Types Of Diabetic Neuropathy Which Diabetics Get Neuropathy? ... What Can Be Done About Neuropathy Introduction Many diabetics go through life with few if any complications from their diabetes. However in the same way that diabetes may damage a variety of organs in the body, it can produce damage to the peripheral nerves, giving rise to a variety of "peripheral neuropathies". What Are Peripheral Nerves? Peripheral nerves are like electrical wires. They run from the spinal cord to the muscles and from the sensory organs in the skin and joints back to the spinal cord. They work by conducting electrical impulses that either signal a muscle to contract or signal when your skin has been exposed to a stimulus such as touch, warmth or cold. Thus good peripheral nerve function is important for muscle strength, good sensation, good coordination and agility. Peripheral nerves, like all other cells in the body, are living structures that require energy. This is provided by glucose and other substances. It is thought that in diabetes the biochemical milieu of the nerves can be abnormal and this will ultimately lead to nerve fiber damage.

Diabetic neuropathies are a family of nerve disorders caused by diabetes. People with diabetes can, over time, have damage to nerves throughout the body. Neuropathies lead to numbness and sometimes pain and weakness in the hands, arms, feet, and legs. Problems may also occur in every organ system, including the digestive tract, heart, and sex organs. People with diabetes can develop nerve problems at any time, but the longer a person has diabetes, the greater the risk. An estimated 50 percent of those with diabetes have some form of neuropathy, but not all with neuropathy have symptoms. The highest rates of neuropathy are among people who have had the disease for at least 25 years. Diabetic neuropathy also appears to be more common in people who have had problems controlling their blood glucose levels, in those with high levels of blood fat and blood pressure, in overweight people, and in people over the age of 40. The most common type is peripheral neuropathy, also called distal symmetric neuropathy, which affects the arms and legs.
Causes
The causes are probably different for different varieties of diabetic neuropathy. Researchers are studying the effect of glucose on nerves to find out exactly how prolonged exposure to high glucose causes neuropathy. Nerve damage is likely due to a combination of factors:

THE INFLUENCE OF MOMORDICA CHARANTIA FRUIT JUICE ON PERIPHERAL NERVE STRUCTURE IN EXPERIMENTAL DIABETES Ahmed I. , Sharma A.K., Samad P.A. Faculty of Med., United Arab Emirates University, Al Ain, UAE Momordica charantia fruit is traditionally used as a vegetable and is known to have hypoglycemic effect in human and experimental diabetes. Ultrastructural observations were made on myelinated fibers in the tibial nerve of streptozotocin-diabetic rats. Four groups of male Wistar rats, aged 12 weeks were studied over a period of four months: untreated diabetic rats, diabetic rats treated with M charantia, controls treated with M charantia and age-matched controls. Blood glucose concentrations were significantly reduced in M charantia M charantia treated diabetic animals, these parameters were intermediate between untreated diabetics and age-matched controls without showing significant difference when compared with either group. The axon to fiber ratio ('g'ratio) did not differ between any of the experimental groups. In conclusion, the administration of M charantia fruit juice reduced blood glucose levels and partially corrected the structural abnormalities of peripheral nerves in experimental diabetes.

    Types of Diabetic Neuropathies Diabetic neuropathies can be classified as peripheral, autonomic, proximal, and focal. Each affects different parts of the body in different ways.
    • Peripheral neuropathy causes either pain or loss of feeling in the toes, feet, legs, hands, and arms.
    • Autonomic neuropathy causes changes in digestion, bowel and bladder function, sexual response, and perspiration. It can also affect the nerves that serve the heart and control blood pressure. Autonomic neuropathy can also cause hypoglycemia (low blood sugar) unawareness, a condition in which people no longer experience the warning signs of hypoglycemia.
    • Proximal neuropathy causes pain in the thighs, hips, or buttocks and leads to weakness in the legs.
    • Focal neuropathy results in the sudden weakness of one nerve, or a group of nerves, causing muscle weakness or pain. Any nerve in the body may be affected.

Introduction Diabetic neuropathy is the most frequent symptomatic complication of diabetes and encompasses a wide range of abnormalities affecting both the peripheral and autonomic nervous systems. These neuropathies may be disabling and even contribute to mortality in diabetic patients. Other than improved glucose control, current therapies are primarily palliative and supportive. Peripheral Neuropathy Distal, symmetric polyneuropathy is the most common form of diabetic peripheral neuropathy and usually occurs in a stocking-glove distribution. Patients with peripheral neuropathy are at risk for long-term complications of ulceration, infection, and amputation. All diabetic patients should receive an annual foot exam, including visual inspection, peripheral pulses, and sensation. The Semmes-Weinstein monofilament exam is currently the best screening test to detect clinically significant neuropathy. Patient education regarding foot care and daily monitoring for skin breakdown is essential. Pharmacologic therapies for painful peripheral neuropathy include, but are not limited to, tricyclic antidepressants, antiepileptics, and topical capsaicin.

    Diabetic neuropathy is a nerve disorder caused by diabetes. Symptoms of neuropathy include numbness and sometimes pain in the hands, feet, or legs. Nerve damage caused by diabetes can also lead to problems with internal organs such as the digestive tract, heart, and sexual organs, causing indigestion, diarrhea or constipation, dizziness, bladder infections, and impotence. In some cases, neuropathy can flare up suddenly, causing weakness and weight loss. Depression may follow.
