H eel spur / plantar fasciitis affects men and women equally. It is a condition in which the bottom of the heel is extremely painful. A heel spur is a bony overgrowth on the bottom of your heel bone. The heel spur is usually a result of an inflamed ligament (plantar fascia) on the bottom of the foot that attaches to the heel bone. Constant abnormal pulling of this ligament irritates the heel bone and the body lays down a bone spur as a protective mechanism. The patient usually complains of pain with the first step in the morning, some relief following activity, but returning after extended amounts of time standing or walking. This condition is a constellation of many causes; overweight, ill fitting shoes, bio-mechanical problems (mal-alignment of the heel), gout, pronation (a complex motion including outward rotation of the heel and inward rotation of the ankle) and rheumatoid arthritis are some of the causes of heel pain. If you have pain in the bottom of your foot and you do not have diabetes or a vascular problem, some of the over-the-counter anti-inflammatory products such as Advil or Ibuprofin are helpful in eradicating the pain. I am an early middle-aged female who is experiencing on going pain from what my doctors have told me is “heel spurs”. Actually, she told me the medical name for it, but I can’t remember what it was. Anyway, I have pain along the sole of my foot and heel, especially during walking. I have tried orthotics for a number of years and they are no longer helping me. My doctor states I have to live with it for the rest of my life. I am asking you for your suggestions. Thank you.

This might be a good time to talk a little bit about “heel spurs,” the condition which is properly known as “plantar fasciitis.” This is an inflammation of the ligamentous structures along the sole of the foot, which causes pain especially with stretching, which occurs unfortunately with just about every step. It can be a very painful and chronic situation. The inflammation is often made worse by overuse, poorly fitting shoes with inadequate arch support, obesity which places an increased load on the foot and other causes.
Physicians diagnose plantar fasciitis, and exclude other causes of pain in the foot, by taking your history, doing a careful physical examination, and taking an x-ray of the foot. The x-ray often shows an osteophyte or exostosis on the heel bone, sometimes called a “bone spur” or “heel spur” hence the popular name.

A heel spur occurs when a calcium deposit forms on the heel bone, causing a bony protrusion. The heel bone is the largest bone in the foot, and absorbs the greatest amount of shock and pressure from walking, running and other activities. The bone is connected to the forefoot by the plantar fascia, a broad band of fibrous tissue located along the bottom surface of the foot. When the plantar fascia is stretched and pulls away from the heel, usually the result of over-pronation (flat feet), calcium deposits can form to fill in the gap, causing heel spurs. People with unusually high arches can develop the problem, and women have a significantly higher incidence of heel spurs than men because of the types of footwear they often wear, including high heels. Dancers or athletes who stretch the plantar fascia on a regular basis are also at risk. According to Dr. Suzanne Belyea, D.P.M., Medical Director of Foot.com, heel spurs themselves do not cause the severe pain. "When people have an over-stretched plantar fascia, it tightens up at night. A heel spur is a bony projection on the sole (plantar) region of the heel bone (also known as the calcaneous). This condition may accompany or result from severe cases of inflammation to the structure called plantar fascia. This associated plantar fascia is a fibrous band of connective tissue on the sole of the foot, extending from the heel to the toes. Description Heel spurs are a common foot problem resulting from excess bone growth on the heel bone. The bone growth is usually located on the underside of the heel bone, extending forward to the toes. One explanation for this excess production of bone is a painful tearing of the plantar fascia connected between the toes and heel. This can result in either a heel spur or an inflammation of the plantar fascia, medically termed plantar fascitis. Because this condition is often correlated to a decrease in the arch of the foot, it is more prevalent after the age of six to eight years, when the arch is fully developed. Causes and symptoms One frequent cause of heel spurs is an abnormal motion and mal-alignment of the foot called pronation. For the foot to function properly, a certain degree of pronation is required. This motion is defined as an inward action of the foot, with dropping of the inside arch as one plants the heel and advances the weight distribution to the toes during walking. When foot pronation becomes extreme from the foot turning in and dropping beyond the normal limit, a condition known as excessive pronation creates a mechanical problem in the foot. In some cases the sole or bottom of the foot flattens and becomes unstable because of this excess pronation, especially during critical times of walking and athletic activities. The portion of the plantar fascia attached into the heel bone or calcaneous begins to stretch and pull away from the heel bone.

A condition in which the bottom of the heel can be extremely painful. A heel spur develops as a bony overgrowth on anterior aspect (forward aspect) of the heel bone (calcaneus). The development of the spur is usually a result of prolonged inflammation of the plantar fascia (a muscle-like ligament) on the bottom of the foot that attaches to the heel bone. Constant abnormal pulling of this ligament irritates the attachment to the heel bone and the body seeks to hold onto the attachment by forming additional bone growth as a protective mechanism.
Symptoms
The patient presents with a dull to sharp pain occurring about the plantar calcaneal area when getting out of bed of a morning. The plantar fascia is more inflexible at those times or when arising from a seated position and continuing during the initial periods of weight bearing. Usually the pain subsides within a few minutes, but may re-occur once this sequence is re-established. The pain is often described by patients as a "stone bruise". Pain can be located at the inferior aspect of the heel either medially, laterally or centrally.

Heel Spurs and Plantar Fasciitis
Heel spurs are the best known heel problems. A heel spur is a point of excess bone growth on the heel. The bone growth usually extends forward towards the toes. Heel spurs are visible on X-ray. Even more common than heel spurs is plantar fasciitis. This common heel problem is actually caused by a painful partial tearing of the plantar fascia connecting the toes and the heel usually at its attachment into the heel bone (calcaneus). This may result in either a heel spur or plantar fasciitis. The term plantar fasciitis is derived from plantar which refers to the bottom of the foot and fascia which is a type of dense fibrous connective tissue. If your foot flattens or becomes unstable during critical times in the walking or running cycle, the attachment of the plantar fascia into your heel bone may begin to stretch and pull away from the heel bone. This will result in pain and possibly swelling. The pain is especially noticeable when you push off with your toes while walking. Since this movement stretches the already inflamed portion of the fascia. Without treatment the pain will usually spread around the heel. The pain is usually centered at a location just in front of the heel toward the arch. When the tearing occurs at the bone itself, a the bone may attempt to heal itself by producing new bone. This results in the development of a heel spur. Without the spur the condition is called plantar fasciitis.

Most commonly the plantar fascia (a broad, ligament-like structure extending from the heel bone to the base of the toes) becomes inflamed and heel pain begins. As this inflammation becomes chronic, a heel spur is likely to form. If heel pain is treated early, conservative therapy is often successful and surgery is usually avoided. Signs and Symptoms of Heel Spurs Early signs of heel pain are usually due to plantar fasciitis , the inflammation of the plantar fascia. It is probably the most common cause of heel pain. It is seen in all groups of people including runners, athletes, weekend athletes, and those who have recently gained weight. The pain most often manifests itself after periods of rest when the plantar fascia is given a chance to shorten. The result is pain felt during the first steps of the day due to sudden strain to the tissue. A heel spur develops when this pain is ignored and the chronic inflammation increases the stress on the fascia. The result is the development of an outgrowth of bone on the bottom of the heel.
