LASIK eye surgery can often correct the common vision problems of near- or far-sightedness, but there are side effectsone of them being eye dryness. Now a new study shows that LASIK patients who have dry eyes going into surgery, while benefiting from it, may also have more-severe eye dryness after the procedure than other patients. Dr. Ikuko Toda, of the Minamiaoyama Eye Clinic in Tokyo, Japan, and colleagues report the findings in the August issue of the Archives of Ophthalmology. During LASIK (for laser-assisted in situ keratomileusis), a doctor attempts to clear blurry vision by removing a thin layer of tissue from the cornea in order to reshape it.

Hi, I wonder if you can help me, I had lasik surgery 5 weeks ago and for the first 14 days the results were better than 20/20 my prescription was -6 and -5.5 with average levels of astigmatism. 5 weeks later my vision has reduced to -0.5 astigmatism and -0.5 prescription (pos -1). I have been told this will improve over the next six weeks as they tend to fluctuate but at the moment they seem to be getting worse.I have also got quite bad night and ghost vision. Is this level of regression common after such a short time and what are the chances of them going back to 20/20? what sort of time frame should I be looking at to get better night vision and what are the chances of getting rid of the ghosting? Keratoconjunctivitis sicca (KCS) Keratoconjunctivitis sicca (KCS) or "dry eye" describes the changes in the eye which result from lack of tear production. To understand "dry eye" it is helpful to know how tears help keep the cornea healthy. The cornea is the optically clear portion of the eye that allows entry of light into the eye. Like all living tissue, the cornea requires a supply of oxygen and energy to remain healthy. Oxygen and nutrients are supplied to most tissues by the blood that moves through the area in blood vessels. The healthy cornea has no blood vessels, if it did it wouldn't be clear, so the oxygen and nutrients are supplied through the three-layered 'tear film.' The outer most layer of the tear film is an oily layer supplied by glands in the eyelids. This layer helps prevent evaporation of the next aqueous layer. The middle layer is the liquid aqueous layer produced by the main tear gland and a gland in the third eyelid. This is the layer that is decreased in dry eye. The innermost layer in direct contact with the cornea is a mucous layer produced by glands located in the folds of the eyelid. The mucus layer helps the aqueous layer adhere to the surface of the cornea.

If your eyes itch, especially as the day wears on, if you have excess mucus discharge upon awakening, or if you suffer from burning, chronic redness, or foreign body sensation (as if you had invisible sand in your eyes), the chances are that you are one of the ten million Americans who suffer from some form of the dry eye syndrome. If we count those who exhibit only marginal symptoms, the number of people afflicted may surpass thirty million. The majority of these patients are middle aged or elderly women but the disease is not limited to them as men and youngsters are also seen in the clinic with such a condition. The causes can be multitudinous but the clinical signs are often the same.
DIAGNOSIS
The common clinical sign for the various dry eye conditions is the vital staining of the ocular surface that often forms specific patterns. Traditionally, the vital dye employed for the diagnosis of keratoconjunctivitis sicca is Rose Bengal even though the dye is quite irritating, especially to dry, delicate eyes. Recently, another vital dye Lissamine green has become available that forms identical staining patterns on the eye surface but is not irritating and the staining can be distinguished better in red, inflamed eyes. Slit-lamp examination, assessment of tear meniscus, Schirmer test of lacrimation, the concentration of certain tear gland proteins such as lysozyme or lactoferrin, and tear film break-up time are other diagnostic methods that can be used to confirm the diagnosis of dry eye states.

These symptoms include dry, red, gritty, and even watery eyes. Often, Dry Eye sufferers report the feeling of something foreign within the eye or eye strain. Some people do not produce enough tears to keep the eye comfortable. The moisture level in the eye is maintained by the balance of tear production and tear loss through drainage and evaporation. When this balance is not sustained, dry spots appear on the surface of the eye and cause irritation. Tears are produced by two different methods. One method produces tears at a slow, steady rate and is responsible for normal eye lubrication. The other method produces large quantities of tears in response to eye irritation or emotions.

Many people experience an inadequate tear film or eye discomfort some time in their lives. In may be in an airplane, a dry, overheated room or a dusty workplace. But there may be more than 10 million Americans who have a significant dry eye condition that may be related to other symptoms. The classic Sjogren's-Associated dry eye affects up to two percent of the population. This triad of symptoms includes: dry eye, dry mouth and arthritis. Dry eyes can be caused by medication, dehydration, inflammation of the eyelids and/or skin, previous eye surgery, systemic diseases such as rheumatoid arthritis, thyroid disease, lupus, sarcoidosis and even poor blinking habits (such as staring at a computer). Tears at not just water. They have three separate components: 1.) Oil (from the Meibomian oil glands in the eyelids); 2.) Mucous (from the goblet cells deep inside the eyelid); 3.) watery tears (the "aqueous" tears from the lachrymal gland and accessory lachrymal glands located in the conjunctiva of the eyelids).
