http://familydoctor.org/handouts/467.html
Information
from Your Family Doctor
Microscopic Hematuria What is microscopic hematuria?
"Microscopic" means something is so small that it can only be seen through a special tool called a microscope. "Hematuria" means blood in the urine. So if you have microscopic hematuria, you have red blood cells in your urine, but you can't see the blood when you urinate. What are some common causes of microscopic hematuria? Here are some common causes of blood in the urine:
  • Urinary tract (bladder) infection
  • Swelling in the filtering system of the kidneys (this is called "glomerulonephritis")
  • A stone in your bladder or in a kidney
  • A disease that runs in families, like cystic kidney disease
  • Some medicines
  • A blood disease, like sickle cell anemia
  • A tumor in your urinary tract (this may or may not be cancer)
  • Exercise (this will usually go away in 24 hours)
How will my doctor check for microscopic hematuria? Your doctor will usually start by asking you for a urine sample. He or she will test your urine (urinalysis) for the presence of red blood cells. Your doctor will also check for other things that might explain what is wrong. For example, white blood cells in your urine usually mean that you have an infection. If you have blood in your urine, your doctor will ask you some questions to find out what caused it. If the cause isn't clear, you may have to have more tests. You might have an ultrasound or an intravenous pyelogram (this is like an X-ray). A cytoscope or endoscope may be used to look inside of your bladder. These tests are usually done by a urologist.

    Urology Hematuria What is hematuria?
Hematuria is the presence of red blood cells (RBCs) in the urine. Often, the urine appears normal to the naked eye, but examination under a microscope shows a high number of RBCs. In gross hematuria, the urine is red or the color of cola, which can be seen with the naked eye. What causes hematuria?
Most of the causes are not serious, but, because hematuria may be the result of a tumor, or other serious problem, a physician should be consulted. Many people have hematuria without having any other related problems. How is hematuria diagnosed?
To determine the cause of hematuria, or to rule out certain causes, a series of tests may be ordered. These include, but are not limited to, the following:
    • urinalysis - laboratory examination of urine for various cells and chemicals, such as RBCs, white blood cells, infection, or excessive protein. blood tests - laboratory examination of blood for high levels of waste products.

      Definition
      • Hematuria is a fancy medical term for "blood in the urine".
      Additional Info
Hematuria falls into two general classes:
      • Gross hematuria means that the blood can be seen in the urine with the naked eye. This condition is usually the result of internal bleeding, usually from the kidneys or bladder. Possible causes include accident, trauma or disease. Seek medical attention immediately.
      • Microscopic hematuria is a degree of bleeding so low that the blood can not be seen with the naked eye. Possible causes include kidney or bladder disease or damage. This condition can be detected through laboratory tests.
      MSDS Relevance
Finding blood in your urine is not normal. Seek medical attention if you suspect you have blood in your urine. Exposure to certain chemicals can cause hematuria. Be sure to read your MSDS's and take appropriate precautions.

Overview of this condition Hematuria is a clinical term referring to the presence of blood, specifically red blood cells, in the urine.
Whether this blood is visible only under a microscope or present in quantities sufficient to be seen with the naked eye, hematuria is a sign that something is causing abnormal bleeding in the patient's genitourinary tract. The source of the bleeding can be located anywhere along this tract: the kidneys, the ureters (the tubes running from the kidneys to the bladder), the prostate gland (in men), the bladder or the urethra (the tube that carries urine from the bladder out of the body). The bleeding may happen only once or it may be recurrent. It may or may not be accompanied by pain or discomfort during urination, and it can indicate different problems in men than in women. The cause may be as routine as an infection or as serious as cancer. Whatever the circumstances, hematuria should be regarded as a danger signal demanding immediate attention. Only a thorough physical examination and medical evaluation can interpret its meaning, determine its cause, or provide the patient with facts needed to make informed decisions about its treatment.

Urinalysis is the examination of urine for various cells and chemicals. In addition to finding RBCs, the doctor may find white blood cells that signal a urinary tract infection or casts (groups of cells molded together in the shape of the kidneys' tiny filtering tubes) that signal kidney disease. Excessive protein in the urine also signals poor kidney function.

Microscopic hematuria: more than 5 red blood cells per high power field in the sediment from 10 mL of centrifuged freshly voided urine. Gross hematuria (visible to the naked eye): may originate from the kidney (may contain red blood cell casts, and is usually brown) or from the bladder and urethra (pink urine, may contain clots). Recurrent gross hematuria Recurrent episodes which usually develop 1-2 days after the onset of a viral upper respiratory tract infection. It resolves over 1-2 weeks, but microscopic hematuria may persist. Biopsy is recommended after the second episode. Underlying diseases: IgA nephropathy (Berger nephropathy)
    Idiopathic hematuria
    Familial nephritis (Alport syndrome)
    Idiopathic hypercalciuria
Berger nephropathy: IgA is the predominant immunoglobulin in mesangial deposits. It affects most commonly males than females (2:1); patients present with gross or microscopic hematuria. Progressive disease develops in 30% of patients. The development of hypertension, diminished renal function or proteinuria > 1 g/ 24 h indicate a poor prognosis. Idiopathic hematuria: the diagnosis is established by normal findings on biopsy. Long term follow up is necessary to exclude Alport syndrome. The development of diminished renal function, proteinuria or hypertension demands a second biopsy.

This means the presence of blood in the urine. This could be noticed by the patient when the urine is frankly bloody (Macroscopic hematuria) or it could be microscopic and only detected on urinalysis Not all red urine is indicative of bleeding. Certain situations may be associated with red urine in the absence of actual bleeding. For example certain food dyes may be excreted in the urine and thus change the urine color. Also, some medications may change the urine color.
What are the causes of hematuria?
    Causes of hematuria include:
    • Tumors Trauma Glomerulonephritis Kidney stones Bleeding disorders Infections
What is the usual workup of hematuria?
The physician after a careful history and physical examination will direct the work up to confirm the diagnosis. Laboratory data may include an IMP (Intravenous Pyelography) which may be abnormal in patients with stones or tumors, CT scan of the abdomen and urinary tract, Urinalysis, collection of urine for protein , examination of the urine for malignant cells (Cytology), urine cultures This site is prepared and maintained by Editorial Staff Legal Notice You are visitor number For feedback or to report problems please email to Webmaster Last updated Saturday, September 16, 2000

    Hematuria (VM 9144)
    S. Dru Forrester, DVM, MS, Diplomate ACVIM
Case No. 1 A 2-year-old male Husky mix is presented for evaluation of hematuria that began 2.5 weeks ago. According to the owner, the urine appears discolored throughout urination. Hematuria was confirmed by the referring vet who found a large quantity of red blood cells on sediment examination of a voided urine sample. Treatment with cephelexin and then enrofloxacin has not been associated with any improvement. Five days ago, the urine appeared to be pure blood and some clots were noted; the dog's gums were very pale and capillary refill time was prolonged. The owner has not noticed any straining, pollakiuria, or polyuria/polydipsia and the dog's appetite is normal. The dog lives indoors but is allowed outside. On presentation, physical examination reveals a mildly enlarged prostate gland that is symmetrical and non-painful.
  • The first step in evaluation of hematuria is to determine the source of hemorrhage (ie, upper urinary tract, lower urinary tract, or genital tract). What is most likely the source of hematuria in this dog and why?
List 3-4 potential causes of hematuria in this dog.
