I have been to my family doctor for further meds. Upon reviewing the side effects of the meds not to mention the cost, I would like to find a natural herbal treatment, if that is possible. I do take vitamins like a multi, b complex b 6 and 12 e 800 iu, c 1K , folic acid etc.

With gout, a type of arthritis, tiny mineral, or urate, crystals collect in certain joints, causing an intense inflammatory reaction. The crystals are composed of uric acid, a metabolic waste product normally excreted in the urine. Gout is more common in males and the big toe is its most common site, although it may also affect other foot joints as well as the fingers, ankles, elbows, wrists and knees. During an attack, the joint/s becomes painfull, red, swollen and hot to the touch. When gout goes untreated, attacks become increasingly frequent and eventually damage the affected joints and sometimes the kidneys and other internal organs. In addition, deposits of urate crystals, called "Tophi", may occur under the skin near affected joints, over the elbow or on the outside edge of the ear. These may eventually break through the skin and become infected. "OTHER CAUSES OF JOINT PAIN" Pseudogout is quite similar to gout, except the attacks are usually less severe and the crystals are composed of Calcium Pyrophosphate dihydrate. Joint pain may come from another form of Arthritis, such as Osteoarthritis, rheumatoid arthritis or Lupus. Bunions and other foot disorders produce pain and toe deformity.

WHAT IS GOUT?
Gout is a type of arthritis that causes sudden, severe attacks of pain, swelling, redness, warmth and tenderness in the joints. It usually affects the joint of the big toe but can occur in feet, ankles, knees, hands and wrists. Gout occurs when a substance called uric acid builds up in the body and forms needle-like crystals in the joints. The first symptoms of gout often occur in the middle of the night or upon rising in the morning. Wearing shoes and moving the joint or standing may be difficult and painful. Gout accounts for about 5 percent of all cases of arthritis.
WHAT CAUSES GOUT?
Gout is caused by a buildup of uric acid that occurs when the body has difficulty breaking down protein substances called purines which are found naturally in foods. Sometimes this happens because the kidneys are not getting rid of uric acid properly, and sometimes it occurs because the body produces too much uric acid. Eating too much of certain foods such as salmon, liver, herring or sardines and drinking too much alcohol may trigger an episode of gout.

9.10 Acute Gouty Arthritis
Presentation
Usually a male patient over 50 years old with an established diagnosis of gout or hyperuricemia rapidly develops an intensely painful monarticular arthritis, often in the middle of the night, but sometimes a few hours following a minor trauma. Any joint may be affected, but most common is the metatarsophalangeal joint of the great toe (podagra). The joint is red, hot, swollen, and intensely tender to touch or movement. There is usually no fever, rash, or other sign of systemic illness. The patient may have predisposing factors that increase his risk of developing gout, like obesity, moderate to heavy alcohol intake, high blood pressure, diabetes and abnormal kidney function, or he may be taking certan drugs including thiazide diuretics, low-dose aspirin, and tuberculosis medications (pyrazinamide and ethambutol).
What to do:
  • If the patient has not been previously diagnosed by arthocentesis that showed crystals, then tap the involved joint as described in acute monarticular arthritis . In addition to ruling out infection, look under the microscope for crystals in the joint fluid. Urate crystals look like needles, and may be in white cells. Polarizing filters above and below the sample can help disclose and differentiate crystals.

Gout
Gout is a condition that most commonly causes a painful, red, swollen, hot joint or joints. It is caused by such high levels of a natural chemical (uric acid) in the blood (hyperuricaemia) that crystals begin to form.
Symptoms
Attacks may last a few days to a few weeks and usually there are long periods between attacks. Mostly, one joint becomes painful, red hot and swollen (inflamed) over a very short time. Most often the first joint to be affected is the big toe, just like the popular image of the person with gout, but in up to a third of people the attack starts elsewhere. Sometimes more than one joint, or soft tissues (muscles, tendons, tissues below the skin) may be affected by inflammation. Sometimes hard, yellowish lumps are visible close beneath the skin (tophi). Crystals may cause troubles elsewhere, sometimes leading to stones in the kidneys.
Causes
Uric acid is a natural substance, produced by the body as a by-product of the breakdown of old body cells, and from foods we eat. Most of it leaves the body via the kidneys, in the urine. Some is passed out in the bile.
Idiopathic gout
Most times the cause is inbuilt and not a direct result of drinking or eating the wrong things. Where no other cause is identified it is known as idiopathic gout. About 20% of people with idiopathic gout have relatives who have hyperuricaemia or gout. The inherited problem may be one of over-production or reduced elimination of uric acid via the kidneys.

Gout SYMPTOMS The main symptom is waking up in the middle of the night with an acute throbbing pain in the big toe, which is swollen. Usually only one of the big toes is affected. The pain lasts for around three or four hours and will then subside and usually not return for a few months. It can be controlled by drugs, which your GP will be able to prescribe. The application of ice or cooling lotions will help during an acute phase. CHIROPODY CARE All three forms of arthritis can benefit from chiropody care.

The uric acid crystallizes and settles in the joints in the body, most often in the big-toe joint. When it does so, it produces an excruciating pain. (Gout is often confused with Osteoarthritis, a condition that is characterized by a much slower onset of pain, and seems worse after a person has been walking or running.)
Gout is generally thought to be a hereditary disease, but there are many causes: increased purine intake (*see diet ), sudden weight loss due to dieting, and/or diuretics can cause elevated uric acid and mimic the symptoms of gout. In rare cases, leukemia's and/or blood dyscrasias can cause elevated uric acid. Gout can be a serious disease and needs medical intervention, generally by an Internist. If gout is not treated properly, it will eventually cause permanent changes in the big-toe joint. It is important to seek medical attention if you suspect the existence of gout. It is encouraging to note that drugs can be prescribed that will control the uric-acid level in the blood. You need not suffer from gout in this day and age.

C Although hyperuricemia predisposes patients to gout and nephrolithiasis, it does not need to be treated in the asymptomatic patient. However, efforts should be made to modify or correct underlying causes. Epidemiology Gouty arthritis is the most common form of inflammatory joint disease in men older than 40 years. The National Health Survey (1983 to 1985) determined the prevalence rate of self-reported gout to be 13.6 cases per 1,000 men and 6.4 cases per 1,000 women. These numbers reflect an approximate threefold increase in the prevalence of gout since 1969. Gout is pain and inflammation in a joint caused by accumulation of crystals. In patients with gout, a substance called uric acid builds up, because either the body produces too much or the kidneys do not eliminate it quickly enough. The crystals are made up of this substance. Will my doctor know I have gout just from my symptoms?
A high level of uric acid in the blood along with typical symptoms may provide the answer. To be certain that the problem is gout and not infection or another cause, your doctor may draw a little fluid from the joint and test it for uric acid crystals. What is the treatment for gout?
Everyone who has had an attack of gout agrees that stopping the pain is the first order of business. A nonsteroidal anti-inflammatory drug often helps. Colchicine, a drug especially for gout, usually gives relief, particularly if taken early in the attack, but it may have side effects. Patients who cannot take these drugs may be helped by an injection of corticosteroid into the joint. Treatment after the first attack varies. Some fortunate people never have another attack. In others, attacks recur and may involve additional joints. Sometimes, mild inflammation continues for months and can slowly damage joints without causing painful attacks. In some cases, over time, chalky deposits (called tophi) may cause nodules around the joints. These nodules may have to be treated with drugs or removed surgically.

Foot Care Products Description Gout is caused by abnormal metabolism of substances called purines that result in the accumulation of uric acid in the blood stream. Purines are a by-product of cell break down. When the excretion of the uric acid is hampered the accumulated uric acid in the blood stream causes crystalline deposits to form in joints or in the soft tissues. When this happens, there is a sudden onset of extreme pain with associated swelling, redness, and increased warmth to the skin or joint. Classic gout occurs in the big toe joint. It also commonly occurs in the knee joint. Rarely is it seen in more than one joint at a time. Uric acid accumulation in other joints and areas of soft tissue is less common. When gout presents in these areas it, may not be recognized as gout by the treating doctor. Diagnosis As the crystalline deposits form in the joints and soft tissue, the uric acid levels in the blood stream can return to normal. Blood tests taken during an attack of gout may demonstrate a normal uric acid level. This can make diagnosis more difficult, and the physician must rely on his or her clinical experience to make the diagnosis. Other areas that gout may present itself are the tops of the foot, the heel and the ankle joint. In the chronic form of the disease, called tophaceous gout, the repeated deposition of uric acid will from nodules about the joints and tendons. These nodules can spontaneously open and drain a chalky like substance. An attack of gout can resemble an infection. An elevated temperature may also be present. This is worrisome to the physician because an infection in a joint can be a very damaging event. Some doctors may wish to take a sample from the joint so that it can be analyzed for gout and cultured for bacteria.
