathology, Stanford University Medical Center. J Am Acad Dermatol 2003 Jan;48(1):82-5 Abstract quote This case report describes a 48-year-old man with multiple spindle cell lipomas of the neck and a dermatofibrosarcoma protuberans (DFSP) with fibrosarcomatous transformation of the chest. The presence of familial and nonfamilial multiple spindle cell lipomas within a single patient is a rare event, with only two reports in the current literature.

What is a lipoma?

What causes a lipoma to form?
How common are lipomas? Will I get more than one lipoma? ... How is a lipoma treated? Edith Jones* remembers the day she first noticed a small, egg-shaped lump under her arm. Worried about breast cancer, she made an immediate appointment with her doctor. To her overwhelming relief, the lump turned out to be something called a lipoma, a noncancerous lump of fatty tissue. Over the next few years, however, she was less than pleased to find a dozen more lipomas cropping up on different parts of her body, including her chest, thighs, and upper arms. "I know that these run in my family," she says. "But I was still hoping to escape them." What is a lipoma? A lipoma is a round, moveable lump under the skin (they range from pea-sized to the size of an egg, or even larger sometimes). Technically, lipomas are a type of tumor, but don't be alarmed by the term they're benign, which means they aren't cancerous. Lipomas are typically small less than one inch in diameter but can grow much larger in some cases. They are soft to the touch and if you manipulate yours with your fingers you may notice that it moves around a bit. Lipomas grow in the layer of fatty tissue beneath the skin, most commonly on the chest, back, shoulders, neck, and upper arms and thighs. The good news is that a lipoma is likely to grow very slowly; the bad news is that if you have one lipoma you may develop several.

What is Dercum's disease?
Morbus Dercum (Adiposis dolorosa) is a chronic disease with mild to intensive pain in the fatty tissue, fatigue and obesity as the most apparent symptoms. But the disease affects the entire body and can present a long list of symptoms. This disorder usually occurs in females between the ages of 25 and 60, but can début in all ages, as well as in younger people and amongst males. It is about 20 times more frequent in females than in males. Dercum's disease, was originally described in the medical literature in 1892 by the American doctor Frances Xavier Dercum.
Symptoms
Typical pain pattern
Other commonly symptom are:
  • Various areas of the body may swell for no apparent reason. The fingers becomes clumsy, you drop things and sometimes the fingers go numb.
  • Disturbed sleep, many have difficulties going to sleep because of the pain, but other forms of sleeping disorders also occur.
  • General fatigue, gets worse with even mild activity.
  • Tendency to become black and blue that seems to arise spontaneously or after alleviated blow.

Brorson/Fagher Article on Dercum's Disease or Adiposis Dolorosa This is an excellent overview article on the disease characterized by painful lipomas (benign fatty tumors). Note that the first part of the article is a biography of Dr. Dercum. Cancer: Causes and Effects Chapter from Dr. Ed Friedlander's online textbook. Adiposis Dolorosa (Dercum's Disease) is mentioned (search for "adiposis"). Even though the lipomas (tumors) are benign, they can cause problems just by being, and he lists a number of examples. Corticosteroid-Induced Juxta-articular Adiposis Dolorosa This article discusses a case of Dercum's Disease in the knee that was apparently caused by corticosteroid usage.

Dercum's disease Dercum's disease is the formation of multiple fatty deposits in various sites. This is a rare disease that predominantly affects women over men in a ratio of 30:1. It is rare for this condition to affect women before the menopause. This condition may be familial but the majority of cases are sporadic. It is characterized by the formation of circumscribed painful adipose tissue deposits in the subcutaneous tissues of the extremities and other parts of the body. The most common site for the deposits is the knees. The size of lesions varies from 0.5 to 5 cm. Patients are often obese. The disease may be associated with emotional instability, fatigue, weakness and, very occasionally, dementia. Histological examination reveals no abnormalities of the adipose tissue. There is no fat necrosis as seen in panniculitis. Giant cell formations may be seen. There is no satisfactory treatment. It has been reported that intravenous lidocaine brought relief in two cases.
