A heart murmur is an abnormal, extra sound during the heartbeat cycle made by blood moving through the heart and its valves. It is detected by the physician's examination using a stethoscope. Description
A heart which is beating normal makes two sounds, "lubb" when the valves between the atria and ventricles close, and "dupp" when the valves between the ventricles and the major arteries close. A heart murmur is a series of vibratory sounds made by turbulent blood flow. The sounds are longer than normal heart sounds and can be heard between the normal sounds of the heart. Heart murmurs are common in children and can also result from heart or valve defects. Nearly two thirds of heart murmurs in children are produced by a normal heart and are harmless. This type of heart murmur is usually called an "innocent" heart murmur. It can also be called "functional" or "physiologic." Innocent heart murmurs are usually very faint, intermittent, and occur in a small area of the chest. Pathologic heart murmurs may indicate the presence of a serious heart defect. One way of thinking about innocent murmurs is to consider the vascular system as a series of tubes and compartments that are joined to each other. At these junctions there can be a normal degree of turbulence as the blood traverses from one tube or chamber to the next. This normal turbulence can set the surrounding tissues into vibration and this vibration is transmitted out through the chest and to the stethoscope.
In the first two months of age it is common to have a mild degree of turbulence at the branch point from the main pulmonary artery to each of the branched pulmonary arteries. This innocent murmur of mild branched pulmonary artery turbulence (also termed mild peripheral pulmonary artery turbulence) has a medium pitch and radiates though out the precordium with specific radiation to the axillae. The blood then traverses the lungs and returns to the left atrium and left ventricle. As the ventricle squeezes blood is ejected though the left ventricular out flow tract past the aortic valve to the aorta.
The flow of blood within the left ventricle may set in vibration support tissue within the left ventricle (left ventricular muscle cords) or the support structures to the mitral valve near the left ventricular out flow tract such as the cordae and papillary muscle attachments to the left ventricle. This causes a soft grade 2/6 crescendo-decrescendo (ejection) type murmur with a medium pitch (relatively pure uniform frequency of 70- 130 cycles per second) and a characteristic musical vibratory quality "like plucking a taught piece of string under water". This is the "Still's-Murmur (George F. Still 1909) " and is frequently also termed benign ventricular ejection type murmur.

Parents are sometimes shocked to hear the doctor say, "I hear a little heart murmur," while he examines their child. The pronouncement sounds so ominous; I don't think any parent can recieve this news with anything but some trepidation. However, it is actually rare that they have anything to fear, as the discussion of functional heart murmurs makes plain. I have included in the /EncyMaster/clopedia very brief discussions of some of the more common specific heart lesions which cause these murmurs. I enter this discussion with some trepidation, because I am not a cardiologist and there are so many different cardiac lesions possible. This discussion is intended to be very baseline type information only. I will discuss a few of the more common ones briefly in the /EncyMaster/clopedia, but I leave full discussion of them to your pediatric cardiologist. With the availability of in-office cardiac ultrasonography nowadays, when parents are referred to a pediatric cardiologist they usually leave the first visit with a definitive diagnosis and prognosis and counsel about what to expect. The discussions here are actually aimed at the idly curious or concerned friends or relatives of parents whose child has been diagnosed with a specific cardiac problem. First a word or two about the normal heart. Ordinarily, the sound of the heart is a steady "lup-dup, lup-dup, lup-dup." with a rising emphasis on the dup sound. These lup-dup sounds, with which everyone is familiar, are simply the closing thumps of the heart valves as blood is pumped in the never-ending cycle of blood flow, fill-eject-refill, through the heart. The first sound is the sound of the inflow valves that fill the ventricles closing with a thump as the main pumping stroke begins; the second sound is the sound of the main outflow valves to the aorta and pulmonary arteries snapping closed at the end of the pumping stroke. The second sound is normally split, that is the aortic and pulmonic valves can be heard closing separately, since the back pressure on the aortic (to the body) valve is considerably higher than that on the pulmonic (to the lungs) valve. So actually,it is more of a "lup - dudup" rythmn.

My 11-week-old was said to have a flow murmur. Should I be concerned? My 11-week-old was said to have a flow murmur. Should I be concerned? When parents hear that their child has a murmur, they are often quite concerned. Some murmurs do signal serious problems in the heart. Others, however, are no problem at all. When people hear the "lub-dub" that they think is the heart beating, they are really listening to the sounds of the valves in the heart closing. The heartbeat itselfthe contraction of the heart muscle to pump out the bloodis silent. Most of the time, blood flowing through the heart is also silent. When the turbulence of blood flowing through the heart can be heard, the person is said to have a murmur (like the murmuring of a babbling brook). Some murmurs indicate an abnormality in the structure of the valves or walls of the heart, or of the major blood vessels. A flow murmur, however, arises from normal hearts, and represents no problem whatsoever. At least 80 percent of kids have an innocent murmur at some time during childhood. I believe that all kids probably do and that we just don't happen to listen at the right moment in some of them.

By kim louise On 26.11.02 19:36 GMT Edit We took our new puppy to the vets for her first injections and the vet discovered the she had a heart murmur. We are a bit concerned as to wether it will shorten her life span or affect her in her future. At the minute she is eating, very active and growing well. Has anyone been in the same postion and can offer some advice or knowledge about this.

Hi. I am a 47 year old female diagnosed by Echocardiogram with mitral valve heart murmur - soft murmur, 2-3 on Scale of 6, trace mitral valve regurgitation, trace tricuspid valve regurgitation, and a small pericardial effusion anteriorly and apically. Normal chamber sizes, valves appear structurally normal. M-Mode Ejection Fraction 75%.
A 24 hour Holter Monitor for chest pains, palpitations and syncopal episodes showed normal sinus rhythm for the most part with occasional PAC/VPC's with no other disrhythmia or any type of conduction delays or blocks. No ischemic changes noted. 24 hour Holter considered within normal limits. An EEG taken reported as follows: Patient is 47 year old woman with seizures described as mixed seizures. She did have a head injury in an MVA and apparently had a CVA according to paperwork. 18 channel EEG w 10-20 Int'l arrangement. Heart rate 54, mostly regular, except for intermittent additional beats. There was considerable artifact throughout this tracing and it seemed to be somewhat attenuated with respect to voltage and reflected low voltage throughout the majority of the tracing aside from the pin artifact. Background rhythm was 11 in posterior regions. Aside from the artifact there was no clear evidence of epileptogenic fossi. Hyperventillation was not performed. Photic stimulation did not reveal good driving. If clinically warranted a repeat EEG may be obtained.

It is frequently difficult to decide when to get a consultation from a pediatric cardiologist. Depending on the confidence of the physician, some practicing pediatricians refer all murmurs while others refer only a few children. In residency it is important to become proficient in evaluating murmurs so that you can be fairly confident of what to do. Clearcut Indications for Referral to the Pediatric Cardiologist:
  • Any child with signs of heart failure, cyanosis, tachypnea, significant murmurs, questionable PEs in the nursery. All children with Down syndrome, chromosomal abnormalities, multiple anomalies. Any child in whom you diagnose an arrhythmia, Kawasaki, collagen vascular disease (r/o myocarditis or pericarditis). Any child with a persistent murmur that sounds organic.
