Skip Menu Communicable Disease Report Current Status in Hawai`i The countries with the highest prevalence rates in the world are neighbors of Hawai`i, the Federated States of Micronesia (FSM) at 35/10,000 and the Republic of Marshall Islands (RMI) at 15/10,000. Therefore, it is not surprising that the number of new patients in Hawai`i originating from FSM and RMI have increased. The majority of newly diagnosed patients in Hawai`i during the past 20 years were born in the Philippines, but as the prevalence rate in the Philippines nears the target at 1.26 per 10,000, these numbers are decreasing. Global Reduction in Prevalence Rates The remarkable improvements in world prevalence rates have been accomplished through the use of multi-drug therapy (rifampin, dapsone, clofasamine) administered through very well managed public health programs. The medicines have been donated by the Sasakawa Memorial Health Foundation given through WHO to almost all of the 55 endemic countries. The Pacific island countries of FSM and RMI became a part of the WHO sponsored multi-drug programs later than most countries and have not yet received the benefits of well focused public health programs. Thus new patients will continue to be diagnosed in Hawai`i for many years as people from these countries enter Hawai`i for reasons of employment, education, and relocation. Effectiveness of Multi-Drug Therapy Continuing Concerns Once the person has completed a full course of treatment, the person is removed from the registry and is no longer considered a patient in terms of calculating the prevalence rate. They may, however, have persisting disabilities.

Back to top Back to top Back to top FREE SITES AND ATTRACTIONS ON O'AHU Arizona Memorial Kamehameha Hwy. 99, via H-1 west, exit 15A, Honolulu - (808) 422-2771 The USS Arizona was completely sunken during the bombing of Pearl Harbor, taking over 1,000 sailors to their death in the harbor. This boat and that event are essentially what entered the United States full-scale into World War II. The USS Arizona Visitor Center is located on the Pearl Harbor Naval Base adjacent to the sunken remains of the USS Arizona. The center is operated and maintained by the National Park Service in a use agreement with the US Navy. The center and USS Arizona Memorial are free of charge to the public and there are no reservations. Everyone visiting the USS Arizona is encouraged to view a 20-minute film documenting the attack on Pearl Harbor. The film is presented by the Park Service, prior to the short US Navy shuttle boat trip to the memorial itself. Be prepared to wait however. The memorial is a very popular site in Honolulu and the crowds at all hours of the day can be overwhelming. Visitors are issued free tickets for the film and the shuttle to the Arizona upon arrival at the center on a first come, first served basis, but waits of one hour or more are commonplace. For this reason, go early! The center is open from 7:30am to 5:00pm but the last program of the day and tour starts at 3:00pm The visitor center is closed Thanksgiving, Christmas and New Year's Day.

By Our Special Correspondent MADURAI, APRIL 1. A vaccine for leprosy will become a reality if the ongoing research reaches its logical conclusion in the near future. A federal- funded research on leprosy has started identifying a large number of ``potential candidates'' to become eligible as a vaccine for one of the most dreaded diseases in the world. Talking to The Hindu here, Dr. Thomas P. Gillis of the National Hansens' Disease Centre, USA, who has undertaken this pioneering research, said TB and leprosy were like ``cousins'' as some of their compositions and genes were similar. It had also been established that the vaccine BCG, given for newborns, had an effect on leprosy also. At the moment, Dr. Gillis said, they had started developing DNA micro arrays to find out the major strain differences in the pathogen, Mycobacterium leprae, that causes the disease. The BCG vaccine had been found to have an effect on leprosy in countries like India and Malawi. Though the research on formulating a vaccine for leprosy was at a ``very early stage,'' he was confident that a result could be achieved. But still, the logistic problems would have to be confronted in tune with the transmission patterns as sub-typing the organism had been found to be difficult. Basically, the researchers at the National Hansens' Disease Centre had started developing DNA micro arrays in order to find out the major strain differences. Though improvement of basic hygiene would be the best way to eradicate leprosy, Dr. Gillis said a vaccine would help to further improve the picture. At the moment, three groups were working all over the world on formulating a leprosy vaccine and research was on to find out whether the BCG vaccine could be used against leprosy also by ``adding a gene.''

ANAND INTRODUCTION It was casually noticed that compared to other districts in Kerala, a relatively large number of patients in the neuro medicine OPD in Alappuzha district having symptoms and signs of carpel tunnel syndrome and lateral popliteal nerve palsy. So we questioned these patients in detail and found that most of them were coir workers, doing coir work from a very early age of 10 or 12 years. They have a peculiar sitting posture with one leg flexed and other extended and keeps rolling the coconut fibre in between their palms for hours together. Coir work involves the following: (a) mechanical compression on nerves produced by the posture and nature of work (b) exposure to dyes and chemicals. MATERIALS AND METHODS 61 individuals – 122 upper limbs and 122 lower limbs in the age group of 17 years to 60 years and doing coir work for varying periods of time from 6 years to 30 years were taken up for study.
