Diverticular Disease
Diverticulosis of the colon is a common condition that afflicts about 50 percent of Americans by age 60 and nearly all by age 80. Only a small percentage of those with diverticulosis have symptoms, and even fewer will ever require surgery.
WHAT IS DIVERTICULOSIS/DIVERTICULITIS?
Diverticula are pockets that develop in the colon wall, usually in the sigmoid or left colon, but may involve the entire colon. Diverticulosis describes the presence of these pockets. Diverticulitis describes inflammation or complications of these pockets.
WHAT ARE THE SYMPTOMS?
The major symptoms of diverticular disease are abdominal pain (usually in the lower left abdomen), diarrhea, cramps, alteration of bowel habit and occasionally, severe rectal bleeding. These symptoms occur in a small percentage of patients with the condition and are sometimes difficult to distinguish from Irritable Bowel Syndrome. Diverticulitis - an infection of the diverticula - may cause one or more of the following symptoms: pain, chills, fever and change in bowel habits. More intense symptoms are associated with serious complications such as perforation, abscess or fistula formation.

    Presentation
    A 65-year-old man presents to the emergency room with lower abdominal pain, pneumaturia and fever. On physical examination, his temperature is 39 degrees Celcius and his pulse is 95. He has moderate left lower quadrant and suprapubic tenderness but no guarding or rebound. His stool is guaiac positive on rectal exam. His white blood cell count (WBC) is 16 K/µl with 80% neutrophils and 5% bands.
    Imaging Findings
    Computed tomography
    Supine and upright plain radiographs of the abdomen are unremarkable. Axial tomograms from a pelvic CT after oral contrast and without intravenous contrast show thickening of the wall of the sigmoid colon with inflammatory changes in the surrounding mesenteric fat ( arrow ) The inflammatory process abuts the superior and posterior aspects of the urinary bladder. Stool, oral contrast and gas are visible within the bladder ( arrow ). There is also a small amount of extraluminal air ( arrow ). Numerous sigmoid diverticula are present.

    Diverticulitis (di-ver-tik-u-LIE-tis) is an inflammation of small pouches called ""diverticula'' that often develop in the wall of the large bowel (colon). Waste from digested food can get trapped in the diverticula and cause swelling and pain.
    Causes
    Frequently no cause can be found. Having a family history of diverticulitis or eating a low-fiber diet may make you more likely to have this problem.
    Signs/Symptoms
    The main symptom is abdominal pain that either comes and goes or lasts all the time. You may also have a fever, feel like throwing up, or develop tenderness in the belly.
    Care
    Your doctor may order x-rays and tests. If the diverticula are infected, you may be given antibiotics. If you have a bad infection or if you need surgery to remove part of the colon, you may need a stay in the hospital.

    Small protruding sacs of the inner lining of the intestine (diverticulosis) can develop in any part of the intestine. They are most common in the colon. These sacs, called diverticula, occur with increasing frequency after the age of 40. When they become inflamed, the condition is known as diverticulitis.
    Diverticulitis is an inflammatory condition where the area around the diverticula becomes irritated and attracts immune cells. It affects 10% to 20% of those with diverticula at some time. These areas can become infected. Risk factors are older age and history of diverticula or prior episodes of diverticulitis. Eating a high-fiber diet and avoiding seeds and nuts may prevent episodes.
