Abnormally short height in childhood may be due to dysfunction of the pituitary gland causing underproduction of growth hormone . A variety of genetic syndromes, a tumor in the pituitary gland , the absence of the pituitary gland, or trauma are some of the known causes of this condition, but in most cases no underlying cause of the deficiency is found.
Growth retardation may become evident in infancy and persists throughout childhood. The "growth curve," demonstrated by plotting sequential height measurements on a standardized growth chart, may range from flat (no growth) to very shallow (minimal growth). Normal puberty may or may not occur, depending on the degree of pituitary insufficiency (inability of the pituitary to produce adequate hormone levels other than growth hormone).

Dyschondrosteoses Also known as diastrophic dysplasia or epiphyseal dysostosis, diastrophic dwarfism is another example of rhizomelic dwarfism. It is autosomal recessive in transmission. The term diastrophic characterizes the twisted scoliosis in this disease. The suspect gene is the diastrophic dysplasia sulfate transporter. This is a rare form of rhizomelic dwarfism, and the incidence is unknown. Overall, cartilage is malformed, resulting in several characteristic radiographic abnormalities. This cartilage abnormality often leads to death from tracheomalacia. If the patient survives infancy, there is a normal lifespan. Scoliosis is not present at birth but often is progressive, especially in the early teens. Treatment of the scoliosis includes bracing and occasionally, spinal fusion. Progressive cervical kyphosis can also occur with subluxation of the cervical spine which can result in spinal cord compression. The characteristic deformity is the "hitchhikers" thumb. At birth, the spine may only be slightly scoliotic. However, there is relentless development of a severe kyphoscoliosis of the lumbar spine. As in achondroplasia, a broad pelvis is encountered with narrowed interpediculate distances in the spine. 25 - 50 % of patients have a cleft palate. Due to inflammation, the ear enlarges, resulting in a "cauliflower" deformity.

The disease is lethal, usually shortly after birth due to respiratory failure. The incidence is approximately 1 in 10,000 births and is the most common lethal bone dysplasia. There are several characteristic radiographic findings, illustrated above. First, the disease is rhizomelic, with relative shortening of the humeri and femora. The femora are curved and in a classic "telephone-receiver" configuration. The ribs are short and horizontal and the thoracic cage is hypoplastic. The iliac wings are small and squared. Contact: Ronda Wendler 713/792-4257 (phone) 713/990-2649 (pager) E-mail: rwendler@dct4.hsc.uth.tmc.edu HOUSTON(6/30/95)The gene that causes a condition known as pseudoachondroplasia, a form of dwarfism, has been identified by a research team led by Dr. Jacqueline Hecht of The University of Texas-Houston Health Science Center . Identification of the gene will allow scientists to make preliminary inroads into understanding and someday curing the condition. The discovery is reported in the July 1 issue of the journal Nature Genetics. The abnormal gene is located on chromosome 19 and is called the cartilage oligomeric matrix protein (COMP) gene for its role in making this particular form of protein, which acts as a building block in the body. The COMP gene is normally present in all individuals, and mutations in this gene cause pseudoachondroplasia. "With the identification of the pseudoachondroplasia gene we plan to study the normal interactions of this protein in order to understand how the abnormal protein disrupts normal bone growth and development," said Hecht, associate professor in the UT-Houston Medical School division of medical genetics.

Dwarfism may be associated with a skeletal dysplasia or may be due to failure of growth, e.g. pituitary failure with absence of growth hormone. The investigation of dwarfism depends on the suspected cause. If a skeletal dysplasia is suspected, a skeletal survey is indicated. If a central cause is suspected, MR imaging of the pituitary gland is performed. The term "dwarfism" is now generally replaced by "short stature". Those who reach adulthood do not suffer any lessening of their mental or sexual abilities, and may have unusual muscular strength. Achondroplasia does not significantly shorten life span. There are more than 320 medical conditions known as dwarfism. A detailed study of dwarfism is outside the scope of this Web site. There are several good sites that address this medical condition.
