Definition Erythema multiforme is a skin disease that causes lesions and redness around the lesions. Description Erythema multiforme appears on the skin and the mucous membranes (the lining of the mouth, digestive tract, vagina, and other organs). Large, symmetrical red blotches appear all over the skin in a circular pattern. On mucous membranes, it begins as blisters and progresses to ulcers. A more advanced form, called Stevens-Johnson syndrome, can be severe and even fatal. Causes and symptoms Erythema multiforme has many causes, most commonly are drugs. Penicillin, sulfonamides, certain epilepsy drugs, aspirin, and acetaminophen are the most likely medication-induced causes. Erythema multiforme can also be caused by certain diseases. Herpes virus and mycoplasma pneumonia are likely infectious causes. Diagnosis The appearance of the rash is sufficiently unique to identify it on sight. Having identified it, the physician will determine the underlying cause.

Erythema multiforme is a self-limiting skin condition characterized by the abrupt appearance of red papules of various sizes, some of which evolve into target lesions. It has been determined that erythema multiforme is often precipitated by herpes simplex virus (HSV) infection. Weston and Morelli conducted a retrospective case series evaluation of 12 children with erythema multiforme to examine its clinical associations, frequency and evolution, and the response to antiviral treatment with acyclovir.

Treatment Erythema multiforme is inadvertently treated when the causative agent, whether it be a drug or a disease, is treated. In severe cases, cortisone-like medication is often used along with general supportive measures and prevention of infection.

    Utslagen uppträder vanligen symmetriskt. Lindriga fall går över på någon vecka, svårare fall kan även drabba slemhinnor. Utslagen sitter ofta på händer och fötter, armar och ben, men kan ibland bli mycket utbredda. Ett typiskt utslag ser ut som en kokard med tre zoner. Centralt ses ett mörkrött område med en mer eller mindre tydlig blåsa, utanför detta en ljusare lätt svullen (ödematös) mellanzon och ytterst en avslutande mörkare röd ringzon som är skarpt avgränsad mot normal hud.     Sjukdomen är att betrakta som en slags allergisk reaktion även om orsaken förblir okänd i ungefär hälften av alla fall. Ibland kan sjukdomen framkallas av något infektionstillstånd, en bakterie- eller virusinfektion, eller av ett läkemedel, eller en kombination av bådadera.     Abstract: BACKGROUND - In adults, erythema multiforme (EM) is thought to be mainly related to herpes infection and Stevens-Johnson syndrome (SJS) to drug reactions.
    AIMS - To investigate this hypothesis in children, and to review our experience in the management of these patients.
    METHODS - A retrospective analysis of 77 paediatric cases of EM or SJS admitted to the Children's Hospital in Bordeaux between 1974 and 1998.
    RESULTS - Thirty five cases, inadequately documented or misdiagnosed mostly as urticarias or non-EM drug reactions were excluded. Among the remaining 42 patients (14 girls and 28 boys), 22 had EM (11 EM minor and 11 EM major), 17 had SJS, and three had isolated mucous membrane involvement and were classified separately. Childhood EM was mostly related to herpes infection and SJS to infectious agents, especially Mycoplasma pneumoniae. Only two cases were firmly attributed to drugs (antibiotics). No patient died. EM and SJS sequelae were minor and steroids were of no overall benefit.
    CONCLUSION - In paediatric practice EM is frequently misdiagnosed. The proposal that SJS is drug related in adults does not apply to children, and in our recruitment EM and SJS are mostly triggered by infectious agents. The course of both diseases, even though dramatic at onset, leads to low morbidity and mortality when appropriate symptomatic treatment is given.

    Etiology and pathogenesis
    The majority of children with EM have a history of herpes simplex virus (HSV) infection. A preceding herpes labialis is seen in 50% of children with EM, said Weston, Chair and Professor, Department of Dermatology, University of Colorado Health Sciences Center, Denver. The episode of herpes labialis usually precedes EM by 7 to 10 days; the herpes labialis can last 1 day to up to 21 days. "Herpes simplex type I virus DNA can be detected within the early red papules or the outer zone of a target lesion in 80% of children with EM by molecular diagnosis," he said. The inflammation within skin lesions is part of the HSV-specific host response.
