Heart failure is an important cause of illness and death in dogs and cats. Congestive heart failure is the most common form of heart failure in small animals. Dogs usually get heart failure from mitral valve regurgitation. The mitral valve is the valve that separates the left atrium from the left ventricle. Regurgitation means that blood moves backwards from the ventricle to the atrium when the ventricle is contracting, instead of flowing out into the aorta and then the body. Mitral valve disease usually occurs from chronic scarring of the valve (often due to bacterial infection), or from dilated cardiomyopathy, where the heart muscle becomes distended and incapable of properly transmitting the electrical current of the heart beat. Pregnant woman's remark Pregnancy has many wonderful moments, but stepping on a scale during prenatal visits usually isn't one of them. Even though pregnant women are supposed to gain weight, cultural biases against weight gain often extend to pregnant women. Concerns about gaining weight can lead health care providers and women to unduly restrict it. Weight is such a loaded subject in our culture that the progress of weight gain is usually closely monitored and managed in pregnancy. But this is not the case in cultures where less importance is placed on women's weight. Without the preoccupation of maintaining a thin appearance, women are free to follow the body's hunger and fullness cues. In this situation, and food is available, women tend to gain an average of 32 pounds in pregnancy. Weight gain during pregnancy in such cultures is usually not intensely scrutinized because women do fine on their own. However, this approach doesn't work as well for many women in highly weight-conscious societies such as ours because food intake may be driven by factors other than the body's hunger and fullness signals. If inborn systems for regulating food intake have been overridden by other motivations for eating or not eating, then it may be necessary to pay close attention to the progress of weight gain in pregnancy. The potential benefits to fetal development and growth, and the baby's future health, make it worthwhile to make sure the right amount of weight is gained during pregnancy.

LOUIS The concept is simple: to halt deterioration of the heart muscle, wrap it in a mesh-like net to prevent further enlargement in patients with heart failure. It's like a jacket for the heart. "This is an incredibly novel approach," says Paul Hauptman, M.D., a SLUCare cardiologist and associate professor at Saint Louis University School of Medicine. "It's a potentially revolutionary approach." Heart failure is a condition in which the heart can't circulate enough blood to meet the needs of the body. Patients who have the problem typically have enlarged hearts that continue to get larger and weaker as the condition progresses. The CorCap Cardiact Support Device is an investigational, mesh-like heart "jacket" that is slipped around the heart during chest surgery and stitched in place to prevent any further enlargement. It is made from a custom knitted polyester fabric similar to material found in other devices permanently implanted in the body. It is intended to be an adjunctive therapy for patients with moderate to severe heart failure. Preliminary testing has shown the CorCap is strong enough to prevent the heart from becoming larger, stretching just enough so that it doesn't interfere with the heart's normal filling and emptying. Saint Louis University School of Medicine is one of 20 sites nationwide selected to test this heart jacket, which is intended to halt the progression of heart failure, reduce heart size and improve cardiac function. Hauptman is leading the school's clinical trial in this area.

DiNubile, MD THE PHYSICIAN AND SPORTSMEDICINE - VOL 26 - NO. 8 - AUGUST 98 In Brief: Congestive heart failure (CHF) patients who exercise regularly can increase exercise tolerance, decrease dyspnea and fatigue, reduce the risk of arrhythmias, and improve quality of life. Initial stress testing is important for assessing a patient's physiologic response to exercise and for establishing a target heart rate for an individualized exercise program. Patients may need to begin aerobic exercise with interval training and progress slowly. Strength exercise is also useful. Exercise used along with the customarily prescribed CHF medications improves symptoms more than either modality alone. T he incidence of congestive heart failure (CHF) and the resulting death rate (1) have risen significantly in recent decades, partially because of the aging of the population. Over 4 million Americans now suffer from the disorder, which, broadly defined, is an inability of the heart to pump enough blood to meet the body's metabolic needs. The causes are many, but the vast majority of cases involve left ventricular dysfunction related to coronary artery disease, hypertension, or idiopathic dilated cardiomyopathy (2). CHF patients were traditionally counseled to rest and avoid exercise in order to delay disease progression and to promote diuresis induced by bed rest (3). However, CHF theory and clinical practice have changed radically, especially in the last decade, as research has demonstrated that exercise offers much gain at little risk (3-9). Indeed, most CHF patients who exercise can markedly improve their functional status and quality of life (3,5-8). In addition, exercise may reduce the risk of death for CHF patients, just as it does for patients who have coronary artery disease (10).
