Causes, incidence, and risk factors Return to top The pituitary gland is a small, oval gland at the base of the brain. It makes several hormones that control the function of other glands in the body including the thyroid gland, adrenal glands, and gonads (ovaries or testes). The pituitary gland is partly surrounded by a bony structure called the sella turcica (Turkish saddle). When the pituitary gland is not visible on CT or MRI scans of the sella turcica, the condition is referred to as the empty sella syndrome. Primary empty sella syndrome occurs when a small anatomical defect above the pituitary gland increases pressure in the sella turcica and causes the gland to flatten out along the walls of the sella. When the sella is empty because the pituitary gland has regressed following an injury (e.g., surgery, head trauma, radiation therapy), the condition is called secondary empty sella syndrome. Primary empty sella syndrome is most often an incidental finding on radiological imaging of the brain. Pituitary function is usually normal, and patients do not have any symptoms. The hormone prolactin is mildly elevated in 10-15 % of patients, and the elevated prolactin may interfere with normal function of the testes or ovaries. Medications that suppress prolactin production (e.g., bromocriptine) are effective in correcting the problem.

The most common cause is a large opening in the diaphragma sellae, a membrane which sits on top of the pituitary and protects it in its socket within the bones at the base of the skull. When this opening is large, the spinal fluid pressure is transmitted to the pituitary and flattens it out within the sella. Generally, in this situation, pituitary function is normal, but a number of patients have headaches, mild hyperprolactinemia, galactorrhea, and irregular menses. In most cases, however, the pituitary functions normally as evidenced by normal thyroid functions, normal tests of adrenal function, normal somatomedin-C levels, and regular menses. Some patients have empty sella syndrome as a result of other processes such as neurosarcoidosis pituitary tumors that have degenerated, etc. Rare patients have a congenital empty sella and a coexisting pituitary tumor. Symptoms unusual facial features increased bone density headaches vision problems Diagnosis x-ray CT (CAT) scan Magnetic resonance imaging (MRI) Treatment Unless the disorder results in other medical problems, treatment is symptomatic and supportive. Subject: Empty Sella Syndrome Topic Area: Neurology - General Forum: The Neurology and Neurosurgery Forum Question Posted By: KYgal on Wednesday, July 24, 2002 Two years ago, I had surgery to revise a VPshunt (orginialshunt placement in 1987)and two weeks later fenestration of a ventricular cyst via neuroendoscopy. A few weeks later, I ended up with a cerebral abcess and subseqent removal of shunt and 8 wks of Vancomycin IV. MRI following treatment showed empty sella syndrome and microcystic fibromylasia where shunt had been. The neurosurgeon said that the empty sella was nothing to be concerned about. Since the surgery, I have had basically no libido, tired all the time, joints ache, difficulty concentrating, poor memory, and now headaches again. Another MRI was done by family Dr. to see if cyst had returned. The MRI showed the same as last. Seen by a neurologist. No one concerned about empty sella. Perhaps it is because I am middle age, overweight, and have hypertension? (yes, I have read about it!) However, I did not have the empty sella syndrome before the surgery and infection, nor did I have these symptoms. Should I be concerned? PS> I am usually a very active professional who enjoys hiking, camping, and traveling in my spare time. I now find everything very challenging and have no spare time, because it takes me so long to get things done!

My problem was not in my brain. My problem was disc problems in the Cervical and Thoracic areas. After my last surgery, I continued to worsen, this time with symptoms which appeared to be autonomic in nature. I have developed all kinds of strange symptoms, some seeming related to the heart. A new brain MRI was done. It shows a Empty Sella. Where did my pituitary gland go to? Do surgeries cause your pituitary gland to disappear and present as an Empty Sella? Could this be causing all these autonomic problems? I have checked your info files and it says that the pituitary gland disappears.
