There are different kinds of goiters. A simple goiter usually occurs when the thyroid gland is not able to produce enough thyroid hormone to meet the body's requirements. The thyroid gland compensates by enlarging, which usually overcomes mild deficiencies of thyroid hormone.
A simple goiter may be classified as either an endemic (colloid) goiter or a sporadic (nontoxic) goiter. Endemic goiters occur within groups of people living in geographical areas with iodine-depleted soil, usually regions away from the sea coast. People in these communities might not get enough iodine in their diet. (Iodine is vital to the formation of thyroid hormone.) The modern use of iodized table salt in the U.S. prevents this deficiency; however, it is still common in central Asia and central Africa. In most cases of sporadic goiter the cause is unknown. Occasionally, certain medications such as lithium or aminoglutethimide can cause a nontoxic goiter.

Definition: Goiter represents an enlargement of the thyroid gland. Prevalence: Rare. Etiology: Fetal goiter can be associated with hyperthyroidism, hypothyroidism or an euthyroid state. Goiter associated with hyperthyroidism can be the result of iodine excess or deficiency, intrauterine exposure to antithyroid drugs or congenital metabolic disorders of thyroid synthesis. Ultrasound features: The goiter is manifested as a solid, anteriorly located symmetric mass, that may result hyperextension of the fetal head. Polyhydramnios is common due to mechanical obstruction of the esophagus. Prognosis: Goiter may cause dystocia by extension of the fetal head during the process of labor. It can also cause upper airway obstruction, leading to acute respiratory failure. The prognosis depends on the basic cause of the goiter. Most cases of fetal goiters occur in women with a history of thyroid disease. Fetal blood sampling can aid in determining fetal thyroid status, especially in women suffering from Grave’s disease where a transplacental transfer of drugs or thyroid-stimulating antibodies may result in fetal goiter. If careful sections are made of numerous areas, 4-17% of these glands removed at surgery will be found to harbor microscopic papillary carcinoma. The variable incidence can most likely be attributed to the different criteria used by the pathologists and the basis of selection of the patients for operation by their physicians. These factors are discussed below.
NATURAL HISTORY OF THE DISEASE
Multinodular goiter is probably a lifelong condition that has its inception in adolescence or at puberty. Minimal diffuse enlargement of the thyroid gland is found in many teenage boys and girls, and is almost a physiologic response to the complex structural and hormonal changes occurring at this time. It usually regresses, but occasionally (much more commonly in girls) it persists and undergoes further growth during pregnancy. This course of events has not been documented as well as might be desired in sporadic nodular goiter, but it is the usual evolution in areas where mild endemic goiter is found. Occasionally, a sudden increase in the size of the gland is associated with sharp pain and tenderness in one area. This event suggests hemorrhage into a nodular cyst of the goiter, which can be confirmed by ultrasound. Within 3-4 days the symptoms subside, and within 2-3 weeks the gland may revert to its previous dimensions. In such a situation, acute thyrotoxicosis may develop and subside spontaneously

A goiter is a swelling of the thyroid gland. It can be caused from a variety of conditions affecting the thyroid, such as underactivity of the gland ( Hypothyroidism ), overactivity ( Hyperthyroidism Thyroid Nodules or inflammation ( Thyroiditis ). The thyroid can also become enlarged temporarily and subside on its own without treatment. Rarely, a goiter can be caused by Thyroid Cancer
Outside of the United States, a goiter occurs most often because of a lack of iodine, which is the substance the gland uses to make thyroid hormone. The thyroid swells because it is being stimulated to produce more thyroid hormone by the pituitary gland, which releases thyroid stimulating hormone (TSH) into the bloodstream.
In the U.S. iodine has been added to table salt so that, today, a goiter caused from a lack of iodine is a rare occurrence. Signs and Symptoms

ONE heartrending disorder is iodine deficiency, simply because it can so easily be prevented.
Unlike many large-scale health initiatives, micronutrient deficiencies (such as lack of iodine) do not require huge capital and personnel investments.
DOST statistics reveal a dismaying picture. In 1998, the prevalence of iodine deficiency among children was 35 percent. In Northern Mindanao 32 percent are victims of severe deficiency, with Bukidnon alone having a prevalence of 71 percent! The Food and Nutrition Research Institute (FNRI) explains: "Iodine Deficiency Disorder (IDD) is due to inadequate intake of iodine either because the soils and water are iodine deficient or because certain naturally existing 'goitrogens' in foods interfere with the utilization of iodine. Iodine is essential for the formation of thyroid hormones that are necessary for growth and development and for proper metabolic function." The recommended daily intake of iodine is 150-300 m g/L. When the thyroid gland does not obtain enough iodine to make these hormones, it increases in size to compensate for its deficiency. This enlarged thyroid gland is called goiter.

Description
The thyroid gland sits astride the trachea (windpipe) and is shaped like a butterfly. It makes thyroxin, a hormone that regulates the metabolic activity of the body, rather like the gas pedal on a car. Too much thyroxin increases the metabolism, causing weight loss, temperature elevation, nervousness, and irritability. Too little thyroxin slows the metabolism down, deepens the voice, causes weight gain and water retention, and retards growth and mental development in children. Both conditions also alter hair and skin growth, bowel function, and menstrual flow. Curiously, the thyroid gland is often enlarged whether it is making too much hormone, too little, or sometimes even when it is functioning normally. The thyroid is controlled by the pituitary gland, which secretes thyroid stimulating hormone (TSH) in response to the amount of thyroxin it finds in the blood. TSH increases the amount of thyroxin secreted by the thyroid and also causes the thyroid gland to grow.
  • Hyperthyroid goiterIf the amount of stimulating hormone is excessive, the thyroid will both enlarge and secrete too much thyroxin. The resulthyperthyroidism with a goiter. Graves' disease is the most common form of this disorder.
