Tender or lumpy breasts are one of the most common reasons why women consult their gynecologists for assessment and treatment. Since painful breasts are not always lumpy, and lumpy breasts are not always painful (and neither is usually abnormal), it is useful to create descriptive categories of symptoms and conditions to replace the generic term "fibrocystic".
Virtually all knowledgeable health care providers agree that the term fibrocystic breast "disease" or "condition" should be abandoned in favor of a more accurate physiologically based description. First of all, the benign breast conditions that are present in almost all of us to some degree should never have been given the "disease" label in the first place. Moreover, the widespread misconception that women with painful or lumpy breasts are at increased risk of breast cancer borders on the tragic. Unfortunately, our health care system requires a diagnostic code to reimburse services, and fibrocystic breast disease has one, even though the medical literature is replete with reasons why it shouldn't. This reinforces misinformation and fear and obscures the safe and simple means that exist for obtaining relief and reassurance.

    Fibrocystic changes in breasts are very common. Almost everyone has extra water in their body a week before their menstrual period (monthly bleeding). The water causes breasts to be a little (or a lot) sore and maybe get larger. The water is in the glands that produce milk after delivery of a baby. There can be water in the tubes that lead from the glands to the nipple too.
    Some women will have scar tissue (fibrous tissue) that develops because of the irritation from extra fluid. Both the fluid filled glands and the scar tissue will feel lumpy. This can be hard to tell from non-fibrocystic breast tissue.
    How will I Know If I Have Fibrocystic Breasts?
    All breasts feel lumpy. It's the glandular tissue and fatty tissue in the breasts that feels lumpy. If they feel lumpier before your period and less lumpy 7 to 10 days after your period, you have fibrocystic changes.
    Now, if you feel a lump that is harder or larger than the other lumps, see your health care provider for a breast exam. The lump will probably just be fibrocystic scar tissue.

In pregnancy when both estrogen and progesterone are very high, the breasts are very full and almost hard. In menopause, without hormone replacement, the breasts are the opposite. They are collapsed and soft.

    At least 60% of women between the ages of 30 to 50 are told by their physician that they have Fibrocystic breast disease. This is a common non cancerous condition that responds to monthly changes in the levels of estrogen and progesterone, female hormones produced by the ovaries during a woman's reproductive years.     Fibrocystic Breast Disease
    Usually, fibrocystic changes can be diagnosed by physical examination or mammography. Fortunately, only about 5% of fibrocystic conditions have the type of changes that would be considered a risk factor of developing breast cancer. The cause of fibrocystic changes is related to the way breast tissue responds to monthly changes in the levels of estrogen and progesterone. During the monthly menstrual cycle, the breast tissue alternately swells and returns to normal. Hormonal stimulation of the breast tissue causes the ducts to enlarge, and the breasts to retain water. The breasts may feel swollen, painful, tender, and lumpy at this time. After menstruation, swelling in the breast usually decreases, and the breasts feel less tender and lumpy. As a result of repeated hormone stimulation, there is an increase in firmness of the tissue, and pockets of fluid called cysts may form in obstructed or enlarged milk ducts feeling like tiny beads scattered throughout the breasts. Generally, fibrocystic changes are found in both breasts and most frequently are found in the upper outer quadrant and the underside of the breast. The condition tends to subside after menopause.     Fibroid Tumors

    A mammogram is simply an X-ray of the breast. Breast tissue is compressed between two plates and held for only a few seconds while the technician steps behind a protective plate and activates the machine. Two views of each breast are taken for a normal screening mammogram. If the woman has had breast cancer or suspicious findings before, the technician may take three views which is then called a diagnostic mammogram. Is the radiation harmful?

    Treatment for fibrocystic lesions varies greatly between standard allopathic medicine and natural medicine. Allopathic medicine may offer hormonal therapies. Standard treatment for severe fibrocystic breasts has been oral androgen therapy to counteract the effect of endogenous estrogens. The commonly prescribed drug danazol reduces the release of all gonadotropins from the pituitary. According to naturopathic theory, people who consume a lot of meat, eggs, and dairy products may also ingest estrogens in small amounts due to hormonal residues in animal products. Animals are often fed estrogens to bring them to market weight more quickly, or to increase milk and egg production. Hormonal residues consumed on a regular basis may also contribute to the hormonal load on the body. Recently, the hormonal influences of accumulated pesticides in the environment has also drawn scientific attention. The view in naturopathic medicine is that, since it is the liver's job to metabolize hormones in circulation, liver disease can lead to the development of hyperestrogenism and fibrocystic breasts. The liver removes active estrogens from circulation, conjugating them and releasing them to the gastrointestinal tract for elimination. When the liver is inundated with work to do, such as processing many hormones, emulsifying fats and cholesterol, removing toxins, pesticides, drugs, alcohol, or other substances from the blood, it may have a difficult job metabolizing estrogens. When the bowels are constipated or when there are undesirable bacterial flora present, estrogens that have been conjugated in the liver for elimination are unconjugated in the intestine, reabsorbed, and returned to the liver a second and even third time, adding to the liver's workload. Methyl-xanthines occurring in coffee, cola, chocolate, and black teas will also aggravate fibrocystic disease.
