Diverticulitis
Definition: Diverticulitis is inflammation of an abnormal pouch (diverticulum) in the intestinal wall, usually found in the large intestine (colon). The presence of the pouches themselves is called diverticulosis.
Causes, incidence, and risk factors: Small protruding sacs of the inner lining of the intestine (diverticulosis) can develop in any part of the intestine. They are most common in the colon. These sacs, called diverticula, occur with increasing frequency after the age of 40. When they become inflamed, the condition is known as diverticulitis.
Diverticulitis is an inflammatory condition where the area around the diverticula becomes irritated and attracts immune cells. It affects 10% to 20% of those with diverticula at some time. These areas can become infected. Risk factors are older age and history of diverticula or prior episodes of diverticulitis. Eating a high-fiber diet and avoiding seeds and nuts may prevent episodes.

Wat zijn divertikels?
Divertikels zijn uitstulpingen van de de slijmvlieslaag door de spierlaag van de darm. Theoretisch kunnen divertikels voorkomen over de gehele lengte van het maagdarmkanaal. Het meest frequent bevinden ze zich echter in de dikke darm ( het colon ) en vooral in het laatste deel ervan ( Diverticulosis (het voorkomen van divertikels in de dikke darm) wordt beschouwd als de meest voorkomende afwijking van de dikke darm in de westerse wereld. De kans dat iemand ooit in zijn leven divertikels krijgt wordt op ongeveer 40 tot 50% geschat. Slechts in zo'n 5 tot 20% van de gevallen geven divertikels aanleiding tot klachten, wat zich meestal uit in een ontsteking van de dikke darm, ookwel diverticulitis genoemd.
Oorzaak van diverticulosi s
De orzaak voor het ontstaan van divertikels is nog niet geheel duidelijk. Wel lijkt er een relatie te bestaan met een tekort aan vezels in de dagelijkse voeding . Het is bekend dat niet alleen een tekort aan vezels maar ook een gebrek aan lichaamsbeweging gepaard gaat met vertraagde passage van ontlasting door de dikke darm en daardoor waarschijnlijk meer kans geeft op diverticulose. In zo'n divertikel kunnen dan namelijk onverteerde voedselresten blijven zitten en daarmee de nauwere hals afsluiten. Door de druk op het divertikel kan de bloedvoorziening van de wand afnemen en daardoor ook dunner worden. Tevens zal door het "blijven steken" van die onverteerde voedselresten ook een overgroei plaatsvinden van bacteriën. Hierdoor ontstaan een hoger risico op een ontsteking van de darm.

The usual symptoms of diverticulitis include: fever, lower abdominal pain, and lower abdominal tenderness. Most patients with an acute episode of diverticulitis severe enough to require hospitalization can be treated with intravenous fluids, bowel rest, and broad spectrum antibiotics and analgesics. Patients who have more than one episode of acute diverticulitis requiring hospitalization should be considered for surgery. In this instance, a section of the bowel needs to be resected or cut away and joined back into continuity. The Overview

Diverticulitis, the risks of which increase with age, is the inflammation of a pocket on the large intestine. Painful attacks of diverticulitis can feel like appendicitis. The good news is that many experts believe adequate fiber may be able to help prevent this condition.
The width of the large intestine decreases as waste moves through. When stomach contents enter the large intestine, they are largely liquid. But as material passes through the intestine toward the colon, most of the moisture is removed. And, as we age, the outer wall of the large intestine thickens, requiring more pressure to move waste out of the body. Small pouches or pockets, called diverticula, can develop where force is required to move stool out of your body. When one of these pockets becomes inflamed, diverticulitis develops.
The Role of Dietary Fiber
Almost half of Americans over 60 have these diverticula, but only 20 percent of these people develop diverticulitis. Many experts believe much diverticular disease can be helped by a high-fiber diet and fiber supplementation where needed. Fiber creates bulkier stool which is easier to eliminate.

What Is Diverticulitis?

Nearly half of Americans older than age 60 have small, bulging pouches (diverticula) in their digestive tracts — a condition known as diverticulosis. Although diverticula can form anywhere, including your throat, stomach and small intestine, most occur in your large intestine (colon) — especially on the left side of the colon just above the rectum. Because these pouches seldom cause any problems, you may never know you have them.
Sometimes, however, one or more pouches may become inflamed or infected, causing severe pain, fever and nausea. When diverticula become infected, the condition is called diverticulitis. Mild cases of diverticulitis can be treated with changes in your diet, rest and antibiotics. But more serious cases may require surgery to remove the diseased portion of your colon. Occasionally, you may develop complications that require emergency surgery.
Fortunately, only 15 percent to 20 percent of people with diverticulosis ever develop diverticulitis. Best of all, you can help prevent both types of diverticular disease simply by including more high-fiber foods in your diet.

Signs and Symptoms
Diverticulitis can feel like appendicitis, except you'll have pain in the lower left side of your abdomen, instead of the lower right. The pain is usually severe and comes on suddenly, but sometimes you may have mild pain that becomes worse over several days, fluctuating in intensity. You may also have abdominal tenderness, fever, nausea, and constipation or diarrhea.

What are Diverticulosis and Diverticulitis? In the colons of most people, small pouches bulge outward through weak spots, like an inner tube pushing through weak places in a tire. Each pouch is called a diverticulum (pouches are diverticula). The condition of having diverticula is called diverticulosis. About half of all Americans age 60 to 80, and almost everyone over age 80, have diverticulosis. When the pouches become infected or inflamed, the condition is called diverticulitis. This happens in 10 to 25 percent of people with diverticulosis. Diverticulosis and diverticulitis are also called diverticular disease
Symptoms of Diverticulosis and Diverticulitis Diverticulosis typically causes little or no discomfort, and most people with diverticulosis are unaware of their condition. Cramps, bloating, and irregular bowel movements may be mild symptoms. Diverticulitis usually causes pain in the lower left abdomen. If diverticulosis is caused by infection, then a person may also experience fever, chills, nausea, vomiting, cramping, and constipation. Moreover, numerous complications can develop from chronic diverticulitis, including diffuse peritonitis, localized abscess, fistulas (such as a colovesical or colovaginal), and obstruction of the colon.
Treating Diverticular Disease Diverticulosis is most often caused by inadequate dietary fiber; it is one of the most common diseases in the US, England and Australia, and results from modern-day low-fiber diets. Many people are able to improve diverticular disease by increasing the quantity of fiber in their diets, such as by increasing their consumption of whole grains (such as whole wheat as opposed to refined white flour), fresh vegetables, and fresh fruits. Treatments for acute diverticulitis may also include pain medications, oral or intravenous antibiotics, a liquid diet, bed rest, and medications to stop muscle spasms in the colon. In severe cases, surgery may be required.

Inflammation of the diverticular mucosa with attendant complications of peridiverticulitis, phlegmon of the bowel wall, perforation, abscess, or peritonitis, with or without obstruction, fistulas, and bleeding.
Symptoms, Signs, and Diagnosis
Usually, the presenting symptoms of diverticular disease are pain, local tenderness in the left lower quadrant of the abdomen, and fever. In the presence of known diverticulosis, diagnosis of diverticulitis is almost certain. Diverticulitis may present with free perforation or with abscess formation, which produces symptoms and signs of peritonitis. Acute appendicitis and colon or ovarian cancer are most commonly confused with diverticular disease. Exploratory laparotomy may be necessary for definitive diagnosis and proper therapy. Extracolonic manifestations of diverticulitis with pericolic abscesses include arthritis and pyoderma gangrenosum.
Complications
Obstruction may occur as a complication of diverticulitis (see Mechanical Intestinal Obstruction in Ch. 25).

Studies shows that diverticula probably result from high intraluminal pressure on areas of weakness in the GI wall, where blood vessels enter. Diet, such as highly refined foods, may be a contributing factor. The like of fiber reduces fecal residue, narrows the bowel lumen, and leads to higher intra - abdominal pressure during defecation.
In this disorder, bulging pouch like herniations in the GI (gastrointestinal ) wall push the mucosal lining through the surrounding muscle. Diverticula occur most commonly in the sigmoid colon, but they may develop anywhere, from the proximal end of the pharynx to the anus. Other typical sites are the duodenum, near the pancreatic border or the ampulla of Vater, and the jejunum.
Diverticular disease of the ileum is the most common congenital anomaly of the GI tract. Diverticular disease has two clinical forms. In Diverticulosis
