Cochrane reviews are regularly checked and updated if necessary. Background: Dermatophyte infection has been estimated to affect the feet of 15% of the general population. Whilst superficial fungal infections are not life-threatening they are associated with considerable morbidity and dermatophytes can be difficult to treat. Clinicians faced with a public demand for effective treatments have a difficult task. Topical preparations are much cheaper than orally administered antifungal agents. Most are over the counter preparations but more expensive topical preparations are available as prescription only medicines. Objectives: The objective of this review is to identify and evaluate the evidence of efficacy for topical treatments for fungal infections of the skin and nails of the human foot. To establish the effectiveness of topical treatments in achieving a cured condition and in preventing recurrence Search strategy: Randomised controlled trials were identified from the MEDLINE, EMBASE and CINHAL databases, from the beginning of these databases to December 1997. Also we screened the Cochrane Controlled Trials Register, the Science Citation Index BIOSIS, CAB - Health, Healthstar and Economic databases. References and unpublished studies were searched, podiatry journals handsearched and the pharmaceutical industry contacted.

Cochrane reviews are regularly checked and updated if necessary. Background: About 15% of the population have fungal infections of the feet (tinea pedis or athlete's foot). Whilst there are many clinical presentations of tinea pedis the most common are between the toes (interdigital) and on the soles, heels and sides of the foot (plantar) which is known as moccasin foot. Once acquired the infection can spread to other sites including the nails, which can be a source of reinfection. Oral therapy is usually used for chronic conditions or when topical treatment has failed. Objectives: To assess the effects and costs of oral treatments for fungal infections of the skin of the foot (tinea pedis). Search strategy: Randomised controlled trials were identified from MEDLINE, EMBASE and CINAHL from the beginning of these databases to January 2000. We also searched the Cochrane Controlled trials Register (Cochrane Library issue 1, 2000) the Science Citation Index, BIOSIS, CAB-Health, Health star and Economic databases. Superficial Mycoses a) Dermatophytosis
    • Also known as ringworm. Caused by: Mircosporum, Trichophyton, Epidemophyton. These fungi can only live on the superficial dead layers of skin.
    b) Yeasts
    • These are commonly caused by Candida and tend to attack those geckos with already compromised immune systems. They tend to affect those tissues that remain moist.

    If you suspect tinea please send generous scrapings of the skin or clippings of the nail to the Laboratory for direct examination and culture and please send these specimens before you start treatment with systemic or topical anti-fungals.
    NOTES:
  • Think of tinea in any scaly rash that is not obviously psoriasis or eczema etc.
    Asymmetrical scaly rashes should alert you to the possibility of tinea.
    Nail dystrophy. Do undress the patients and look carefully for psoriasis.
    Remember that candida does not respond to Griseofulvin or systemic Terbinafine (Lamisil).
    Scalp ringworm and kerion do not respond to topical anti-fungal treatment alone.
  • Terbinafine (Lamisil) Itraconazole (Sporanox This has the advantage of treating yeasts (such as candida) as well as dermatophytes. It is often prescribed for nails in the Sporanox Pulse form, giving it for one week out of four.
    Pityriasis Versicolor
    Use Nizoral Shampoo, or Cream if localised, or alternatively Selsun Shampoo. If very widespread give Itraconazole (Sporanox) orally 200 mg per day for seven days. Pityriasis Images Index More than you usually need or want to know about fungi, but useful for looking up odd reports from the local laboratory.
