GLAUCOMA in Veterinary Patients
Glaucoma is the elevation of pressure inside the eye, known as intraocular pressure (IOP) beyond a specific point at which vision is compromised or is no longer possible. Glaucoma is a frequent cause of blindness in humans and animals. To understand glaucoma, it is necessary to understand how the normal flow of intraocular fluid maintains normal intraocular pressure. The fluid inside the eye is called the aqueous humor which is produced in the ciliary body which is located behind the iris. This fluid flows through the pupil and drains from the eye through a sieve-like network located at the junction of the cornea and the iris called the iridocorneal cleft or drainage angle. The aqueous humor is produced and drains from the eye at approximately the same rate, resulting in a stable pressure inside the eye of 15 to 30 mmHg (millimetres of mercury). Glaucoma occurs as a consequence of inadequate outflow of aqueous humor and a subsequent buildup of pressure inside the eye. The resulting high pressure damages the optic nerve and results in blindness. There are two categories of glaucoma.

What is glaucoma?
Glaucoma is the name for a group of eye conditions in which the optic nerve is damaged at the point where it leaves the eye. As the diagram opposite shows, this nerve carries information from the light sensitive layer in your eye, the retina, to the brain where it is perceived as a picture. Your eye needs a certain amount of pressure to keep the eyeball in shape so that it can work properly. In some people, the damage is caused by raised eye pressure. Others may have an eye pressure within normal limits but damage occurs because there is a weakness in the optic nerve. In most cases both factors are involved but to a varying extent. Eye pressure is largely independent of blood pressure.
What controls pressure in the eye?
A layer of cells behind the iris (the coloured part of the eye) produces a watery fluid, called aqueous. The fluid passes through a hole in the centre of the iris (called the pupil) to leave the eye through tiny drainage channels. These are in the angle between the front of the eye (the cornea) and the iris and return the fluid to the blood stream. Normally the fluid produced is balanced by the fluid draining out, but if it cannot escape, or too much is produced, then your eye pressure will rise. (The aqueous fluid has nothing to do with tears.)

Our interests include AMD/ARMD, corneal scarring, diabetes, ectodermal dysplasia, genetics, glaucoma, melanoma and PVR. Our Unit is comprised of both the Academic Unit of Ophthalmology and the clinical St Paul's Eye Unit. The Academic Unit is located in the Department of Medicine which is part of the Faculty of Medicine at the University of Liverpool RLBUHT ). Q : What is glaucoma?
Glaucoma is a group of diseases usually associated with increased pressure within the eye. (Some types of glaucoma are primary open-angle, angle-closure, secondary, congenital, juvenile and low-tension.) This pressure can cause damage to the cells that form the optic nerve, the structure responsible for transmitting visual information from the eye to the brain. The damage is progressive with loss of peripheral vision first, followed by reductions in central vision and, potentially, blindness.
Q : How many people have glaucoma?
Between two million and three million Americans age 40 and over, or about one in every 30 people in that age group have glaucoma. This includes those who are unaware they have the eye disease.

Glaucoma is an eye disease in which the internal pressure in your eyes increases enough to damage the nerve fibers in your optic nerve and cause vision loss. The increase in pressure happens when the passages that normally allow fluid in your eyes to drain become clogged or blocked. The reasons that the passages become blocked are not known.
Noticeable symptoms of glaucoma may be a gradual loss of side vision (above) or blurred vision (below).
Glaucoma is one of the leading causes of blindness in the U.S. It most often occurs in people over age 40. People with a family history of glaucoma, African Americans, and those who are very nearsighted or diabetic are at a higher risk of developing the disease. The most common type of glaucoma develops gradually and painlessly, without symptoms. A rarer type occurs rapidly and its symptoms may include blurred vision, loss of side vision, seeing colored rings around lights and pain or redness in the eyes. Glaucoma cannot be prevented, but if diagnosed and treated early, it can be controlled. Vision lost to glaucoma cannot be restored. That is why the American Optometric Association recommends annual eye examinations for people at risk for glaucoma (your doctor may, depending on your condition, recommend more frequent examinations). A comprehensive optometric examination will include a tonometry test to measure the pressure in your eyes; an examination of the inside of your eyes and optic nerves; and a visual field test to check for changes in central and side vision.

Si es mayor la cantidad de líquido que entra que la que sale, la presión dentro del ojo aumentará. Al elevarse la presión dentro del ojo se produce atrofia de algunas estructuras: las mas afectadas son las compuestas por tejido nervioso, principalmente el nervio óptico. Estas estructuras nerviosas no son capaces de repararse a sí mismas una vez lesionadas, es decir que si existe un daño avanzado por glaucoma, éste no podrá ser reparado. Es por esto que el tratamiento en etapas avanzadas consiste en conservar la poca visión que todavía exista para no llegar a la ceguera absoluta. Solo si el tratamiento se inicia tempranamente se logra evitar completamente la pérdida de visión.

The most common form of the disease is open-angle glaucoma, which affects about three million Americans, half of whom don't know they have it. Glaucoma has no symptoms at first but over the years can steal its victims' sight, with side vision being effected first. It is estimated that nearly 100,000 individuals in the US suffer from glaucoma due to a mutation in the GLC1A gene, found on chromosome 1. There has been some speculation as to the role of the gene product in the eye. As it is found in the structures of the eye involved in pressure regulation, it may cause increased pressure in the eye by obstructing the aqueous outflow. With early treatment, serious loss of vision and blindness can be prevented. The cloning of the GLCA1 gene is the first step towards an understanding of the pathology of glaucoma at the molecular level, and may help in the development of tests for the early detection of the disease, as well as providing a basis for research into effective therapies.

Glaucoma
Glaucoma is not a single disease but a group of conditions with various causes that ultimately are associated with progressive damage to the optic nerve and subsequent loss of visual function. Glaucoma has been called the "Sneak Thief of Sight" because progressive damage to the optic nerve often results with no symptoms. Glaucoma is the third most common cause of blindness in the United States with over 80,000 Americans blind from Glaucoma. It has been estimated that less than half the patients who have glaucoma have actually been diagnosed as having glaucoma. Glaucoma is generally classified as either the Open Angle (Chronic) Form or the Narrow Closed Angle (Acute) Form of Glaucoma. Diagnosis - There are three major factors which are considered in determining if a patient has glaucoma.
  • Eye Pressure (Intraocular Pressure ): Normally the eye produces fluid (aqueous humor) which after bathing the internal structures of the eye, is drained from the eye.
    • The normal eye maintains an internal pressure of between 12 and 22 mm of Mercury. In patients with glaucoma, the intraocular pressure is often elevated as a result of a decrease in the ability of the eye to adequately drain the intraocular fluid.
