Endocrine Abstracts previous abstract next abstract Abstract BENIGN INTRACRANIAL HYPERTENSION - RARE CAUSE OF EMPTY SELLA SYNDROME Further enquiry revealed past neurological events: he had presented in 1981 with an idiopathic sixth nerve palsy (CT scan, carotid angiography normal) and in 1991 with benign intracranial hypertension thought to be secondary to a sagittal sinus thrombosis: he was treated with dexamethasone but it proved necessary to insert a lumbar peritoneal shunt. An association between benign intracranial hypertension and the empty sella syndrome is rare but recognised. A causative mechanism has been proposed.   • No specific treatment if pituitary function is normal Medication to lower prolactin levels (e.g., bromocriptine) if the prolactin level is high and interfering with gonadal function Secondary empty sella syndrome: therapy is directed at replacing hormones that are deficient as a result of abnormal pituitary gland function. Expectations (prognosis):
  • Primary empty sella syndrome does not have adverse health consequences or alter life expectancy. The specific cause of pituitary gland injury and the effects of hypopituitarism influence prognosis in secondary empty sella syndrome.

Complications:

Empty sella is a radiological diagnosis based on CT or MR investigation. Either a normal sized (empty sella) or enlarge sella (empty enlarged sella) presents partly or totally filled with cerebrospinal fluid. The radiological diagnosis does not mean a pathological situation in every instance. Many patients present without specific symptoms and the diagnosis is made by chance. Empty sella syndrome is the pathological variant of a radio logically verified empty sella. Primary empty sella is an idiopathic form of an empty sella which occurs in the absence of prior pituitary surgery or radiation therapy or medication with DOPA agonists. Secondary empty sella occurs as a result of surgical resection or irradiation of a sellar expansion. Anatomy: The disphragma sellae normally forms a circular fold that constitutes a roof for the sella turcica with only a small central opening for the passage of the pituitary stalk.

The pituitary may be completely flattened as a pancake at the bottom of the sella as a result. Clinical hypopituitarysm accompanies this. Adenoma s nevertheless may develop from such compressed, atrophic pituitary glands.

Pituitary fossa enlargement is a result of a communicating extension of the subarachnoid space into the pituitary fossa, which promotes remodeling and enlargement of the bony sella and flattening of the pituitary gland against the sella floor. An empty sella may occur as a result of:
  • Primary empty sella - a congenital diaphragmatic defect
    This is a fairly common incidental finding and is usually associated with only minor disturbances of pituitary function.
    Mild hyperprolactinemia
  • Secondary empty sella - as a result of damage to the diaphragm by surgery, radiotherapy, or pituitary tumor infarction.
    If the empty sella is associated with destruction of the pituitary gland, complete or partial hypopituitarism is the outcome.
    Measurement of serum pituitary and target organ hormones is necessary to establish the diagnosis of hormone deficiency and to determine the central (hypothalamic or pituitary) cause of the dysfunction (e.g., serum thyrotropin and T4 ; corticotropin, cortisol; LH and testosterone).     Varanasi Introduction Top Empty sella refers to the radiological appearance of an enlarged or deformed sella turcica that is partially or completely filled with cerebrospinal fluid. In the earlier days, the radiographic appearance of an enlarged sella turcica was frequently equated with the presence of an intra or perisellar neoplasm. It was found, however, in some of these patients on pneumoencephalography, that the enlarged sella was not occupied by a tumour mass, but a significant amount of air was reaching the sella indicating that the subarachnoid space was extending well below the clinoid processes. To describe this finding, several different terms were used such as intrasellar arachnoid diverticulum, intrasellar cysts, deficient sellar diaphragm, intrasellar cistern . Busch , while reporting the presence of empty sella in 5 percent of normal subjects on autopsy studies, used the term "empty sella" to describe this condition. However, the term empty sella in fact, is a misnomer as the sella is not completely empty, but the pituitary is always present both anatomically and functionally, though often it is displaced downwards and compressed by CSF pressure. An empty sella thus is usually an incidental anatomic finding and occasionally it results in abnormal pituitary function. Regardless of the size, the sella is completely or partially filled with CSF. It can occur at any age and in either sex. However, it is more common in women and increases in frequency with age

An enlarged sella is seen on skull x-ray, and a low density CSF filled cavity occupying most the sella is seen on CT. The remaining space contains pituitary gland, tumour or both. The condition is thought to arise from a change in CSF circulation with subsequent cisternal herniation. There is progressive compression of the pituitary parenchyma and reduction in portal vessel perfusion. It is usually found in obese, often hypertensive, females. Presenting symptoms tend to non-specific - frequent headache and transient visual problems. Endocrine disturbance is unusual but when present, includes amenorrhoea in one - third of cases, usually due to hypogonadotropism. However, many patients enjoy normal pituitary function. There is a risk that aneurysms may develop within the dilated sella which should be examined for by angiography. Pituitary replacement therapy is required. Occasionally, it may be necessary to decompress the chiasm surgically.
