It's one of five common contagious childhood ailments that cause fever and a rash. After doctors figured out what to call measles, rubella, roseola, and scarlet fever, they apparently ran out of catchy names and called the fifth disease just that. Its technical name is erythema infectiosum, but the illness does have a more colorful moniker: "slapped-cheeks disease." That's because after the contagious period has passed, children with fifth disease often develop an angry red rash on their cheeks. This fades to a pink lacy pattern as it spreads to the torso, arms, thighs, and buttocks. Physical activity or bathing can worsen the rash, which may itch. Occasionally a child may have joint pain with the rash, requiring medical attention. The actual illness precedes the rash by seven to 10 days and is usually so mild that parents barely take note of it. Its symptoms include a fever (usually below 103 degrees F) that may be accompanied by a headache, reddish eyes, fatigue, and a sore throat. In some cases, however, there are no symptoms at all.

    Fifth disease, erythema infectiosum or slapped child disease, is a not infrequent childhood infectious disease in which children have fevers, tiredness and extensive red rashes of the cheeks and face. It is caused by parovirus B19 and spreads like chicken pox or measles in preschool and school age children. Every once in awhile, an obstetrician diagnoses a baby who has died before birth with a condition in which there is massive fluid swelling (hydrops) of the fetus. In earlier times this was mostly due to Rh isosensitization in which the mother had an Rh negative blood type and the baby had an Rh positive blood type. In the last 20-30 years, treatment of Rh negative mothers with Rhogam has drastically reduced this immune hydrops occurance. Now the most frequent cause of hydrops is non-immune and is often caused by B19 parovirus infection in pregnant women. Since B19 parvovirus infection in pregnancy has been associated with stillborn babies, the question becomes should pregnant women be excluded from the workplace when there is a epidemic of fifth disease? Another concern would be if any of your family was exposed while you were pregnant, how likely is it that your unborn child would be affected? A recent article by Harger and others

    Eastern Virginia Medical School Fifth's Disease is a highly contagious viral illness that is very common, but not serious in children. Adults are commonly exposed to it as well. Once you have had this infection, you develop a lifelong immunity to it. Fifth's Disease, caused by the human Parvovirus B 19, is not dangerous in most adults, but it can be very harmful to a pregnant woman. I am Dr. Sue Kelly Sayegh, assistant professor of obstetrics and gynecology at Eastern Virginia Medical School. If a pregnant mother develops Fifth's Disease, there is a slight risk that it will infect her unborn child. The fetus is unable to fight off infections and is more susceptible to the effects of the virus than children or adults. The virus can infect the bone marrow and decrease the production of red blood cells, resulting in anemia. It can also infect the heart muscle and cause heart failure. Both of these problems lead the baby to develop severe swelling, which is called hydrops. Depending on the degree of severity, the baby can recover on its own, but in some cases, can die inside the mother's body. Fortunately, the chance of losing an unborn child to Fifth's Disease if very small. Over 90% of the time, there is no effect at all on the baby. The infection is most dangerous in the first trimester. The most common time to see the effects on the baby is 3 - 6 weeks after the mother's infection, but it can be as late as 12 weeks after exposure.
