A friend of mine without health insurance was just diagnosed with epididymitis and orchitis. He initially presented with fevers, headache, blood in the urine and was told by an emergency room doctor that he had a urinary tract infection and was given a ten day dose of Bactrim. After a day or so he developed pain and swelling in the testicles. He then went to a free clinic and was diagnosed with epididymitis and orchitis. He was given a shot of ceftriaxone and a gram or two of azithromycin to drink. This was a few days ago. Last night he had a fever of 104 degrees F. and threw up. He still has pain in the testicles. Is the situation getting worse and should he seek additional medical attention?

: I'm a 33 year old white male who first saw a Urologist 7 years ago for chronic bact. prostatitis and epididymitis. My urine flow/stream was always way below average for someone my age. After chronic repeat infections and a poor stream, he performed video-urodynamic testing on me. I can't pronounce the diagnosis, in short he said it was a nuerogenic bladder. He recommended self-cath, which I don't do because it is very hard on my urethra. I continue to suffer from chronic epididymitis, the cord is always swollen and painfull, whether infection is present or not. : Can I truly have a nuerogenic bladder w/out any presence of other nuerologic disease? Is this a muscle or nerve problem? My Dr. scoped me and ruled out a stricture. He said the neck/sphincture of my bladder, and rectum, "go the wrong way".? I still have a hard time holding urine and voiding completely. The epididymitis is still chronic. Are there other diagnostic tests I should under-go? Please help. THANK YOU.

Description
Epididymitis is the most common cause of pain in the scrotum. The acute form is usually associated with the most severe pain and swelling. If symptoms last for more than six weeks after treatment begins, the condition is considered chronic. Epididymitis can occur any time after the onset of puberty but is most common between the ages of 18 and 40. It is especially common among members of the military who exercise for extended periods without emptying their bladders. Factors that increase the risk of developing epididymitis include:
  • infection of the bladder, kidney, prostate, or urinary tract
  • other recent illness
  • narrowing of the urethra (the tube that drains urine from the bladder) use of a urethral catheter

Epididymitis is an infection of the epididymis. The epididymis is a soft, coiled tubular structure on the back of the testicle.
What is going on in the body?
Infections generally spread to the epididymis from the bladder or the urethra. The urethra is the tube that allows urine to pass from the bladder to the outside of the body. In young children or older men, bacteria that cause urinary tract infections are often the cause. Sexually transmitted diseases are more likely to be the cause in sexually active young men.
Infection in the epididymis causes severe pain and swelling. The infection may spread to the testicle. When both structures are inflamed, the condition is known as epididymo-orchitis. It is rare that an infection will occur in the testicle alone. The bacteria will usually infect the epididymis first.
What are the signs and symptoms of the disease?
Symptoms of the condition include:
  • pain in the scrotum. This can range from moderate to severe.

Epididymitis caused by sexually transmitted enteric organisms (e.g., Escherichia coli Although most patients can be treated on an outpatient basis, hospitalization should be considered when severe pain suggests other diagnoses (e.g., torsion, testicular infarction, or abscess) or when patients are febrile or might be noncompliant with an antimicrobial regimen.
Diagnostic Considerations
Men who have epididymitis typically have unilateral testicular pain and tenderness; hydrocele and palpable swelling of the epididymis usually are present. Testicular torsion, a surgical emergency, should be considered in all cases, but it occurs more frequently among adolescents and in men without evidence of inflammation or infection. Emergency testing for torsion may be indicated when the onset of pain is sudden, pain is severe, or the test results available during the initial examination do not support a diagnosis of urethritis or urinary-tract infection. If the diagnosis is questionable, a specialist should be consulted immediately, because testicular viability may be compromised. The evaluation of men for epididymitis should include the following procedures.

The progression of epididymitis usually is gradual in nature, with symptoms often peaking within 24 hours of onset. Initially, the patient may note abdominal or flank pain because cellular inflammation typically begins in the vas deferens. As the inflammation descends to the lower segment of the epididymis, the patient notes discomfort localized to the scrotum. Younger patients, or any patient with a sexually transmitted epididymitis, may note symptoms related to urethritis. A recent history of instrumentation or urinary tract infection is more common in the older patients.

All urologists know that and insistence on part of the patient is not to the patient's benefit. Epididymitis Epididymitis is a disease of the tubules surrounding the testicle and is frequently a symptom of sexually transmitted disease. After sperm are produced in the testicle, they are secreted into the epididymis where they gain the energy for movement and the ability to fertilize an egg. Infection of the epididymis produces painful swelling of the testicle that may last for weeks despite treatment. Epididymitis typically results from infection with the common sexually transmitted bacteria: chlamydia and gonorrhea. Treatment of epididymitis requires antibiotics and is similar to that for other chlamydia infections such as NGU There is no doubt that epididymitis can affect a man's fertility, but no one knows how serious this loss of fertility typically is. Men who develop epididymitis frequently have decreased numbers of sperm during and shortly after their infection, but experts think this is usually only a temporary problem. Whether some men suffer permanent loss of fertility from epididymitis is still unknown, but much evidence suggests that such fertility loss does occasionally occur. This problem is similar to the risk to fertility that women face from pelvic inflammatory disease, often known as PID. The causes are similar and the prevention is identical: avoid becoming infected with sexually-transmitted bacteria that can cause scarring of either male or female tubes. For most couples, condoms remain the best way to avoid infection and keep the highest possible level of fertility.

Recommendations The epididymis is a structure of tubules that transport sperm from the testicle. It is palpable on physical examination on the posterior or rear of the testicle. The tubules within the epididymis merge to form the vas deferens. The vas deferens is the tube that is cut during a vasectomy to prevent sperm from mixing with the remainder of ejaculatory fluid.
Epididymitis is an inflammation or infection of the epididymis. This can occur as the result of trauma or infection. Frequently, the cause is not determined. Symptoms of epididymitis include discomfort in the region of the testicle. This pain and tenderness is often referred into the groin and occasionally into the abdomen. Swelling and firmness in these tissues can also be a component of epididymitis. As with any change in testicular exam or sensation, this should be examined by a physician.

Definition
Epididymitis is inflammation or infection of the epididymis. In this long coiled tube attached to the upper part of each testicle, sperm mature and are stored before ejaculation. Description Epididymitis is the most common cause of pain in the scrotum. The acute form is usually associated with the most severe pain and swelling. If symptoms last for more than six weeks after treatment begins, the condition is considered chronic. Epididymitis can occur any time after the onset of puberty but is most common between the ages of 18 and 40. It is especially common among members of the military who exercise for extended periods without emptying their bladders. Factors that increase the risk of developing epididymitis include:
  • infection of the bladder, kidney, prostate, or urinary tract
  • other recent illness
  • narrowing of the urethra (the tube that drains urine from the bladder) use of a urethral catheter
Causes and symptoms Although epididymitis can be caused by the same organisms that cause some sexually transmitted diseases (STDs) or occur after prostate surgery, the condition is generally due to pus-generating bacteria associated with infections in other parts of the body.

Die Entzündung des Nebenhodens ist die häufigste Ursache für eine Hodenschwellung.
Der Nebenhoden wird Epididymis genannt. Aufbau und Funktion der Nebenhoden werden hier beschrieben. Die akute Nebenhodenentzündung ist die häufigste Ursache einer akuten Hodenschwellung. Während bei Kindern noch die Hodentorsion dominiert, wird die Diagnose der Entzündung mit dem Alter immer wahrscheinlicher, wobei vor allem Männer in den Zwanzigern, sowie zwischen dem 40. und 60. Lebensjahr erkranken.
