Division of Digestive Diseases and Nutrition
The consultative services include faculty, fellows, residents, and students. The division has multiple conferences at all institutions including a comprehensive gastroenterology conference involving not only the division, but also radiology and the division of gastroenterological surgery. Inpatient consultative services recommend appropriate enteral and parenteral hyperalimentation as well as inserting peg tubes for nourishment. The division offers a three-year fellowship program designed to train fellows in the field of digestive diseases and nutrition. Fellows rotate through all four teaching facilities. Research activities include the field of laser therapy, endoscopic ultrasound, progression of polyps and the natural history of polyps leading to cancer. Gastroesophageal reflux disease refers to the pathologic consequences of the reflux of gastric contents into the esophagus, manifesting various symptoms. Endoscopic examination and empiric trials of PPI contribute to the diagnosis of this disease. Achalasia is an esophageal emptying disorder caused by absence of esophageal peristalsis and incomplete LES relaxation. Pneumatic balloon dilation of LES offers long-term remission.
Esophageal candidiasis is quite common in endoscopic findings, but most of the cases do not require antifungal treatment. Endoscopy is the most useful method to diagnose and to treat esophageal varices and Mallory-Weiss syndrome.

General Information
Diseases of the chest are among the most common ailments affecting adult Americans today. Here in western Pennsylvania, lung and esophageal diseases are particularly prominent health-care problems. The Allegheny Center for Lung and Thoracic Disease is designed to provide comprehensive, innovative clinical services specifically directed toward a wide range of problems affecting the lungs, mediastinum, and the esophagus. Because the center unites and coordinates the services of specialists in respiratory diseases, thoracic surgery, medical oncology and radiation oncology into one program, the center offers patients with thoracic diseases one of the most accessible and proficient programs of care available in the country. Particular focus upon the diagnosis and management of chronic obstructive pulmonary disease (emphysema, asthma and bronchitis), interstitial and environmental lung diseases, and sleep-related breathing disorders are provided by the pulmonary specialists dedicated to the center. Thoracic surgeons of the center work closely with these pulmonologists to evaluate patients with these disorders who may benefit from lung transplantation or lung volume reduction surgery specifically directed toward patients with pulmonary emphysema. The center's pulmonary medical and thoracic surgical team also work closely with the medical and radiation oncology specialists to provide state-of-the-art, comprehensive treatment for patients with lung and esophageal cancer. Furthermore, the management of benign diseases of the esophagus, such as gastroesophageal reflux and disorders of esophageal motility (i.e., achalasia), are also focused upon by the center's physician team.

Gastroesophageal reflux disease (GERD) is a digestive disorder that affects the lower esophageal sphincter (LES)the muscle connecting the esophagus with the stomach. Many people, including pregnant women, suffer from heartburn or acid indigestion caused by GERD. Doctors believe that some people suffer from GERD due to a condition called hiatal hernia. In most cases, heartburn can be relieved through diet and lifestyle changes; however, some people may require medication or surgery. This fact sheet provides information on GERD-its causes, symptoms, treatment, and long-term complications.
What Is Gastro-
esophageal Reflux?
Gastroesophageal refers to the stomach and esophagus. Reflux means to flow back or return. Therefore, gastroesophageal reflux is the return of the stomach's contents back up into the esophagus.
Figure 1 In normal digestion, the LES opens to allow food to pass into the stomach and closes to prevent food and acidic stomach juices from flowing back into the esophagus. Gastroesophageal reflux occurs when the LES is weak or relaxes inappropriately allowing the stomach's contents to flow up into the esophagus. Figure 1 shows the location of the LES between the esophagus and the stomach.

Esophageal Cancer What is Esophageal Cancer?

Esophageal cancer is cancer which originates in the esophagus - the long tube that carries food from the throat down into the stomach. There are two forms of esophageal cancer: squamous cell carcinoma and adenocarcinoma. Squamous cell carcinoma occurs in the squamous cells that line the middle and upper portions of the esophagus. Adenocarcinoma occurs in glandular tissue located in the lower portion of the esophagus. Either type of cancer may spread ("metastasize") to the lymph nodes and then to other parts of the body. Methods of diagnosis and treatment are similar for both forms of esophageal cancer. Risk Factors
Esophageal cancer can result after many years of irritation to the esophagus. Use of alcohol or tobacco are major risk factors that increase a person's risk of developing esophageal cancer.     Causes, Incidence, and Risk Factors
    Esophageal cancer is relatively uncommon in the United States, and occurs most often in men over 50 years old. It affects less than 5 in 100,000 people. There are two main types of esophageal cancer, distinguished by the way they look under the microscope: squamous cell carcinoma and adenocarcinoma. Squamous cell cancer is associated with smoking and alcohol consumption . The incidence of this disease in the United States has remained relatively constant, while the incidence of adenocarcinoma of the esophagus has risen dramatically. Barrett's esophagus , a complication of gastroesophageal reflux disease (GERD), is a risk factor for the development of adenocarcinoma of the esophagus.
