The Erythromelalgia Association (TEA) is an international, all-volunteer organization serving patients, families, and friends.
Erythromelalgia (EM) is a rare condition that can cause redness and pain in all parts of the body, but most frequently the feet and hands (extremities). It is characterized by intense, burning pain, severe redness (erythema), and increased skin temperature that may be episodic or almost continuous in nature. Although Erythromelalgia typically affects both sides of the body (bilateral), it may sometimes involve only one side (unilateral). In addition, the disease course may be extremely variable from case to case. For example, in some individuals, symptom onset may be gradual, with the condition potentially remaining relatively mild for years. However, in others, it may have a sudden onset, possibly spreading and becoming severe over weeks.
The specific underlying cause of erythromelalgia remains unknown. However, the condition is thought to result from vasomotor abnormalities or dysfunction in the normal constriction and dilation of the diameter of certain blood vessels, leading to abnormalities of blood flow to the extremities. Erythromelalgia may be an isolated, primary condition or occur secondary to various underlying disorders.

Erythromelalgia
A rare syndrome of paroxysmal vasodilation with burning pain, increased skin temperature, and redness of the feet and, less often, the hands. The cause of primary erythromelalgia is unknown. Secondary erythromelalgia may occur in patients who have myeloproliferative disorders, hypertension, venous insufficiency, diabetes mellitus, SLE, RA, lichen sclerosus et atrophicus, gout, spinal cord disease, or multiple sclerosis. Diagnosis of erythromelalgia is based on the relationship of complaints to increased skin temperature. Secondary types should be differentiated from the rare primary type because correction of an underlying disorder may relieve symptoms. Differential diagnosis includes posttraumatic reflex dystrophies, shoulder-hand syndrome, causalgia, Fabry's disease, and bacterial cellulitis. Because erythromelalgia may precede a myeloproliferative disorder (even by several years), frequent blood counts may be indicated to allow early diagnosis and treatment of the latter. Attacks can be avoided or aborted by rest, elevation of the extremity, and cold applications. Therapy is not always successful. In primary erythromelalgia, aspirin 650 mg 1 to 4 times/day may provide prompt, prolonged relief. Avoiding factors that produce vasodilation is usually helpful, and vasoconstrictors (eg, ephedrine 25 mg po, propranolol 10 to 40 mg po qid, methysergide 1 mg po q 4 h) may also provide relief. In secondary erythromelalgia, the underlying disease should be treated.

The following medical description should help you and your physician better understand this condition. Jay Cohen, MD, Chairman of TEA’s Medical Advisory Committee, defines Erythromelalgia in the following way:
“EM results from a maldistribution of blood flow with some areas not getting enough blood and calling for more. The extra blood gets through other open vessels, including arteriovenous anastomosis, that is, the connection between the arterial and venous systems. This vicious cycle continues to worsen until the overall appearance of the skin is of too much blood flow. Vasodilating drugs sometimes work to open the shut vessels. When that happens, the vessels stop calling for more blood and the overall system may normalize or at least improve its function.”
Dr. Cohen further says: “even mild erythromelalgia can greatly affect normal functioning and quality of life. Patients avoid warm weather and limit their activities to cool or air conditioned locations. Some move to cooler climates...many patients cannot wear socks or closed shoes even in winter.” When patients are severely affected, they can be house bound because of the continually painful symptoms. Elevation of the affected limbs becomes necessary and normal life is disrupted.

I enjoy reading your column and your article discussing Raynaud's disease was of particular interest to me. However, my feet get extremely hot and red. Both plantar fasciitis and peripheral neuropathy were suggested as possibilities. After much frustration, I went to Marshfield Clinic. The doctor did some nerve conduction tests and said it was more likely a problem with blood vessels called erythromelalgia. I am still at a loss as to why I have it and what I can do to treat it. Is that the end of my story or can you tell me more about it and where I might find someone familiar with this very uncomfortable situation?
A: You have a rare problem, that is almost the opposite of Raynaud's , in which the fingers and toes turn color and get painful with exposure to cold. With erythromelalgia, the hands and feet get very red and painful in a warm environment. It is more common in men and usually begins around middle age. The feet are usually more involved than the hands, especially when the feet are hanging down for long periods. Erythromelalgia may be associated with high blood pressure, too many platelets or red cells in the blood, or a side effect of medications like nifedipine (blood pressure medicine).

Background: Erythromelalgia is a rare disorder characterized by burning pain and warmth and redness of the extremities. Confusion exists in the literature concerning nomenclature and classification, but, in general, a distinction is made between primary (idiopathic) and secondary erythromelalgia (most commonly associated with myeloproliferative disorders), as well as early- and late-onset disease. erythros (red), melos (extremities), and algos (pain). In 1938, Smith and Allen proposed the term erythermalgia to emphasize the characteristic warmth. They also proposed a distinction between primary (idiopathic) and secondary (due to underlying neurologic, hematologic, or vascular problems). Pathophysiology: Arteriolar fibrosis and occlusion with platelet thrombi occur, but not in all cases. Platelets have been demonstrated to have abnormal hyperaggregability. Platelet kinetic studies show decreased platelet survival, predominantly due to consumption. Prostaglandins and cyclooxygenase apparently play an important pathogenetic role. Pathophysiology in cases not associated with thrombocythemia is less clear but does not involve platelet-mediated inflammation and thrombosis. This is the major reason that Drenth and Michiels distinguish between erythromelalgia and erythermalgia. One postulate is maldistribution of skin perfusion resulting from arteriovenous shunting, leading to an imbalance between thermoregulatory and nutritive perfusion. Another is decreased sympathetic vasoconstrictor response to certain stimuli.

Welcome!!! We wish to introduce you to our informal group on erythromelalgia (EM).
What is Erythromelalgia? Erythromelalgia is a painful condition that has a very distinctive pattern of symptoms. It is diagnosed when the extremities are red, warm and painful- the pain is often described as burning. This pain is only relieved by cooling (often in cold water) or the elevation of the affected extremities.
Secondary Erythromelalgia: This condition often accompanies tissue destroying disorders such as arthritis, lupus, or diabetes. It also often associated with potentially fatal disorders of the blood as well- which is why all who suffer from these symptoms should seek a diagnosis and treatment from a physician and have periodic blood tests thereafter. Secondary erythromelalgia can very often be treated successfully by your physician.
Primary Erythromelalgia: Where no cause can be found, the condition is called primary erythromelalgia. Primary erythromelalgia is typically difficult to treat. This should be diagnosed by a physician in order to rule out some underlying disease process.

I don't even know for sure if this is the correct forum, my searches at many sites come up empty, but since Erythromelalgia is connected to problems with the vascular system, I decided to try here. I'm trying to find GOOD, basic information/explanations for Erythromelalgia. I have been diagnosed but without any clear understanding as to what this will mean - prognosis. It is secondary to peripheral neuropathy, but doesn't seem to be a neurological disorder, although many of the symptoms overlap. I'm confused! I'll appreciate any help you can give me. Thanks!

QUERY: In the past 4-5 yrs. any increase in body temperature(even slightly)
ex.. wearing a sweater, drinking hot coffee, going inside during the winter
or working inside all day if its too warm...
causes my hands to swell and turn very red. It's almost painful.
Now I'm noticing my feet are doing this as well. I haven't always been this
way, I used to wear sweaters constantly,sleep with an electric blanket. Now I
can't go in the sun in moderate heat with out being miserable. My doctors
ran some tests, Its not diabetes, thyroid disorder, blood pressure, or
glandular related, so they say. My allergies are, sulfa, shellfish, codiene
are amoxicillian (for acne) 1 and 1/2 yrs. Celexa 30 mg for 8 months. If
the temp decreases I am not as miserable, but still not normal. This is getting very frustrating and the doctors think I'm looney . I'm getting scared, It's very uncomfortable and is keeping me from being in a lot of situations. ANSWER: I'm glad you are pursuing the answers for your problem, especially since it seems to be getting worse with time. So far, so good - the thyroid
