I am excited to have the opportunity to present some outstanding web sites concerning this disease on this page. They have given me insight and knowledge into this awful affliction. I salute all those who have and are currently grappling with this illness. Hopefully all of the information that I have included below will be helpful to anyone who is interested. What is Graves Disease? T he Mayo Clinic is one of the hospitals/clinics leading the way in terms of diagnosing and treating Graves Disease. This page is easy to understand and offers a great, albeit simple, summary of the Disease. If one scrolls down the screen, there is a good section on the thyroid gland (one of the major players in Graves Disease). This page also offers info on symptoms such as: weight loss: irritability; changes in menstrual patterns; and an irregular heartbeat. There is a brief section on how Graves Disease affects the skin and eyes, and portion devoted to corrective procedures. The Thyroid Foundation of America T he Thyroid Foundation of America's (TFA) page on Graves Disease is interesting. The page give a brief description of the history of Graves Disease, which was something that I didn't find on any other web site. The TFA also has a section dedicated to the discussion of who gets Graves Disease and reasons why. There is a slightly more detailed list of symptoms, as well as a section pertaining to the diagnosis of Graves Disease.

In a normal person the Thyroid system behaves as in the diagram on the left, Thyroid Stimulating Hormone (TSH) is released by the Pituitary gland into the blood stream. When this Hormone comes into contact with the a Thyroid gland cell, it attaches to a TSH receptor on the surface of the cell, which sends signals into the cell to begin releasing Thyroid hormones. The Thyroid hormones will then form a negative feedback system, ie. when the Thyroid hormones are at a high concentration it inhibits the pituitary glands production of TSH. If the concentration of Thyroid Hormone is low then the pituitary gland will release TSH, and increase the levels of Thyroid hormone as a result.
In a sufferer of Grave's disease the System works the same, except an Autoimmune Antibody acts like a TSH molecule. The Antibody attaches to the Cell, and stimulates the production of Thyroid Hormones as if the Antibody was TSH itself. However, because the production of Autoimmune Antibody is independent of the Pituitary gland the -ve feedback system has no effect. Antibody will continue to be produced, and Thyroid hormones will be continually released, causing physiological damage.

This means that the body's own immune system produces antibodies that mistakenly "attack" the thyroid gland as if it were a virus. In the case of Graves' Disease, this forces the thyroid to over produce thyroid hormones that control metabolism. These hormones speed up or slow down the body's functions as needed. The over production of thyroid hormones can have serious consequences, depending upon the severity of the disease when diagnosed and the patient's age. Some of the symptoms include accelerated heart rate (which can be very serious in the older patient), insomnia, increased appetite, weight loss, mood swings, and irritability. No one knows what precisely causes Graves' Disease. It is hereditary and occurs mostly in women. It is believed that a stressful or life changing event, like the death of a spouse, can cause a genetically pre-disposed person to develop Graves Disease, but not all patients experience a stressful event. The good news is that Graves' Disease is easily treated. There are anti-thyroid drugs that can control the disease by inhibiting the production of thyroid hormones. Unfortunately, the illness often returns once the medication is discontinued. The most popular form of treatment is to destroy all or part of the thyroid gland with radioactive iodine. Since the radioactive iodine is absorbed only by the thyroid and is passed out of the body within days, this treatment is considered safe. Most patients develop hypothyroidism (shortage of thyroid hormones) within months, and must take hormone replacements (Synthroid) for the rest of their life. The third treatment option is to surgically remove part of the gland. This is not a popular treatment because radioactive iodine is considered to be safer and more efficient. In thyroid surgery there is a risk of damage to the other organs in the neck.

Graves' disease or diffuse toxic goiter is the most common form of hyperthyroidism. 90% of people who are hyperthyroid have Graves' disease. Diffuse means the entire gland is sick, not just part of it. Although the gland weighs in at only an ounce or so, it produces hormones that regulate your metabolism, digestion, heart beat, body temperature, and the smooth working of your muscles not to mention your energy levels. This little gland controls over 500 functions in the body. The energy use of virtually every cell and organ in the body is regulated by this small gland in the neck. The brain's pituitary gland (the body's master gland) produces a thyroid stimulating hormone (TSH) which causes the thyroid gland to produce thyroxine (T4) and triiodothyronine (T3), the two vital hormones that regulate all of the afore-mentioned functions. The pituitary gland will secrete increased amounts of TSH only when the T4 and/or T3 levels are low. The number after the T indicates the number of atoms of iodine, which that specific hormone contains. T4 has four iodine atoms, etc. T3 is the most potent thyroid hormone and is required through out the body for cell metabolism. T4 must be converted to T3 in order to interact in the cells metabolically. But T3 has an extremely short life span so the thyroid produces significantly more T4 than it does T3.

Radiotherapy Does Not Help Graves' Disease Eye Problems
For years, radiation therapy to the eye has been used to treat eye problems associated with the thyroid condition Graves' disease. Now a new study questions whether this treatment is actually useful Graves' disease triggers an overproduction of hormones from the body's thyroid gland, a key regulator of metabolism and other vital functions. A small percentage of people with the disorder have a complication called Graves' ophthalmopathy, which is characterized by bulging eyes, double vision and other eye problems. To investigate the effectiveness of radiotherapy for these eye complications, investigators studied 42 patients with Graves' ophthalmopathy. The patients exhibited a variety of symptoms including higher-than-normal volumes of eye muscle and fat, bulging of the eyes, less eye range of motion and double vision. The patients received radiotherapy in one eye and a "sham" treatment in the other eye.

Introduction
Hyperthyroidism, also called thyrotoxicosis, results when the body's tissues are exposed to excessive levels of thyroid hormone. This condition can be caused by several different disorders. Graves' Disease (GD) is the most common form of hyperthyroidism in persons who are not taking thyroid hormones.
Graves' Disease is an autoimmune disorder, meaning that there is a defect in the immune system, causing production of immunoglobulins (antibodies) which stimulate and attack the thyroid gland, causing growth of the gland and overproduction of thyroid hormone. Antibodies also attack tissues in the eye muscles and in the skin on the front of the lower legs (tibial areas).
The choice of treatment modality is based on symptoms, individual preference, and the experience of the health care provider. Many of the treatment modalities for Graves' Disease render the thyroid gland non-functional and lifetime replacement of thyroid hormone by pill is required.

For more than 150 years, conditions known as Graves Disease and chronic autoimmune thyroditits have been associated eye problems (ophthalmopathy). In 1825, Caleb Parry described a condition of goitre and eye protrusion. In 1835, Robert Graves wrote about cases of thyroid disease and proptosis (protrusion) of the eye, and this form of hyper-thyroidism was named after him. In 1840, Carl von Basedow noted swelling of tissues around the eye in patients with hyperthyroidism. Although considerable research is being done, it is not yet known why the eye is involved in thyroid conditions.
Pathogenesis (cause)
Some ophthalmologists think eye changes result from either an autoimmune abnormality or a regulatory cell disorder that allows some antibodies to attack the ocular muscles, which leads to inflammation and other signs of discomfort.
Grave's ophthalmopathy focuses on the muscles around the eye, which help position the eye within the bony orbit or cavity where it rests on a bed of fat. This fat and the tear gland can also be affected in Grave's Disease although to a lesser degree. More often, apical enlargement of the eye muscles stimulates other fibroblasts. Fluid is drawn into the muscle and stored, eventually leading to the development of scar tissue. In addition, the swelling of the muscles pushes the eye forward causing it to protrude. As a result patients may have trouble closing their eyes, which causes drying, irritation and increased inflammation. Increased effort to bring the eyelids together may result in a frown, or swollen muscles may compress the optic nerve leading to loss of vision if treatment is not sought early on.

Graves' disease occurs in less than one-quarter of 1 percent of the population, and every year about a half a million new cases are diagnosed. It is four to eight times more common in women than men and typically begins between the ages of 20 and 40, although it can occur in children and the elderly. While the exact cause of Graves' disease is not known, a genetic disposition to autoimmune disorders seems to be needed in order to develop the disease. Other factors that contribute to the development of Graves' disease include heredity, one's immune system, age, sex hormones and stress. Many thyroid specialists believe that Graves' disease may have its onset after an external stressor (such as death in the family), a viral infection or pregnancy. Although Graves' disease is not contagious, it has been known to occur coincidentally with husbands and wives. Symptoms
Like other thyroid problems, Graves' disease usually begins slowly. Symptoms include fatigue, weight loss, restlessness, rapid heartbeat, muscle weakness (especially involving your upper arms and thighs), heat intolerance, tremors, enlarged thyroid gland, heart palpitations, increased sweating, nervousness and irritability, thinning hair, increased appetite, lighter menstrual periods and more frequent bowel movements. Graves' disease can also affect the eyes, usually an elevation of the upper eyelids, making the eyes appear larger. About 5 percent of patients, however, experience more serious problems in which the tissue behind the eyes becomes inflamed and swollen, making the eyes protrude. A few patients have blurred or double vision.

Home Patient Resources Thyroid Information Graves' Disease Graves' disease is the type of hyperthyroidism caused by a generalized overactivity of the entire thyroid gland. It is also called "diffuse toxic goiter": "diffuse" because the entire thyroid gland is involved in the disease process; "toxic" because the patient appears hot and flushed, as if feverish due to an infection; and "goiter" because the thyroid gland enlarges in this condition. It is often referred to as Graves' disease in honor of the Irish physician Robert Graves who was the first to describe this form of hyperthyroidism. An Autoimmune Disorder Current research shows that the process which triggers Graves' disease involves the immune system of the body, which normally protects us from foreign invaders such as bacteria and viruses, as well as from abnormal body cells such as cancer cells. The immune system recognizes foreign invaders or abnormal cells and destroys them by means of antibodies produced by blood cells known as lymphocytes.
It is likely that 10 to 15 percent of people inherit an immune system with a problem. Their lymphocytes have the capacity to make antibodies against their own tissues which stimulate or damage those cells. In Graves' disease, antibodies are produced against certain proteins on the surface of thyroid cells, stimulating those cells to overproduce thyroid hormones. This results in an overactive thyroid.

Graves' Disease Graves' disease is the most common cause of hyperthyroidism in the United States. Other causes of hyperthyroidism include multinodular goiter, toxic solitary nodules of the thyroid, and functioning thyroid cancer. Other rare causes for hyperthyroidism include TSH-secreting pituitary tumors and hCG-secreting trophoblastic tumors of the ovary or testis. Struma ovarii is another rare cause of hyperthyroidism. All of the latter conditions cause increased thyroid radioactive iodine uptake on scanning. Additional causes of hyperthyroidism include subacute thyroiditis and certain phases of Hashimoto's thyroiditis as well as exogenous thyroid hormone intake and iodide-induced hyperthyroidism (due to kelp, amiodarone x-ray contrast). Symptoms of Graves' disease are those of hyperthyroidism and may include anxiety, restlessness, irritability, emotional instability, fatigue, muscle weakness or cramps, increased appetite, palpitations and heat intolerance. Patients may also have a fast heart beat, atrial fibrillation, tremor, an eye stare and changes in hair. Osteoporosis tends to occur if hyperthyroidism left untreated. There may also be increased sweating and increased numbers of bowel movements, shortness of breath on exertion and weight changes. Usually there is weight loss, but occasional patients gain weight ironically due to an increased appetite. Periodic paralysis with low potassium may occur in men who are of Asian, Filipino, or Native American ethnic backgrounds. Menstrual irregularities and cardiac angina are also common.
