The management of benign and malignant esophageal disease continues to be a major clinical challenge. Our thoracic surgeons have wide experience in the surgical management of esophageal diseases. Benign conditions, which often require surgical intervention include disorders such as achalasia, gastroesophageal reflux, motility disorders, and Zenker's Diverticulum. Our surgeons employ standard open surgical approaches, as well as minimally invasive or thorascopic techniques in treating these conditions. Carcinoma of the esophagus is not a particular frequent entity in the United States, but does effect 12,000 to 15,000 individuals annually and is considered a highly lethal tumor. Symptoms such as dysphasia (the inability to swallow) caused by carcinoma of the esophagus are well known, but tend to occur later in the course of the disease. Only 15%-20% of patients diagnosed with esophageal carcinoma are candidates for curative resection. Many other patients benefit from palliative procedures. Malignant and premalignant conditions of the esophagus are generally managed with a multimodality approach. The combination of preoperative radiotherapy and chemotherapy has improved not only the resectability of these tumors, but also has improved the survival in these patients.

Dysphagia that is episodic and occurs with both liquids and solids from the outset suggests a motor disorder, whereas when the dysphagia is initially for solids such as meat and bread, and then progresses with time to semisolids and liquids, one should suspect a structural cause (e.g., stricture). If such a progression is rapid and associated with significant weight loss, a malignant stricture is suspected. Associated symptoms help determine the etiology of dysphagia. For instance, a reflux-induced stricture should be suspected if the dysphagia is associated with heartburn or regurgitation, esophageal cancer if there is associated mid-back pain and weight loss, a motor disorder such as diffuse esophageal spasm if there is angina-like chest pain, and a "scleroderma esophagus" if there is arthralgia, skin changes or Raynaud's phenomenon. 4.1.2 ODYNOPHAGIA

    The Society was founded in 1979 by Professor Komei Nakayama in Tokyo in Order to contribute widely to the field of Diseases of the Esophagus internationally, drawing upon the expertise that had been built up in this field both in Japan and in other countries.

    1) To Provide a forum for the exchange of knowledge experience and techniques concerning the treatment of esophageal diseases.
    2) To disseminate the results of the progress made in the field of esophageal diseases and to accumulate data on a worldwide basis.

    The following committees have been established and are each following their own schedules: a) Journal Committee The journal of the Society (Diseases of the Esophagus) is published quarterly by Blackwell Science Asia.     I have the special pleasure to invite you to participate in the VIIIth World Congress of the International Society for Diseases of the Esophagus - ISDE, that will be held in Sao Paulo - Brazil, from 5th to 8th September 2001.
    The continuous progress of knowledge of the physiopathology of esophageal diseases, as well as the diagnosis means and treatment procedures in benign and malignant diseases, are requiring constant attention from physicians. This important world meeting of ISDE will certainly be an excellent opportunity for all those who are interested in esophageal diseases.
    The Congress will be developed under the form of free papers and video presentations, symposia, round tables and posters. For young physicians interested in the specialty there will be didactic sessions, in small theaters, so callled training and teaching rooms.
    Renowned international specialists will deliver conferences and specific participations in symposia and round tables. Besides the scientific sessions there will be social programs as visits to historical places and museums of art.

We provide complete treatments for esophageal disorders, including esophageal cancer and benign esophageal diseasessuch as Barrett's esophagus, gastroesophageal reflux disease (GERD), and achalasia:
    • Esophageal cancer is often treated by surgical removal, but we also perform simpler procedures to improve swallowing, such as laser therapy and stent placement. In many patients with severe symptoms, GERD is treated by fundoplication surgeryusually performed laparoscopically. Surgery to treat achalasia is often performed laparoscopically as well.
