Essential tremor (ET) is considered the most common neurologic movement disorder affecting nearly 10 million people in the United States. ET is a chronic condition characterized by involuntary, rhythmic tremor of a body part, most typically the hands and arms, often the head and voice, but rarely the legs. ET is generally considered a slowly progressive disorder, although many individuals may have a mild form of ET throughout life that never requires treatment. The most common form of ET affects the arms and hands, usually bilaterally, and is most prominent with the arms held against gravity (postural tremor) or in action (kinetic tremor) such as when writing or drinking from a cup. Unlike patients with Parkinson's disease, patients with ET rarely exhibit a tremor when the arm is at rest. [return to top]

OMIM catalog of human genes and disorders Information News release from the National Institute of Neurological Disorders and Stroke, NIH Information from the Essential Tremor Foundation TREMOR , or uncontrollable shaking, is a common symptom of neurological disorders such as Parkinson disease, head trauma and stroke. However, many people with tremor have what is called idiopathic or essential tremor. In these cases, which number 3-4 million people in the US, the tremor itself is the only symptom of the disorder. While essential tremor may involve other parts of the body, the hands and head are most often affected. In more than half of cases, essential tremor is inherited as an autosomal dominant trait, which means that children of an affected individual will have a 50 percent chance of also developing the disorder. In 1997, the ETM1 gene (also called FET1) was mapped to chromosome 3 in a study of Icelandic families, while another gene, called ETM2, was mapped to chromosome 2 in a large American family of Czech descent. That two genes for essential tremor have been found on two different chromosomes demonstrates that mutations in a variety of genes may lead to essential tremor. While the mainstays of treatment are drugs such as propranolol and primidone, alternative drugs and surgical treatments are also available. Further understanding of the molecular mechanism behind the disease awaits the discovery and cloning of an essential tremor gene.

Tremor Tremor is an involuntary, rhythmic, oscillating movement of any body part caused by contractions of reciprocally innervated antagonist muscles. Physiologic Tremor Present in all individuals Exaggerated Physiologic Tremor can be seen related to: Mental state: anxiety, stress, sleep deprivation Metabolic: fever, thyrotoxicosis Drugs: beta-adrenergic agonists: Phenylpropanolamine, Pseudoephedrine Fluoxetine (Prozac), Haloperidol (Haldol), Lithium, Methylphenidate (Ritalin), Metoclopramide (Reglan), Theophylline, Valproic acid Diet: caffeine Essential Tremor Kinetic tremor greater than resting tremor May affect: hands, head, voice, legs Treatment: Alcohol: with time, larger amount may be needed. Judicious use of small amount before meals may be reasonable. Primidone (Mysoline): More effective than Propranolol.

Introduction
Essential tremor (ET) is a common movement disorder in which a high frequency (typically 7 hz) postural tremor occurs in the hands, variably accompanied by a head and/or vocal tremor. The tremor ordinarily is abolished by ingestion of alcohol, and is increased by nervousness and medications that increase adrenalin levels. Essential tremor often runs in families, and is generally not associated with brain lesions.
Differential Diagnosis:
Essential tremor is generally easily distinguished from the common tremor of Parkinsonism, which is slower and present at rest.
Treatment
Beta blockers such as propranolol, benzodiazepines, and mysoline are all commonly used in treatment. Recently it has been reported that ET can be treated with bilateral thalamic stimulation, but as this procedure is associated with significant morbidity, its usefulness seems very limited. Background: Essential tremor (ET) is the most common movement disorder. It is a syndrome characterized by a slowly progressive postural and/or kinetic tremor, usually affecting both upper extremities. Pathophysiology: The pathophysiology of ET is not known. No pathological findings are known to be associated consistently with ET. However, the following has been hypothesized:
  • ET is the result of an abnormally functioning central oscillator, which is located in the Guillain Mollaret triangle near the brain stem and involves the inferior olivary nucleus.
  • The cerebellar-brainstem-thalamic-cortical circuits probably are involved.
Harmaline, a monoamine oxidase (MAO) inhibitor, when administered to primates with lesions of the ventromedial tegmental tract or lateral cerebellum, produces an ET-like tremor. In these animals, inferior olivary nucleus neurons fire synchronously at the tremor frequency. C-2-deoxyglucose positron emission tomography (PET) studies demonstrate hypermetabolism in the inferior olivary nuclei of rats and cats with harmaline-induced tremor. In patients with ET, [

These contractions are involuntary and difficult to control. The exact cause of essential tremor is not known, however therapies exist that target various areas of the brain as the source of the tremor., What are the symptoms? Essential tremors usually involve the hands, head and arms. The tremor usually does not occur at rest. Instead, the tremor is noticed when the arms/hands are deliberately moved to perform a task or action. Emotion, exercise and fatigue can make an essential tremor worse. How is it diagnosed? There is no specific test that can diagnose essential tremor. The diagnosis is based on the clinical picture, the frequency(i.e. the number of movements per second) of the tremor - which is usually 4-6Hz, and the abscence of a pathologic disease or lesion. Causes, incidence, and risk factors Return to top All people have some degree of tremor that occurs during movement,. This shaking is called physiologic (occurs normally), and it mainly involves the hands. Stressful circumstances, such as fatigue, anger, fear or using certain substances (such as coffee or cigarettes) may cause transient worsening of physiologic tremor, to the point that it may become visible to the naked eye. Essential tremor is the most common form of abnormal tremor, which resembles an exaggerated physiologic shaking. Essential tremor is worsened by the same factors that enhance physiologic tremor. Although the cause is unknown, new research shows that one part of the brain called cerebellum does not appear to function properly in patients who have essential tremor. The cerebellum is the part of the brain that coordinates muscle movements and therefore, provides accuracy and harmony to voluntary movement. Nevertheless, no evident brain lesions have been identified in patients with this form of tremor. There is evidence that there are several different types of essential tremor (such as young-onset essential tremor or essential tremor with head tremor). These types differ in their response to treatment.
